1. Getting Started• Your enrollment invitation email will include a link and your

temporary ac cess code• Click on the link to go to the enrollment page and enter your

temporary access code• Select Next to continue

2. Enroll• Enter your Date of Birth, Create a User Name and Password• Password must contain 8 characters, an uppercase and lowercase

character and a number or special character• Password may NOT contain your first name, last name, user name

or date of birth• Check the box to confirm you have read and agreed to the Terms &

Conditions• Select Create New Account• A confirmation page will appear and a confirmation message will be

sent to your email account

3. Access Your Account• Enter your User Name and Password to login• My Care includes your Personal Health Record (PHR) information• My Diagnosis and Health Concerns includes: diagnosis date, stage

of cancer and any notable health issues including chronic diseases.• My Lab Results lists your current and past clinical results released by

your physician. To expand and view details of prior clincial results click on the date(s).

• My Medications provides a list of therapies, prescriptions, supple­ments, vitamins, over­the­counter drugs you are currently taking or have reported taking in the past and an easy­reference dosage guide.

• My Allergies includes your medications and other allergies, along with reported symptoms, reactions and the severity – as well as the date you experienced any allergic reactions.

5. Your PHR: Printing and Saving• Select the Print icon and choose to print All Pages or This Page

from the drop­down menu• Select the Download icon, click Yes to proceed and a copy of your

PHR will be saved to your computer• To protect your information, do not save (or view) this information

on a public computer• All information submitted or posted by you on My Care Plus is

subject to the My Care Plus Privacy Policy which can be viewed at any time by selecting the Privacy link at the bottom of any My Care Plus page
