New OCI Card-Minor – US PASSPORT HOLDER (INDIVIDUAL)

The documents in the Checklist are mandatory for your application to be processed.

This list of documents is not exhaustive and is subject to change based on the requirements of the

Indian Embassy / Consulate.

The Indian Embassy / Consulate have the right to request for additional documentation.

Complete the OCI Online Application Form on the Government Site

(https://passport.gov.in/oci/)

Once the OCI Online Application Form is completed and printed, the followingshould be submitted along with the application package

Print out PART A and PART B.

Submit BOTH parts for the OCI Application.

MINOR SIGNATURE/THUMB

For child of 5 years and above, signature / name of the minor child should bein the signature box under their photo on the first page of the application.

For child less than 5 years of age thumb impression of the minor child should

be in the signature box under their photo using only a BLACK or BLUE inkpad.

(Use Left Thumb for Boys & Right Thumb for girls).

PARENT CANNOT SIGN ON BEHALF OF THE CHILD UNDER ANY

CIRCUMSTANCES ON THE PART A.

Applications should contain BOTH PARENTS signature on the PART B ONLY

AND DULY NOTARIZED.

MINORS DO NOT SIGNPART B OF THE APPLICATION.

Wherever parent’s sign, the signature must match with the passport signature.

Applications will be put on hold and a new application will be asked for ifparent’s signature does not match with the passport signature.

NOTE - If the signature is OUTSIDE of the boxes provided, the application will be

REJECTED.

Marriage certificate from marriage registering authority only will be

recognized

Certificates issued by community institutions like Church/ mosque/ temple

are not acceptable.

Photos to be 2inches x 2 inches in size, in colour, depicting front pose strictly

APPLICATION. ORIGINAL US PASSPORT REQUIRED ONLY AT THE TIME OF OCI

MATCHUP.

IMPORTANT: Applications submitted without the above will be considered

INCOMPLETE and processing will be delayed.

COPY OF PARENTS OCI CARDS

If parents have OCI cards, it is mandatory to provide copies of OCI cards as

well

If parents OCI Cards are copied, BOTH information pages on the OCI card

should be copied.

VISA STATUS FOR PARENTS – ONLY IF PARENT(s) ARE FOREIGN CITIZEN(s) (NON US)

Provide copy of the following:-

Permanent Resident Card – Both Sides

o

Employment Authorization Card which states ‘Serves as 1-512’ or

Copy of Pages showing H1 / H4, L1 / L4, F1 / F2, J1 / J2 etc.

COPIES OF PARENTS PROOF OF RENUNCIATION

If one parent or both parents are naturalized citizens, proof of renunciation for parent(s) must be provided one of the following MUST be produced as

proof:-

o

Certificate of renunciation issued by an Embassy/Consulate of India

OR

o

Cancelled Indian Passport with stamp stating the passport is

cancelled due to obtaining US citizenship

If parent NEED to obtain RENUNCIATION CERTIFICATE, then they need to

apply for renunciation certificate PRIOR to submitting application for minor.

BIRTH CERTIFICATE

For applicants born outside of India, a photocopy of the birth certificate ORany government issued document that shows the following must be

provided:-

o Parents Name

o Date of Birth

o Nationality

If the applicant is applying through parents, one parent was born outside

India then Birth Certificate and Passport copy of the parent born outside

India.

If the BIRTH CERTIFICATE / Documents are in any other language besides

English, the document needs to be translated into English and it has to be

notarized.

NATURALIZATION CERTIFICATE

If minor gained naturalization through his/her parents and possesses no

separate naturalization certificate, then copies of the parents naturalization

certificates and a Notarized statement signed by BOTH parents is required

stating the circumstances that their child received his/her naturalization status

and why the child does not possess his/her own certificate.

If you acquired US Nationality through Naturalization, a copy of your

Naturalization Certificate is required.

If one has obtained more than one nationality after having Indian Citizenship

then provide ALL NATURALIZATION COPIES OR PROOF OF NATIONALITY

FROM ALL COUNTRIES that you have held citizenship.

If you do not have an Indian passport and is applying for OCI on the basis that

the parents and/or grandparents were Indian nationals – then PARENTSAND/OR GRANDPARENTS NATURALIZATION CERTIFICATE IS NEEDED

The US naturalization Certificate should state ‘FORMER NATIONALITY’ AS

INDIA. Certificates that state ‘Country of Birth’ as India CANNOT be accepted.

PROOF OF INDIAN ORIGIN

Copy of first three and last two pages of the last Indian passport held

OR

Any document that provides sufficient evidence that the applicant has held

Indian Citizenship like:-

o

Any other proof substantiating the request. Usually applicants are able

to submit a certificate of residence or place of birth of self / parents /

grandparents from First Class Magistrate / District Magistrate (DM) of

the concerned place.

o Notarized copy of educational certificate issued by an institution of

India and land ownership certificate by which eligibility may be

reasonably ascertained

OR

o A duly notarized affidavit to that effect

OR

o A copy of an Ancestral Nativity Certificate issued by an Indian official

or magistrate with sufficient authority to confirm Indian Origin of the

applicant.

The decision to whether accept this type of document as proof of IndianCitizenship will be made by the Embassy/Consulate Officer and will be FINAL

in such cases

PARENTS/GRANDPARENTS PROOF OF ORIGIN

If the applicant does NOT HAVE an Indian Passport and is applying for OCI on

the basis that his parents and / or grandparents were Indian Nationals, then

the applicant must enclose the following documents:-

o Copy of Proof of both parents current nationality and previous

nationality

o Applicants birth certificate (in which your parent's name is

mentioned)

o If the applicant is applying as a grandchild of an Indian National, then

the applicant must submit his birth certificate as well as his parent’s

birth certificate. All submitted birth certificates must show the

parent’s names clearly so that it establishes the relationship with the

applicants parents and grandparents

AND

o Copies of first three and last two pages of BOTH

parent/grandparent's Indian passport,

Copy of BOTH parent/grandparent's domicile certificate issued by the

competent authority,

OR

o

Copy of parents / grandparents OCI cards including BOTH information

pages on OCI cards

Note: If parents / grandparents have OCI / PIO cards, it is mandatory to give copies of

BOTH – Indian Passport & OCI Card

PROOF OF ADDRESS

For Minors, a proof of address from BOTH PARENTS or LEGAL GUARDIAN may be

submitted.

Only State Issued – Driving Licence (validity for at least 6 months pending)

OR

Utility Bill – Water (should be recent, no more than 3 months old)

OR

Utility Bill – Electricity (should be recent, no more than 3 months old) OR

Utility Bill – Gas (should be recent, no more than 3 months old)

OR

Valid lease signed by landlord and tenant (applicable ONLY FOR Atlanta and

Houston. For New York, lease document must be notarized.)

For College / University Students living on campus in University housing

/dormitories, a letter from your institution on the institution letter head

explaining the exact address that you presently reside in will suffice (Not

Applicable for Applications from Washington DC).

NOTE: DO NOT CUT YOUR DRIVER’S LICENCE COPY DOWN TO A SMALLER SIZE

NAME CHANGE DOCUMENTATION

If your name in any of the supporting documents provided is different in any

way from the name in the US Passport, a copy of ONE of the following

documents must be provided

o

Official Court Order for the name change

OR

o Endorsement of name change during naturalization process

If in case the middle name was changed to an initial then a notarized

statement letter is required.

PROOF OF RENUNCIATION OF INDIAN CITIZENSHIP:

If you ever held an Indian Passport, it is MANDATORY to provide proof of

Renunciation of Indian Citizenship along with your OCI application. One of the

following MUST be produced as proof:-

o Certificate of renunciation issued by the Embassy/Consulate of India

OR

o Cancelled Indian Passport with stamp stating the passport is

cancelled due to obtaining US citizenship

OR

o Deemed surrender certificate issued by the Embassy/Consulate of

India
