Abstract In order to be considered as a potential bidder for the forthcoming Invitation to Tender, the respondent shall be either a single firm (hereinafter referred to as “the Firm”) or a group of firms (hereinafter referred to as “the Group of Firms”). The Firm or the Group of Firms shall demonstrate that it meets the applicable qualification criteria by completing this Qualification Questionnaire and by providing the requested documents and references.

1. INFORMATION ABOUT THE FIRM OR THE GROUP OF FIRMS

1.1 Names and Addresses

Firms interested in participating in the forthcoming Invitation to Tender procedure as a member of a Group of Firms1 shall return a SINGLE questionnaire for the Group of Firms, to be completed by the Lead Firm and signed by each of its members.The number of firms in the Group of Firms shall not exceed two.

2. GENERAL QUALIFICATION CRITERIA

CERN reserves the right not to consider the Firm or the Group of Firms for the forthcoming Invitation to Tender procedure in the event that:It has replied “yes” to at least one question in § 2.1;It has made a false declaration in the present Qualification Questionnaire;

Notwithstanding § 3.1.2, CERN reasonably concludes that the Firm or Group of Firms will be unable to complete or fulfil its obligations under the Contract due to the Firm’s or Group of Firm’s financial situation.

Only firms that have replied to the Market Survey and are qualified and selected on the basis of the following criteria shall be invited to tender: The technical, financial and other qualification criteria defined in this document;CERN’s previous experience with the firm, if any;The number of firms to be selected from each Member State2, as specified in § 2.3 hereunder; Successful delivery of sample units (refer to § 2.4 of this Qualification Questionnaire).

2.2 Subcontracting3

Subcontracting will be permitted for the execution of the Contract.However, the following parts of the Supply cannot be subcontracted: Storage of materials provided by CERN; Assembly and test of optical fibre connectors.

In case of a Group of Firms, the following activities shall be performed by the Lead Firm: Storage of materials provided by CERN; Assembly of optical fibre connectors.Subcontracting is subject to CERN’s prior written approval of each proposed subcontractor. CERN reserves the right to refuse the participation of a subcontractor and to make checks about the proposed subcontractor(s).

2.3 Country of Origin

The term “country of origin” shall mean the country(ies) of manufacture or last major transformation of the Supply (including sub-assemblies and components) by the Contractor or its subcontractor(s).The Firm, or each member of the Group of Firms, shall be established within one of the CERN Member States. The Supply must originate in the Member States. However, CERN reserves the right not to invite to tender the Firm, or the Group of Firms, if: It proposes a Supply originating in an associate member state, or an associate member state in

the pre-stage of accession to membership; and The combined annual financial value of orders, contracts and appointments already committed

for that country:o Exceeds the amount of the country’s financial contribution to CERN for the financial year

concerned; or o Would exceed the financial contribution if the Firm, or the Group of Firms, were awarded

the Contract.

In addition, with a view to improving the distribution of purchases among its Member States, and in order to decide on the number of potential bidders to be selected from each Member State, CERN shall take into account the Member State’s contribution, its industrial return coefficient and its estimated share of the Contract (refer to the Appendix: “Selection and Adjudication Criteria for Supply Contracts”).Please indicate the origin of the Supply in the following table.In the case of a Single Firm:

2.4 Qualification Process

A two-stage qualification process is foreseen to qualify interested Firms and Groups of Firms within this Market Survey. A Firm or Group of Firms shall qualify successfully for the first stage before the qualification process for the second stage is initiated. In this respect, only Firms and Groups of Firms having successfully qualified in both stages will be eligible to receive the subsequent Invitation to Tender.The qualification of the first stage is based on the documents returned by the Firm or Group of Firms in response to this Market Survey. Based on the qualification criteria defined in this document, only Firms and Groups of Firms that fulfil all requirements will be qualified for stage two.During the second stage, Firms and Groups of Firms qualified and selected in stage one may be requested to supply samples from their standard production and test reports for verification that the qualification criteria are met. The samples shall be provided at no charge to CERN and shall include: Two units of duplex patch cord, single-mode, pre-fitted with E2000/APC connectors; Two units of fan-out patch cord, multi-mode (G.651.1/OM3), pre-fitted with one MPO

connector, 12 LC/PC connectors and the fan-out kit for dividing individual fibres into simplex cords;

One optical fibre terminal module, single-mode, pigtails with E2000/APC connectors.

Stage two of the qualification process will last until the first quarter of 2020.CERN requires three months per set of sample units to carry out the qualification. It is the responsibility of the Firm or the Group of Firms to ensure the delivery of sample units within the timescales required by CERN for the qualification. If required by the Firm, CERN will return the samples after the qualification process.

Only Firms and Groups of Firms having demonstrated their compliance with the qualification criteria and the requirements defined in the Technical Description based on the supply of sample units and test reports to CERN will be eligible to receive the subsequent Invitation to Tender.

3. FINANCIAL QUALIFICATION CRITERIA INCLUDING RESOURCES

3.1 Size of the Firm or Group of Firms

4. TECHNICAL QUALIFICATION CRITERIA

4.1 Competence and Experience

4.1.1 Pre-Fitted Optical Fibre Cable Production

The Firm, or the Lead Firm (in the case of a Group of Firms), shall have proven competence and experience in the field of pre-fitted optical fibre cable production that are necessary to meet the requirements of the scope, complexity and volume specified in the Technical Description.Is this criterion fulfilled?

4.1.5 Relevant References

The Firm, or the members of the Group of Firms collectively, shall provide at least three relevant references corresponding to the above-mentioned competence and experience in the last five years that are similar to the future Contract in terms of scope, complexity and volume.CERN reserves the right to check the references provided. 4.1.6 Technical Documentation

The Firm, or the members of the Group of Firms collectively, shall provide with its response to this Market Survey the technical documentation (datasheets) of the components it foresees to use as the baseline for the Supply as described in the Technical Description. CERN shall use such technical documentation to verify the Firm of the Group of Firm’s compliance with the qualification criteria set forth in this § 4.1.Please complete the following table, or use a separate sheet with a table in the same format:

4.2 Equipment and Facilities

4.2.1 Testing Equipment

The Firm, or the Firm performing the tests in case of a Group of Firms, shall have adequate in-house test facilities and equipment needed to carry out tests and measurements as stated in the Technical Description. Specifically, the Firm, or the Firm performing the tests in case of a Group of Firms, shall have available at the time of this Market Survey the following testing equipment: Optical fibre testing equipment; Microscopy devices for visual inspection of optical fibre connectors and adapters; Interferometry devices for connector end face geometry measurements; Devices for ferrule concentricity measurements and connector tuning; Devices for insertion loss and return loss measurement; Software for generating test reports.
