[Additional Information on study] This questionnaire is to be used in a research/scientific study, which will be performed by the research team who is based at the University of Sussex, the University of Oxford and the Institute of Human Development, India. Total anonymity is guaranteed upon request. If the subject of the interview allows for his/her identifying information to be recorded with responses, confidentiality cannot be guaranteed. However, all efforts will be done by the research team for confidentiality to be secured. Namely: • name and contact of the respondent will be stored separately from responses; • every initiated questionnaire is to be stored at the end of the same day it is answered in a locked compartment with the research team leader; • every interviewer signed a confidentiality agreement (which guarantees no sharing of information regarding questionnaire responses); • no one will have authorized access to responses apart from the research team leader and the interviewer for his/her respective interviews. • after the study is completed physical records will be destroyed and data will be stored in a unique computer under password access under the supervision of the research team leader.
