These clinic services are primarily provided by local health departments, hospitals and Federally Qualified Health Centers (FQHCs). The inclusion of any individual or organization in this Directory does not constitute or imply an endorsement by the New Jersey Department of Health. The information contained in the document is from organizations that responded to a "Dental Health Services Survey" conducted by the New Jersey Department of Health. Please note that clinic information is subject to change. The New Jersey Dental Clinic Directory is designed to assist people who have difficulty accessing dental care due to insurance or financial constraints. Many of the clinics have financial and/or residency requirements. Patients should be advised to call for an appointment and ask about required documentation regarding income or residency. program requirements include: a complete application with all required documents and a $25 non-refundable application fee. Youth applying to the program must be between the ages of 10-18 and not currently wearing braces, a household taxable income at or below 200% of the Federal Poverty Level and if approved, the family must be willing to pay $500 toward program participation.
