[Read out italics] I am part of a team conducting a study on economic hardship, crime and safety in Maharashtra. We are also interested in studying the quality of public service provision. The study is for academic purposes only. We are interviewing approximately 1000 households across Maharashtra. Your household has been selected randomly. We will not use your name if you so wish. Your name is not important to our study, and all data will be kept anonymous. Each interview will last approximately 45 minutes
[Additional Information on study] This questionnaire is to be used in a research/scientific study, which will be performed by the research team who is based at the University of Sussex, the University of Oxford and the Institute of Human Development, India. Total anonymity is guaranteed upon request. If the subject of the interview allows for his/her identifying information to be recorded with responses, confidentiality cannot be guaranteed. However, all efforts will be done by the research team for confidentiality to be secured. Namely: • name and contact of the respondent will be stored separately from responses; • every initiated questionnaire is to be stored at the end of the same day it is answered in a locked compartment with the research team leader; • every interviewer signed a confidentiality agreement (which guarantees no sharing of information regarding questionnaire responses); • no one will have authorized access to responses apart from the research team leader and the interviewer for his/her respective interviews. • after the study is completed physical records will be destroyed and data will be stored in a unique computer under password access under the supervision of the research team leader. Participation in this study is voluntary. Refusal to participate will involve no penalty or loss of benefits to which the subject is otherwise entitled.
