My Purple FolderImproving health care for

My Purple Folder
The Purple Folder will help them to understand important things about you. For example: How you show pain How you like to be communicated with
What worries you.
You can use it at all appointments with health professionals for example doctors, dentists, pharmacists, opticians and hospital appointments.
The Purple Folder is for people with learning disabilities in Hertfordshire. It helps health professionals to understand:- What health issues you have had before.
What your health issues are now.
This folder contains important information that supports effective health care for people with learning disabilities.
This folder MAY be completed by people without healthcare training and MUST be supported by other health records

Completing the Purple Folder
The Purple Folder needs to be filled out by you.
You can ask someone who knows you well to help you.
It is very important that the Purple Folder has got the correct information in it about you.
It is important that this is regularly checked and kept up to date.
How to use the Purple Folder
• Health Appointments
When you have an appointment with a health professional show them your Purple Folder.
This will help them understand your health problem and get you the right support.
The health professional will complete the health appointment record within the Purple Folder.
This is a record of what has happened in the appointment.
• Staying in Hospital
If you need to stay in a hospital, show the doctors, nurses, and x-ray staff your Purple Folder.
The Purple Folder will help them understand your support needs while you are in hospital. For example, help with:
communication
eating and drinking
moving
managing your fears
managing your epilepsy.
This folder contains important information that supports effective health care for people with learning disabilities.
This folder MAY be completed by people without healthcare training and MUST be supported by other health records

It is your choice
If you lose your Purple Folder
We think it is a good idea for you to have a Purple Folder but it is your choice.
If you do not want one you do not have to have one.
If you have somebody who supports you - let them know if you lose your Purple Folder.
Let the Health Liaison Team know by phoning 01438-845372.
To get a Purple Folder Please ask your Adult Disability Team, a paid carer, a health or social care worker.
