The purpose of the inspection was to determine the facility's compliance with the terms and conditions of the facility's NPDES Permit, Number 0PR00143*CD. You and Kevin Thrasher were present for the inspection. As a result of the inspection, I have the following comments: The facility is in violation of Part Ill, Item 15 of your NPDES permit, which states: "All discharges authorized herein shall be consistent with the terms and conditions of this permit. The discharge of any pollutant identified in this permit more frequently than, or at a level in excess of, that authorized by this permit shall constitute a violation of the terms and conditions of this permit. Such violations may result in the imposition of civil and/or criminal penalties as provided for in Section 309 of the Act and Ohio Revised Code Sections 6111.09 and 6111.99." A review of your discharge monitoring reports (DMRs) from January 1, 2012 to August 1, 2013 indicates you are in significant noncompliance for violation of nitrogen ammonia limits and E. coil limits. A listing of the violations and a guidance document explaining significant non-compliance is enclosed. If the facility does not return to substantial compliance in a timely manner, the facility will be subject to enforcement action. Within thirty (30) days of receipt of this letter, you must submit a plan on how the plant will return to and maintain compliance. This plan must describe the repair work, upgrades, or facility replacement that will be completed to achieve compliance, and a schedule with fixed dates for completing interim steps and a final date for returning to full compliance. 2. During the inspection the wastewater in the aeration basin appeared to be ahealthy brown color and the sand filters were clean and draining freely.

3. The plant is equipped with an ozonator downstream of the sand filters to providedisinfection The ozonator was not in operation at the time of my inspection andyou cited Part II, Item F of your permit which states, "Effluent disinfection is notdirectly required, however, the entity is required to meet all applicable dischargepermit limits." Based on the recent E. co//limit violations you should beginoperating the ozonator.

4. The final effluent appeared clear and free of odor. There were no observableimpacts to the stream.

Please address and provide a response to items #1 and 3 within thirty (30) days uponreceipt of this letter.

The Ohio EPA strongly encourages pollution prevention as the preferred approach forwaste management. The first priority of pollution prevention is to eliminate thegeneration of wastes and pollutants at the source (source reduction).
