Overview Process - SHS• You will first complete your ‘Placement Details Form’ on My Career Zone. Your

Module lead (Martin Jones) will log in to approve the placement on My Career Zone.

• If you do not have a contract we will create ‘Placement Agreement’ and send this to your employer to sign and will also request some insurance information. If you have a contract we will send a ‘Letter of Expectation’.

• Once we receive confirmation and the signed document, we will then email you to countersign the placement agreement and sign a risk assessment form.

• If you are undertaking an international placement there will be some additional insurance forms to complete.

• You will need to complete/sign and email these forms back to us BEFORE you can start counting your hours towards your assessment.

• You will also need to complete the online ‘Induction Checklist’ during the 1st week of your placement to ensure your Health and Safety.

• Don’t forget to get your ‘Hours Form’ signed off by your employer, and hand this in with your assessment to your module leader. Please also email a copy to [email protected] - You need to complete 40hrs!

Why do we have placement procedures?

Health and Safety • The University has a legal obligation to do its best to

ensure that you will be safe whilst on placement.

Insurance • We need to be confident that you are insured whilst on

placement and that your actions are also insured. • All placements that form part of a degree programme

have to be approved in advance to ensure you would be covered by the placement provider’s Employers Liability Insurance

• Supporting you in finding a suitable and worthwhile placement

• Once your placement has been approved, your Placement Agreement will be signed by the Module Lead

• Once your Module Lead has approved this, we will send your employer your ‘Placement Agreement’.

• This is your employment contract.

• We will ask them to sign and return the document, as well as providing their insurance information.

• Once we have received these two signatures we will send you a copy of the ‘Placement Agreement’.

• You will need to print, sign, scan and return the whole document to us. This ensures version control.

• We will also send you a ‘Risk Assessment’ form to sign and return.

International Placement

• If you are undertaking an international placement we will also include a ‘International Insurance Form’ for you to sign.

• You must complete this before commencing your placement.

• As the placement is international your placement provider may not be obliged by their country’s law to provide employers and liability insurance.

Induction Checklist

You need to ensure you complete the online ‘Induction Checklist’ during the first week of your placement for your health and safety.

Hours Form

Don’t forget to get your ‘Hours Form’ signed off by your employer, and hand this in with your assessment to your module leader. Please also email a copy to [email protected]

Placement Procedures

1 • Complete your online ‘Placement Details Form’. Your module lead will then log in to My Career Zone to approve the placement.

2 • Once your placement is approved a ‘Placement Agreement’or ‘Letter of Expectation’ will be sent to you, your module lead and your placement provider for signing. Your placement provider will also be asked to provide their insurance details and we will send you a ‘Risk Assessment’ document to complete and sign.

3 • If you are undertaking an international placement you will be asked to complete an ‘International Insurance Form’ also.

4 • An online ‘Induction Checklist’ will be required to be completed during the 1st week of your placement. Please also remember to complete your ‘Hours Form’ with your employer, and hand this in with your assessment to your module leader. Please also email a copy to [email protected]

1. Discuss your concerns with your Supervisor or Line Manager at your work placement
