Student Notice of Concern Referral Process
The process for submitting a Student Notice of Concern Referral Form begins by clicking on the Student Notice of Concern
/Enter link.
Section/Utilities/Attendance Screen
You can also access the Student Notice of Concern Referral through the Attendance Enter link, when you begin the
Attendance Process

Select the week of attendance.
If you wish to enter a NOC, but have not entered your attendance, you will be asked to select a week of attendance. Clicking on the drop-
down arrow will display any additional weeks of attendance that has not been entered.

Click on the YES button and you will be taken to the page that allows you to select which students have completed the class early.
Click on the NO button to be taken to the Student Notice of Concern Referral process.
Student Selection
From the list of students displayed you may select the student for whom you would like
to submit a Student Notice of Concern Referral.
Notice of Concern Referral Form
The first question regarding communication with the student is required. If a response
is not selected, an error message will appear.
Academic Issues
If the concern is regarding Academic Issues or Non-Academic Issues that do not involve
Threatening or Non-Threatening Behavior, the Academic Office will have access to a daily list of
these concerns and the information will be forwarded to the appropriate counselor or advisor
for follow up.
Non-Academic Issues
If the concern is a Non-Academic Issue and involves Disruptive behavior in class, you will be required to select whether or not the behavior is threatening or
non-threatening. When SUBMIT is clicked, an email notification
will be sent immediately to the Academic Advisor, Counselor or Director of
Counseling/Advising

Comments that do not fall under any other category, may be entered in the additional
comments textbox.Click on Submit.
Verification
Before processing the form, you will be able to verify the data you have entered for the student. Click the PROCESS
button to submit the Student Notice of Concern.

You may view/save/print a PDF of this form.
Preview
Select whether you would like to view/print forms for students in the current section, or if you would like to print all the forms for all students in all your sections
for this quarter.

If you choose to view/print forms for students in the current section, you will be given a list of students to select. You may
select one or more; click Submit. If you would like to view/print all the forms, click Submit All.
Tips:
Be brief and to the point
Comments should be addressed to the Advisor or Counselor, not the student. DO NOT format text in the textboxes (i.e. bullets, bold, italics, tabs, etc.)
Limit the number of characters (150 allowed for all textboxes except for the Final Comments textbox, which allows 250.) All text beyond this will be cut off.
The student record will not be updated until the Process button has been clicked.
