A health behavior contract is a written strategy that helps a person reach a health goal.

Health Goal: I will follow a plan to manage stress. Effect on My Health: If I manage stress, I will sleep better. I will decrease the risk of being overstressed and becoming ill. I will be more relaxed. I will be able to concentrate in school.My Calendar: Mon. Tues. Wed. Thurs. Fri. Sat. Sun.
How My Plan Worked: I was less anxious during the test. I had a good night’s sleep and ate breakfast. However, skipping Saturday’s task was not a good idea because my notes were incomplete in a few places. Next time, I will try to do everything on my list.
