Notes on Major Changes

Introduction:The NFPA 99 1999 revision of Chapter 4 defi-nitely has fewer significant technical changes thanhave many previous editions. This said, the 1999revision is undoubtedly one of the more signifi-cant in the history of the standard. The mostsignificant changes are found not in Chapter 4but in the parts of the standard which determinehow Chapter 4 is applied - in Chapters 12 to 20,the occupancy chapters. In the changes to theseoccupancy chapters, we are seeing the furtherworking out of the “Levels” strategy first hinted atin the 1996 edition, and which will be a corner-stone of the standard for years into the future.(see the discussion on Levels which follows)

Within Chapter 4, a first pass review will revealthat the sections for Level 2 - marked “Reserved”in the 1996 edition now are fleshed out, there areseveral revisions to diagrams, there are some newdiagrams, and there is a remarkable amount oftext marked as changed by the line to the left.(The changed text marked by the lines is notnecessarily changed. Some has merely beenreorganized, and unfortunately there are realchanges which are not indicated. The lines arenot very useful.)

There are two currents which will be noted in thechanges: a continuing effort to make the bookmore “user friendly” offset by a concern that thebook’s user friendly features are being abused,leading to a need to tighten up, especially asregards Figures and Tables.

Changes - General:• Laboratories are more fully included in thescope of the chapter. That is, there are nowfewer cases where laboratories are not covered.(see Definition of Laboratory)

• Waste Anesthetic Gas Disposal now has anofficial name - “Waste Anesthetic Gas Disposal”or “WAGD”. This name will replace “evacua-tion”, “scavenging”, etc. WAGD is now officiallywithin the scope of Chapter 4, ending a longand acrimonious debate on whether it belongsto Chapter 5 as an Environmental System orChapter 4 as a piped medical gas system. (see4-1.1 Scope)

• The recommended Vacuum sizing methodol-ogy which has been in the Appendix since theincorporation of NFPA 56K into the NFPA 99,has been restored and moved into Appendix C.(see C-4.3)

• A new document, the CGA E-10 “Recom-mended Practice for Maintenance of MedicalGas and Vacuum Systems in Healthcare Facili-ties” has been included in the references. (seeB-1.2.6)

Changes - Level 1 Systems:• Where a manifold room opens onto an egresscorridor (see NFPA 101) mechanical ventilationis required. (see 4-3.1.1.2(c))

• A new Figure 4-3.1.1.8(h) has been brokenout of the old Figure 4-3.1.2 to more com-pletely illustrate the Emergency Oxygen Con-nection. (see Figure 4-3.1.1.8(h)

• Figure 4-3.1.1.9 has been dramaticallychanged. This was felt to be one of the mostabused figures in the book, and has beenchanged to reflect the text more exactly, as wellas to guide the user to the appropriate text.(note that some users have reported incorrectparagraph references).

The figure should be used as a graphical tableof contents, to find the relevant text. It is notand never was intended to be used by itself.(see Figure 4-3.1.1.9)

• A point of confusion regarding alarms onmedical air systems has been resolved: Certainoperating alarms are now allowed to be relayedto the master alarms as a “Compressor Fault”signal in lieu of having individual indicators.(see Illustration 1 overleaf)

• Two alternate methods of piping air acces-sory items are explicitly permitted. (see 4-3.1.1.9 (h) and Figures 4-3.1.1.9 (h)1 and 4-3.1.1.9 (h)2

• There is a new Table 4-3.1.2.4 includingthree important pieces of information:

First, it eliminates the need to reference aseparate Compressed Gas Association docu-ment for color coding and symbology.

Second, it provides a listing of “StandardPressures” for reference when considering theparagraphs on “Nonstandard Pressures” (4-3.1.2.11).

Third, it provides a color coding scheme fornon-medical air and non-medical vacuum(practically, these will be found most useful forLevel 3 installations) and for Laboratory Airand Laboratory Vacuum.(see Table 4-3.1.2.4)

• There is no longer any allowance for brazersto be qualified on site - they must qualify in thesame manner and by the same tests in all cases.

• In the 1996 edition, when corrections wereperformed prior to verification, the verifier wasrequired to report these corrections. This is nolonger true. (see 4-3.4.1.1)

Major Changes

• Table 4-3.4.1.4 has been completely re-worked - it is now much more comprehensive.(see Table 4-3.4.1.4)

• Three changes of note have been made in thetesting arena:First: when a failure occurs in the requiredparticulate testing of 25% of all zones, allzones (100%) must then be tested. (see 4-3.4.1.3(e)Second: Some tests previously mandated to beperformed with nitrogen may now be done withsource gas. (see 4-3.4.1.3)Third: A transient flow test has been added foroxygen and air outlets (see 4-3.4.1.3(h)6)

Changes - Level 2 Systems:• This section, marked “Reserved” in the 1996edition has been fleshed out to include a wholeseries of requirements, largely borrowed di-rectly from the equivalent section in Level 1.(see 4-4)See the Section on “A Discussion of Levels” inthis pamphlet for more details.

Changes - Level 3 Systems:• Editorially, the paragraph references in the1996 edition have been replaced with theactual text in this edition.A number of changes have occurred in thisprocess. Among them:- The prohibition on flexible connectors inpatient systems (see 4-3.1.2.9(h) incorpo-rated by reference in 1996 was not included inthe 1999. Interesting enough, this prohibitiondoes exist for Gas Powered Device gas systems.- testing for soft tubing buried in slabs nowfollows the same rules as any other tubing.- flared fittings are permitted only at outletsand manifolds where exposed.- 95-5 solder may no longer be used on GasPowered Device gas systems. (see 4-5.1.3.1(c))- a paragraph “grandfathering” older PipedPatient gas systems has been added. (see 4-5.4.1.1)

About Levels

A Discussion on Levels

Introduction:The NFPA 99 1999 greatly extends the Levelsinitiative NFPA started with the 1996 edition. Thisinitiative began a proposal made for the 1996edition which involved changing the requirementsfor medical gas systems based on the complexity ofthe procedures intended to be performed there.

This proposal was not incorporated into the 1996edition due to it’s sweeping implications and thewide effect it would have in other areas of thestandard. However, pieces of it did enter the codeand can be seen in the exceptions found in Chapters12-19, for example in NFPA 99 1996 13-3.4.1

“If installed, patient gas systems shall conform to Level1 gas systems of Chapter 4.

A single alarm panel, as described in 4-4.3.1.2.1(b)2shall be mounted in an area of continuous surveillancewhile the facility is in operation.

Exception No. 1: Medical air compressors shall bepermitted to be simplex.

Exception No. 2: Dryers, after-coolers, filters, andregulators, as listed in 4-3.1.1.9 (g) shall be permittedto be simplex.

Exception No. 3: One alarm panel that complies with 4-3.1.2.1(b)3a,b,c and d and with 4-3.1.2.1(c)2 and5 shall be permitted.

Exception No. 4: Pressure switches shall be mounted atthe source with a pressure gauge or readout located atthe master alarm panel.”

These exceptions in the 1996 standard all derivefrom the original proposal for “Level 2”.

The underlying concept of less complex systems forless dependant patient populations continued todrive the thinking as the 1999 was in preparation.The 1999 reflects the next iteration in this process.Thus the sections on Level 2 no longer say simply“Reserved” as placeholders for something in the

future, Level 3 now no longer references Level 1but is nearly stand-alone, and the occupancychapters (12-20) now contain the beginnings of a“decision tree”, helping choose which Level appliesin a given facility.

The eventual goal is to be able to provide anappropriate level of medical gases (and ultimatelyother systems as well, eg. essential electricalsystems) based on the procedures done in thatfacility, not the name over the door. This is aresponse to the simple fact that in today’shealthcare environment the name over the door isbecoming less and less meaningful.

The levels program explicitly throws theresponsibility for a crucial decision back on theowner (where it always has belonged) However, afact of today’s healthcare environment is that someowners are uncomfortably willing to cut corners tosave a few dollars. The Levels program thereforeopens an opportunity to the owner who is willingto trade risk for money. It is the resultant risk topatients which is the weakness of the plan.

Compounding this potential problem is the simplefact that the decision trees as now written aresimplistic, incomplete and difficult to interpret.There is no additional guidance on how to fit specificcases.

Overall, Beacon recommends, except where theowner requires a specific level (and is thereforewilling to accept the consequences of that decision)that only Level 1 be used as a basis for design.

As NFPA refines the decision trees in the 2002edition edition and beyond, expect the Levels tobecome more useful and eventually to be thestarting point in any med gas design process.
