NATIONAL NOTIFIABLE DISEASE MONTHLY BULLETIN: ISSUE 12: VOLUME 12: 2017 The Notifiable Diseases analyses has been compiled from the Notifiable Disease Certificates received from 112 public health facilities, 101 private health facilities and 2 private labs nationally. This report has been compiled from 88% data from December 2016 and 78% data from December 2017 (public health facilities only). The following division is congratulated for being the best division in NNDSS reporting for this period: The Central division had the lowest rates of reporting at 64% for December 2017 compared to 88% reporting for December 2016 (a 24% decrease in reporting). The Western division stands at 76% reporting for December 2017 compared to 92% reporting for December 2016 (a 16% decrease in reporting). Similarly, the Eastern division stands at 81% reporting for the same period in 2017 compared to 72% for December 2016 (a 9% decrease in reporting). The rates were calculated using 2017 projections from Fiji Bureau Of Statistics (872948) and reported as per 10,000 population. The predominance of ARI, Viral illness and Diarrhoea is noted in both 2017 and 2016. The 5th leading condition in 2017 & 2016 was pneumonia for this reporting period. Reported cases of diseases that have significance in the international arena and those that are being vaccinated against were noted and referred to the VPD surveillance mechanism for verification (refer to tables).

There was a decrease in the reports received in 2017 compared to 2016. There is a time lag noted for this reporting period as a result of pending submissions. Time lags affect analysis and comprehensivity of reports markedly. Please note that an epi week commences from Sunday and ends on the following Saturday (7 days). Diarrhoea in the Northern division continues to push the ranking for Diarrhoea in the National dataset.

On average there was 78% reporting for this period from the divisions which makes surveillance for infectious diseases difficult as sensitivity of the system reduces with reduction of complete and timely reports.

The rates were calculated using 2017

projections from Fiji Bureau Of Statistics

(Central 370620, Western 341865, Northern

124911 and Eastern 35553) and reported as

per 10,000 population. Over both the years,

a predominance of diseases with viral origins

was noted, such as influenza, ARI and viral

illnesses. Diarrhoea of both viral and

bacterial aetiology are captured among the

top five conditions 2016 and 2017.

Central and Western divisions simulate the national conditions but in different rank order. Northern and Eastern division simulate the national conditions partially but in differing rank order. Northern 4th leading condition is Dengue Fever and Eastern divisions 5th leading condition is Conjunctivitis. Generally, reporting was lower in 2017 compared to the same period last year. Diarrhoea in the Northern division continues to have an impact in the ranking for diarrhoea in the National dataset. Dengue Fever was mostly reported from

Northern (n=132), Central (n=68), Western

(n=41) and Eastern (n=2). Mumps was

reported in the Western (n=122), Central

(n=59), Northern (n=20) and Eastern (n=3)

Conjunctivitis was reported in the Central

(n=20), Western (n=8) and Northern (n=6)

divisions, signaling the need for early public

health interventions in these areas.

The male and female categories simulate the national rankings. Generally, there is no difference between the two sexes. Similar to previous reiterations, reporting was generally lower in 2017 compared to 2016.

The cases of unknown sex made up 23.6% (n= 2518) in 2017 and 18.4% in 2016 (n=2049); it is important for those reporting Notifiable diseases to specify sex, ethnicity and age. The current percentage demonstrates that those reporting on NNDSS are still unresponsive to the request to clearly state sex, ethnicity and age. A remainder to all divisional heads to ensure reporting is made on the basis of sex, age, ethnicity and facility.

The cases of unknown ethnicity made up 21.0% (n= 2242) in 2017 and 15.8% in 2016 (n=1755); it is important for those reporting Notifiable diseases to specify sex, ethnicity and age. The Central and Northern Divisions demonstrate the highest frequency of unknown cases by ethnicity. This demonstrates that these divisions may not be reporting according to prescribed processes.

The rates were calculated using 2017 projections from Fiji Bureau Of Statistics (Suva 217670, Serua 29504, Rewa 84659, Tailevu 19770 and Naitasiri 19017) and reported as per 10,000 population. Out of all the cases received nationally, the Central division reported 38.1% of the total cases for 2017 and 43.2% of the total cases from 2016. A total of 8866 cases were reported between the year 2016 and 2017; 22.1% (n=4,810) were from 2016 and 18.6% (n=4,056) from 2017. Suva subdivision simulates the national ranking but in different rank order. All other subdivisions simulates the national ranking partially but in different rank order. The leading conditions for all of these subdivisions are: Conjunctivitis in Serua, Tailevu and Naitasiri; Dengue Fever in Rewa and Naitasiri and Leptospirosis in Naitasiri SD. Dengue was reported from the Rewa (n=44), Suva (n=23) and Naitasiri (n=1) sub division, signaling the need for public health interventions in these areas to reduce risks of outbreaks and institute effective control. Mumps was reported in the Rewa (n=15) and Suva (n=5) sub division. Conjunctivitis were reported from Suva (n=172), Rewa (n=16), Tailevu (n=9), Naitasiri (n=3) and Serua (n=1). Please note that Suva SD (CWM Hosp O&G, ED, Eye department, STI Clinic, Raiwaqa HC, Samabula HC), Serua SD (Navua HC & Korovisilou HC), Rewa SD (Nausori Maternity and Wainibokasi)) and Naitasiri SD (Vunidawa Hosp, Naqali HC & Laselevu HC) had pending notifiables when the issue was compiled. * This seems to be a recurring issue where these facilities have been noted to submit their notifiables in an untimely manner.

The rates were calculated using 2017 projections from Fiji Bureau Of Statistics (Lautoka 97503, Nadi 87392, Ba 54526, Tavua 25609, Nadroga 50021 and Ra 26814) and reported as per 10,000 population. Out of all the cases received nationally, the Western division reported 40.4% of the total cases for 2016 and 47.9% of the total cases for 2017. A total of 9,607 cases were reported in the Western division between the year 2016 and 2017; 20.7% (n=4,501) were from 2016 and 23.4% (n= 5,106) from 2017.

All SDs have different disease category in the top 5 conditions. This may indicate greater public health interventions to reduce risks of outbreaks.

Dengue Fever was reported from Nadi SD (n=27), Nadroga (n=8) Lautoka (n=5) and Ra (n=1).

Mumps was reported from the Nadroga (n=6) and Ba SD (n=2).

Please note that Lautoka SD (Lautoka Paeds, GOPD, Lautoka DOTS, Punjas Medical, Kamikamica HC & Nacula HC), Nadi SD (Nadi Hosp and Bukuya HC), Tavua SD (Tavua Hosp and Nadarivatu HC), and Ra SD (Rakiraki Hosp, Nanukuloa HC, Namarai HC & Nasau HC) had pending notifiables when the issue was compiled. * This seems to be a recurring issue where these facilities have been noted to submit their notifiables in an untimely manner.

The rates were calculated using 2017 projections from Fiji Bureau Of Statistics (Macuata 58721, Cakaudrove 33787, Bua 16099 and Taveuni 16304) and reported as per 10,000 population. Out of all the cases received nationally, the Northern division reported 11.5% of the total cases for 2017 and 12.3% of the total cases from 2016. A total of 2, 594 cases were reported in the Northern division between the year 2016 and 2017; 6.3% (n=1,370) were from 2016 and 5.6% (n= 1,224) from 2017.

All SDs have different disease category in the top 5 conditions. This may indicate greater public health interventions to reduce risks of outbreaks. Bua SD continues to drive the increased ranking of diarrhea in the Northern and National datasets.

Conjunctivitis was reported from Cakaudrove (n=36), Bua (n=27), Macuata (n=7) and Taveuni (n=1).

Dengue fever was reported from Macuata (n=102), Bua (n=18), Taveuni (n=8) and Cakaudrove (n=4) signaling the need for public health interventions in these areas to reduce risks of outbreaks.

The rates were calculated using 2017

projections from Fiji Bureau Of Statistics

(Lomaiviti 14400, Lakeba 6593, Rotuma 1703,

Lomaloma 2318 and Kadavu 10539) and

reported as per 10,000 population. Out of all

the cases received nationally, the Eastern

division reported 2.5% of the total cases for

2017 and 4.0% of the total cases from 2016. A

total of 715 cases were reported in the Eastern

division between the year 2016 and 2017; 2.1%

(n=447) were from 2016 and 1.2% (n= 268)

from 2017.

Diarrhoeal diseases, Influenza, Viral illness and

ARI were the most common diseases afflicting

the Eastern division.

Please note that Lomaiviti SD (Koro HC), Lakeba SD (Lakeba Hosp, Matuku HC, Kabara HC), Lomaloma SD (Cicia HC) and Kadavu SD (Kavala HC) had pending notifiables when the issue was compiled. * Acknowledgement to all the health facilities in Eastern division for submitting the timely reports in the month of December.
