HEALTH CARE REFORM AND THE PEDIATRIC GASTROENTEROLOGIST

Ever since the passage of the Affordable Care Act (ACA akaldquoObamacarerdquo) physicians are being bombarded with speculation on howhealth care reform will affect us The real truth is that no one knows The bill is incredibly complex and implementation of the ACA will varydramatically from state to state While Irsquom not an expert I do read the papers and Irsquod like to give you my take on what IS SUPPOSEDto happen and what MIGHT happen My primary source for this piece is an article on the ACA published in the August 24 issue of The Economist magazine

Here is what is supposed to happen according to the new law

1 As of 2014 insurers will no longer be allowed to ldquocharge the sick higherratesrdquo or discriminate against pre-existing conditions

2 Universal coverage will be mandated and people who do not havehealth insurance will be required to sign up for it (possibly providedthrough health care exchanges) Those individuals that do not havehealth insurance will be forced to pay a tax

As to what MIGHT happen that depends on whether you listen toMSNBC or the Fox News channel One side says the ACA will lead provide health care to 30 million people help control costs result incheaper insurance and make it easier for people to change jobs The otherside says the ACA will lead to a dramatic reduction in the number of full-time jobs (because the ACA does not apply to part-time workers) arapid explosion of health care costs another recession and a ldquogovernmenttakeover of health carerdquo

How does this affect us My best guess is that more people will have insurance but the insurance will not be as comprehensive as current policies Increasingly I am seeing patients with ldquotiered plansrdquo that try toshunt patients to lower-cost hospitals plans with limited drug coverage or plans with much higher copays Instead of having a group of ldquohavesrdquo(those with insurance) and ldquohave notsrdquo (those without insurance) we willhave a lot of patients with ldquohave a littlerdquo This group of patients who ldquohavea littlerdquo insurance will have a policy that is supposed to provide care if illness occurs but with high cost to the patient

There will be increasing pressure from patients insurers and governmentto lower cost reduce the number of tests we do and also limit the number

of treatments we provide I also believe there will be increasing pressure onNASPGHAN to establish guidelines and standards of care for various GIillnesses I sincerely hope that such micromanagement from governmentdoes not squelch our ability to think innovate and provide the best care forour patients At the end of the day though the ldquodoctor-patient relationshiprdquois being gradually replaced by a ldquodoctor-payor-patient relationshiprdquo inwhich the benefits of an intervention need to be weighed not only againstthe risks but against the costs

Carlo Di Lorenzo has done a fantastic job organizing this yearrsquos AnnualMeeting and I can hardly wait As always Irsquom looking forward not onlyto the terrific presentations but to seeing so many friends The ChicagoMarathon is the same weekend so downtown may be quite busy

We have two new innovations in this yearrsquos meeting First there will be a new symposium on nutritional issues for registered dietitians on Saturday courtesy of the Nutrition Committee We hope to makethis an annual event Second the Endoscopy amp Procedures Committee has organized a one-time ldquohands on sessionrdquo at the ASGE endoscopyheadquarters in Chicago This is in addition to our hands on endoscopysession (run by Marsha Kay) and our hands on motility session (run byJoe Croffie and the Motility Committee)

We have plenty of CME developed by both NASPGHAN and theNASPGHAN Foundation on our website JPGN has also published anumber of recent guidelines articles and reviews that are available for CME credit

A Little Of Old And A Little Of NewDear Friends

The 27th Annual NASPGHAN Meeting is getting close Registrations for both the Postgraduate Course and the regular Annual Meeting are coming in at a record pace Hotel rooms are being sold out and the speakers are submitting their slides to the organizing committee so that the presentations will flow smoothly The Annual Meeting will have a familiar structure two plenary sessions in the morning followed by multiple clinical and research concurrent sessions Meet-the-Professor breakfast sessions will open the day and we will continue to have Virtual sessions Hand-on Endoscopy and Manometry sessions and will end the meeting with the GI Jeopardy

There are some exciting novelties though For the first time we will have a Nutrition Symposium aimed at Registered Dietitians While universally considered invaluable partners of pediatric gastroenterologists in both academic centers and private practices dietitians in the past have rarely attended NASPGHAN meetings This year Praveen Goday and the Nutrition Committee put together a tremendous Symposium directed to Registered Dietitians but that could be of great interest to many other health care providers The Nutrition Symposium will be a one-day event held on Saturday Oct 12 and will cover diverse topics including but not limited to Food Allergies Behavioral issues related to eating Nutrition in Inflammatory Bowel Disease Energy balance and its implications for weight management and Nutrition and Bone HealthThese presentations will be followed by round table discussions on parenteral nutrition shortages blenderized tube feeding functional abdominalpainFODMAPS and g-tube placement We hope to make this an annual event

One of the unique features of this yearrsquos meeting will be the presence of speakers giving ldquoResearch Lessons Learnedrdquo as state of the art research lectures integrated within the abstract sessions We have recruited several of the most accomplished researchers in our field to give these lecturesthat will provide an historical perspective on different research topics such as neonatal liver disease intestinal stem cells gut development functional GI disorders and IBD

We have expanded the LASPGHAN portion of the program Dr Miguel Saps Dr Madrazo de la Garza and Dr Orsi have put together an extraordinary program in Spanish that will include invited lectures debates on common GI diseases and Meet-the-Professor (Reunion con el Professor) breakfasts

Finally on Sunday morning we will take advantage of the recent move of the ASGE headquarters to the new ASGE Institute for Training and Technology (ITampT) in Chicago The ASGE will allow us to use their new state-of-the-art integrated hands-on and didactic medical training facility and under the leadership of Dr Jenifer Lightdale and Dr Doug Fishman we will host a basic endoscopy course which will complement well the Hands-on Endoscopy course directed by Dr Marsha Kay during the NASPGHAN meeting We will be the first to use this center

A new CME case-based newsletter entitled ldquoEnteral Nutrition asFirst Line Therapy in Treating Children and Adolescents withCrohnrsquos Diseaserdquo is now complete NASPGHAN and APGNNmembers will receive a printed copy of the newsletter The newsletteroffers up to 20 AMA PRA Category 1 CME creditsTM

EEN offers a viable alternative to corticosteroids in inducing remission in pediatric Crohnrsquos Disease (CD) However despite the evidence nutritional therapy has not been universally adopted by many centers Furthermore there is a wide variance in treatmentprotocols used for induction of remission including type of formularoute of administration and duration of induction therapy Evengreater diversity is present for maintenance therapy protocols

In deference to the wide variability in the use of enteral nutrition asa primary therapy for CD and the variation in protocols employed

the NASPGHAN IBD Committee formed the Enteral NutritionWorking Group (ENWG) to review the current medical literaturesurrounding these issues A Clinical Report was published on whichthis CME newsletter is based Critch J Day AS et al on behalf of the NASPGHAN IBD Committee ldquoThe Utilization of Enteral Nutrition to Control Intestinal Inflammation in Pediatric CrohnsDiseaserdquo J Pediatr Gastroenterol Nutr 201254298-305

The NASPGHAN MOC Task Force has created web-based quality improvement and practice performance projects that will meet therequisite 40 MOC Part IV credits for each 5-year cycle

Currently three modules are available on the topics of Colonoscopy Upper Endoscopy and Failure to Thrive A fourth module on InformedConsent will be available shortly Each module will take a minimum of 4 months to complete If your MOC cycle ends in 2013 we highlyrecommend that you register now to allow ample time for completion of activities

NASPGHAN will charge a nominal fee of $250 for participation in any or all of the 4 modules Each module will be worth 25 MOC PartIV credits thus taking two modules will fulfill all ABP Part 4 MOC requirement for a five-year cycle

CLINICAL CARE AND QUALITY COMMITTEE

The Clinical Care and Quality Committee has three annual responsibilities to select articles for MOC part 2 to review the clinical vignettes and to manage the clinical guidelines and reportsThis year there were over 170 clinical vignettes sent in for the AnnualMeeting which represented a significant increase and will providegreat diversity of clinical cases for the Annual Meeting Hopefully the articles picked for MOC-2 will be used by the Board to create aninteresting question-based learning experience

With regard to guidelines we are happy to report that the cholestasisguideline is being revised by the Hepatology Committee There is anew guideline being developed on Esophageal Atresia and a new clinical report in the works on short bowel syndrome Still pending is a combined international guideline on constipation

This year the CCQ committee has also undertaken two new initiatives in clinical care We are going to produce a gap analysis of the guidelines and reports with the goal of providing as thorough a list of useful clinical support documents as possible A second project is the creation of a list of off-label use of medications to support clinical care in practice Many committees are being asked to share their expertise to make this as thorough aspossible with the hope that this would be a living resource that can grow over time

Our third active project is attempting to develop small quality improvement projects that could be piloted by CCQ sites and if useful shared with the membership at large Several ideas are beingconsidered including IBD check lists immunization in special populations

As always we encourage your ideas and suggestions whether it is an article for MOC a suggested topic for a guideline or an idea for aQI project please share your ideas

NUTRITION COMMITTEE

This report highlights but does not encompass all of the NutritionCommitteersquos work Within the past year the Nutrition Committeehas continued work on new initiatives as well as completion of existing action plans in collaboration with other NASPGHAN committeesndashnotably Public Education (information for gikids website) Advocacy (FDAlipids nutritional shortages obesity) Professional Education (Annual Meeting) and Research (grant reviews research agenda)

Several projects have been underway over the past year In April of2013 the second N2U Nutrition Course utilizing case-based learningwas successfully completed (special acknowledgement to the CME reviewers speakers NASPGHAN support staff and Nutricia) Theenteral nutrition slide set supported by Nestleacute was distributedthrough the NASPGHAN website for educational use by membersMark Corkins and Asim Maqbool hosted a fiber educational podcastin the fall of 2012 supported by Procter amp Gamble Clinical reportsare in development regarding formula thickeners arsenic content infoods and management of nutrition in short bowel syndrome

We are very excited about the launch of the inaugural Dietitianrsquos Symposium at NASPGHAN 2013 directed by Praveen Goday theincoming chair of the Nutrition Committee

As my tenure ends on the Nutrition Committee I would like to thank all current and past Nutrition Committee members who havetirelessly served on the Committee along the way allowing the committeersquos action plans to become realized Special acknowledgementto my predecessors Maria Mascarenhas and Rob Shulman and especially to Praveen Goday who has graciously accepted the ldquoNutritiontorchrdquo in addition to the NASPGHAN staff (Margaret Stallings Kim Rose Heidi Jetter) as well as NASPGHAN Foundation (AmyManela)

The Committee is moving forward and starting to work on a fewmore clinical reports A very successful Single Topic Symposium titled ldquoDiscovering the future of Pediatric Inflammatory Bowel Disease Carerdquo was organized at last yearrsquos Annual Meeting Talks were focused on risk stratification Quality Improvement emerging concepts in pathogenesis and future medical treatments and prioritization of future efforts on research and QI delivery This wasattended by almost 300 attendees and was very well received

Members of the Committee have also been active in providing support for the development of content for the GIkidsorg websiteNASPGHAN Foundation grant review CCFA Pediatric AffairsCommittee Camp Oasis Advocacy Pediatric Program in a Box(PIAB) and APGNN IBD slide set review

Last but not the least a working group comprised of NASPGHANAGA and AAP members developed IBD Quality Measures whichwere posted for public comments in March of this year and are in theprocess of being adapted based on the feedback

In short this has been an active and a productive year for the Committee

The Professional Development Committee has continued to focusduring this past year on ways to support and enhance the overall professional development of our membership and facilitate growthand networking between all aspects of our organization The Committee held the inaugural Professional Development Commit-tee dinner symposium at the end of the Postgraduate course last year in Salt Lake City to complete the 3-year cycle of the outgoing highlysuccessful chair Mitchell Shub MD This year the Professional Development Committee (PDC) begins the next 3 year cycle withthe first installment of the Professional Development Symposium Series at the Annual Meeting in Chicago This symposia series at each of the Annual Meetings over a 3 year period is designed to provide guidance to our membership and help them achieve successful and rewarding careers It is our hope that these symposiatouch on all aspects of professional development and growth andhopes to reach a broad spectrum of our societyrsquos members This yearsworkshop is titled Career growth and what leadership meansmdashfroma personal to an organizational perspective The symposium will befeatured at the conclusion of the Thursday October 10 2013 Post-graduate course with a light dinner and will feature an all-star cast of notable NASPGHAN celebrities from 630 pmmdash930 pm Speakers for the meeting this year will include the Division Directorfrom Childrenrsquos Hospital of Philadelphia David Piccoli pastNASPGHAN President Dick Colletti the PI of Improve Care Now and initiator of the PediGI bulletin board as well as rising starNicola Jones MD from the Hospital for Sick Children in OntarioCanada Topics for this yearrsquos exciting symposium will include leadership skills in practice vs academic medicine leadership in organizations and executive functioning as well as mentoring in allspheres of life

In the next two years the 2nd and 3rd part of this 3 part series sponsoredand organized by the Professional Development Committee will include the following themes Womenrsquos and Diversity issues in pediatric gastroenterology at the Annual Meeting in Atlanta GA2014 with topics such as how to break the ldquoglass ceilingrdquomdashworking towards achieving leadership positions having children and the

ldquointerruptedrdquo career pathway organizing within our society buildingbridges with other organizations and how to achieve effective work-life balancemdashachieving the balance Year 3 of the Symposia Serieswill be held at the Annual Meeting in Washington DC 2015 andin keeping with the venue being our nationrsquos capital the overall theme will be Advocacy for our patients for faculty divisions andpractices as well as advocacy for trainees Topics to be covered at this3rd symposium of the series will include Advocacy 101 how to get involved how to better advocate within your academic sphere how advocacy can also include fundraising and will address issues regarding how to do international work

We have continued to serve the interests of our society in various ways including providing representatives to the GastroenterologyWomens Coalition participating in development of the curriculumfor the Annual Meeting and thanks to the efforts of PDC memberGitit Tomer MD NASPGHAN will be represented with speakersat a special symposium given by the GWC at DDW 2014

The PDC has been tasked with assuming the solemn responsibilityfor making sure that obituaries are written to honor the life and accomplishments of our recently deceased members Further we are continuing the work started under Dr Shubrsquos leadership with a proposal to resume the NASPGHAN Mentoring and Career Development Program with the help of Drs Samra Blanchard andBinita Kamath Dr Jacqueline Fridge continues to work on revisingthe Career Choice Survey for fellows and junior faculty and Dr BessSchoen will oversee the Members Highlight section in the QuarterlyNewsletter

Finally a critical component of the PDC is the selection of new members to the variety of NASPGHAN committees A key task ofthe PDC over the coming year will be to develop specific guidelinesand criteria for committee membership selection and a process that isstreamlined to facilitate the PDC and Council in selecting the mostqualified and enthusiastic members for the right committees

The Professional Development Committee continues to be a vibrantand active committee within our NASPGHAN organization andwill continue to advocate for ways to improve the overall developmentof our members

Carlo Di Lorenzo MD is the 2013 recipient of the award for Senior Investigator in Pediatrics from the InternationalFoundation for Functional Gastrointestinal Disorders He was among eight investigators given awards by IFFGD for their contributions to scientific and clinical advancements in the area of functional gastrointestinal and motility disorders Dr Di Lorenzo and seven others were recognized at the 10th International Symposium on Functional Gastrointestinal Disorders in Milwaukee WI in April

The FDA Center for Drug Evaluation and Research in co-sponsorshipwith AGA ACP the Crohns and Colitis Foundation of AmericaInc NASPGHAN and the Pediatric IBD Foundation is holding a two-day public workshop entitled ldquoGastroenterology Regulatory Endpoints and the Advancement of Therapeutics (GREAT II)rdquoPartners and stakeholders planning the workshop also include patientsand representatives from the Eunice Kennedy Shriver National Institute of Child Health and Human Development at NIH

This workshop will provide a forum to consider issues related toendpoints that can support drug development in pediatric and adultCrohnrsquos disease Presenters and panelists for the workshop includeCrohnrsquos disease patients to academic investigators regulatory colleaguesfrom FDA EMA Health Canada and the PMDA and industrypartners

I have been asked by NASPGHAN leadership to make a few personal comments about our friend and colleague David Branski who passed away August 1 David and I workedclosely together the past three years as co-Editors-in-Chief for the Journal of Pediatric Gastroenterology and Nutrition (JPGN) David earned my utmost admiration and respect Wedeveloped a truly collegial working relationship and a friendship I will miss deeply He was extremely intelligent thoughtful modest and dedicated to our societys journal Even while fighting a terminal illness David continued his work for JPGN and for our societies

David leaves a wide-reaching impact on our profession as a respected investigator teacher clinician and leader He was noted for scientific contributions in diarrheal diseases and in celiac disease his leadership as a Pediatric Chair at Hadassah University Hospitals in Jerusalem and most recently for service to the Journal of Pediatric Gastroenterology and Nutrition as Editor-in Chief Eastern Hemisphere His achievements were recently recognized with the ESPGHAN Distinguished Service Award in May 2013

Our thoughts and deepest sympathy go out to his wife Effie their four children and two grandchildren David will be missed by all who knew him

Recently the SOGHN developed a proposal to add food securityto the AAP Agenda for ChildrenStrategic Plan which was well received by leadership ldquoPoverty and Child Healthrdquo (including foodsecurity) has been added to the strategic plan and will be a new areaof focus for the Academy and its 60000 members

This yearrsquos National Conference and Exhibition (NCE) the Annual Meeting for AAP members with over 12000 attendees will take place October 26ndash29 2013 in Orlando This yearrsquos Section Program will focus on intermittent paroxysmal abdominalpain and vomiting and Jeffrey Bornstein Bruno Chumpitazi Carlo Di Lorenzo Alessio Fasano Mark Gilger B Li ColinRudolph and I will present a host of GI sessions of interest to boththe subspecialist and general pediatrician The SOGHN has playeda significant role in recommending topics specific to our specialtyWe hope to see many of you there

NASPGHAN VISITS THE NIH AN OVERVIEW AND USEFUL LINKSIn early June members of the NASPGHAN Research and Advocacy Committees visited the NIH in Bethesda MD and Capitol Hill A key goal was to highlight NASPGHANrsquos recently published Pediatric GI Research Agenda (httpwwwnaspghanorgwmspagecfmparm1=289)At the NIH we had productive meetings with leaders from NIAID (National Institute of Allergy and Infectious Diseases) NICHD (NationalInstitute of Child Health and Human Development) and NIDDK (National Institute of Diabetes and Digestive and Kidney Diseases)

NIAID (httpwwwniaidnihgovpagesdefaultaspxwtac=tnHome)Drs Fred Cassells and Richard Gorman emphasized that any investigator irrespective of funding status can take advantage of NIAID contractresources (httpwwwniaidnihgovlabsandresourcesresourcesPagesdefaultaspx) Vaccine development (eg for diarrheal diseases) andpartnerships for technology development were highlighted Helpful information on grant writing including tutorials and samples is availableon the NIAID website (httpwwwniaidnihgovresearchfundinggrantpagesaagaspx)

NICHD (httpwwwnichdnihgovPagesindexaspx)Dr Gilman Grave noted that 10 of NICHD funding supports GI research (much of it pertaining to necrotizing enterocolitis and the microbiome) Since 2010 NICHD has undergone a Vision process (httpwwwnichdnihgovvisionpagesindexaspx) which was a formidableattempt to develop an identity that sets the Institute apart from other NIH institutes Themes which are in implementation as of 2013 includeDevelopment Pregnancy and Reproduction Plasticity and childhood behavior and cognition

NIDDK (httpwww2niddknihgov)The folks from NIDDK and in particular the Digestive Diseases and Nutrition Division (httpdigestiveniddknihgov) came out in full force They included Drs Gregory Germino Stephen James Frank Hamilton Judy Podskalny Mike Grey Robert Tilghman and Megan Miller They had previously reviewed the NASPGHAN Pediatric GI Research Agenda and provided concrete examples of topic areasthat encompassed many of the research priorities identified by NASPGHAN These included an extensive portfolio of consortia for IBD gastroparesis liver disease in children intestinal stem cells and obesity They offered the potential for Pediatric GI researchers to tap into existing networks For example celiac disease researchers might want to utilize the TEDDY study (The Environmental Determinants of Diabetes in the Young) Several underrepresented topics such as exocrine pancreatic research were also mentioned as areas of interest for NIDDK

As required by law on August 1 pharma ceutical and device companies started collecting information on payments and other transfers of value to physicians as well as ownership or investment interests held by physicians and their family members Information collected between Aug 1mdashDec 31 2013 will be transmitted to the Centers for Medicare and Medicaid Services (CMS) and eventually made available to the public in September 2014

What physicians should be doing throughout this initial data collection period is keep track of those payments and transfers of value made to themand also be mindful about investment interests During the second quarter of 2014 physicians will have an opportunity to preview their industry-reported information and dispute any inaccuracies this is why tracking payments and transfers is important

CMS has developed a number of resources for physicians to help them navigate the new ldquoOpen Paymentsrdquo initiative including a free mobile app thatis available in iOS and Android platforms The ldquoOpen Paymentsrdquo application can be found by searching the app store on a mobile device A similarapp is available for industry CMS is hoping that with this tool the number of disputes between physicians and industry will be minimized

An important feature of the mobile app is that physicians can use the app to send their profile information to the industry app which is accomplished through a QR code Similarly the industry app can send information to a physicianrsquos mobile device also through QR code As a result upon interaction with a vendor the physician can capture information about the event that occurred eg nature of valuepayment howmuch when Physicians should download the physician app become familiar with it and begin using it

A Little Of Old And A Little Of NewDear Friends

The 27th Annual NASPGHAN Meeting is getting close Registrations for both the Postgraduate Course and the regular Annual Meeting are coming in at a record pace Hotel rooms are being sold out and the speakers are submitting their slides to the organizing committee so that the presentations will flow smoothly The Annual Meeting will have a familiar structure two plenary sessions in the morning followed by multiple clinical and research concurrent sessions Meet-the-Professor breakfast sessions will open the day and we will continue to have Virtual sessions Hand-on Endoscopy and Manometry sessions and will end the meeting with the GI Jeopardy

There are some exciting novelties though For the first time we will have a Nutrition Symposium aimed at Registered Dietitians While universally considered invaluable partners of pediatric gastroenterologists in both academic centers and private practices dietitians in the past have rarely attended NASPGHAN meetings This year Praveen Goday and the Nutrition Committee put together a tremendous Symposium directed to Registered Dietitians but that could be of great interest to many other health care providers The Nutrition Symposium will be a one-day event held on Saturday Oct 12 and will cover diverse topics including but not limited to Food Allergies Behavioral issues related to eating Nutrition in Inflammatory Bowel Disease Energy balance and its implications for weight management and Nutrition and Bone HealthThese presentations will be followed by round table discussions on parenteral nutrition shortages blenderized tube feeding functional abdominalpainFODMAPS and g-tube placement We hope to make this an annual event

One of the unique features of this yearrsquos meeting will be the presence of speakers giving ldquoResearch Lessons Learnedrdquo as state of the art research lectures integrated within the abstract sessions We have recruited several of the most accomplished researchers in our field to give these lecturesthat will provide an historical perspective on different research topics such as neonatal liver disease intestinal stem cells gut development functional GI disorders and IBD

We have expanded the LASPGHAN portion of the program Dr Miguel Saps Dr Madrazo de la Garza and Dr Orsi have put together an extraordinary program in Spanish that will include invited lectures debates on common GI diseases and Meet-the-Professor (Reunion con el Professor) breakfasts

Finally on Sunday morning we will take advantage of the recent move of the ASGE headquarters to the new ASGE Institute for Training and Technology (ITampT) in Chicago The ASGE will allow us to use their new state-of-the-art integrated hands-on and didactic medical training facility and under the leadership of Dr Jenifer Lightdale and Dr Doug Fishman we will host a basic endoscopy course which will complement well the Hands-on Endoscopy course directed by Dr Marsha Kay during the NASPGHAN meeting We will be the first to use this center
