Chapter 1. Executive Summary
1.1 PROJECT BACKGROUND
The Odisha Works Department (OWD) has a medium to long-term comprehensive programme to rehabilitate, upgrade and expand the road network throughout the state to improve accessibility, con-nectivity and enhance the overall development. It is the pre-eminent state level road authority in Od-isha which deals with issues relating to Feasibility Study and preparation of Detailed Project Report including the Route Selection, even for relatively low volume roads.
The Government of Odisha has secured a loan from International Bank for Reconstruction and Development (The World Bank) towards the cost of Widening and Strengthening of existing car-riageway to 2-lane for Chandbali–Bhadrak–Anandpur Road Project (88 Km).
Chandbali – Bhadrak – Anandpur Road Project is part of 825 km of roads under Phase-I which is a Section for promoting trade and regional integration between districts by improving transport com-munication. The project road Chandbali – Bhadrak – Anandpur is a part of the Phase-I road devel-opment, which was built several years ago with two lane configuration and currently serves moderate level of traffic. The project involves widening of the existing road which has an average carriageway width of 5.5 m to 6.8 m and 1.5m. Earthen shoulder on both sides, to a 7.0 m carriageway and 2.5m shoulders on both sides in flat and rolling sections. The expected outcome of the project in-clude reduced transportation time of goods and better connectivity between districts along with im-proved safety standards; reduced transit time for import and export goods via port.
1.2 THE PROJECT
The Bhadrak–Anandpur (SH-53) section from Km. 0/00 to Km. 43/00 is a part of Jagatpur–Kendrapada–Chandbali–Bhadrak 152 Km. project corridor. The road section Bhadrak–Anandpur starts from Chainage 0/00 Km. of Bhadrak and ends at Km. 43/00 of Anandpur Road, in this Bhadrak is district headquarter. About 35% of the road length passes through built up area.
The total length of the project road is 43.000 Km. the road passes completely through plain terrain. This stretch of the road falls in the District of Bhadrak & Keonjhar. This stretch passes through major human settlements as Bhadrak, Randia, Barpada, Bonth, Basantia, Chenapadi, Fakirpur and Anandpur.
The location map of the project road is shown below in Figure 1-1;

2.5 ROUTINE MAINTENANCE As per Contract, the Contractor is required to maintain the portion of the road which has been handed over to him. The maintenance work is being done by the Contractor in the handed over stretch as and when it is required.
2.6 PROGRAMME OF WORK
The Contractor has given three months work programme for January‟2019, February‟2019 and March‟2019 for an amount of Rs.4.15 Cr., Rs. 3.00 Cr. and Rs. 4.04 Cr. respectively.

2.10 STATUS OF EXTENSION OF TIME
So far the progress of work has been very slow with respect to the work programme. The scheduled completion date of the project is 11.02.2019and progress achieved by end of December ‟18 is only 18.87%. As such it is not possible to complete the project by scheduled date. But the Contractor has not yet applied for EOT.

Chapter 4. Status of Risk
4.1 HANDING OVER OF SECTION
As stipulated in the Contract, land for road work (SH53) has been handed over to the Contractor on 12.03.2018 . The Contractor had to examine it and inform to the Employer the details of encumbranc-es at site if any.
Contractor had informed regarding the encumbrances, which after the scrutiny, Engineer had submit-ted it to the PMU. 4.2 STATUS OF UTILITY SHIFTING

Note:- *As instructed by PMU, Telephone poles can be removed by Contractor immediately dur-ing or before the execution of work. Therefore Telephone Poles are not treated as encum-brances.

4.3 ANY OTHER ISSUE;-
The „Contract Agreement was signed on 12th March‟2018 for the balance work of Bhadrak- Anandpur section of SH53. The work programme was prepared and submitted by the Contractor for the stipulated Contract duration i.e. from the month of Mar.‟18 to Feb‟19.
The work planned for December‟2018 was for Rs. 5.71 Cr. while, the achieved work done is only of Rs. 1.45 Cr. (with a short fall is Rs. 4.26).
The Contractor could not achieve the planned progress as per work programme given for December‟18 mainly due to shortage of materials & manpower like soil & moorum for earthwork & subgrade work respectively, shortage of labour gangs etc. This has affected the construction of Earthwork for embankment, Subgrade, GSB, WMM, DBM layers and structure work.
4.4 RISK INVOLVED The progress has been slow during the month due to the reasons as stated above, This
will result in delay in completion of work.

Chapter 5. Environmental Monitoring and Safety
5.1 ENVIRONMENT MANAGEMENT PLAN

Location and Installation of Crushers, Hot Mix Plants and Batching Plants
The Contractor has to submit Consent to Operate (CTO) from OSPCB but not submitted yet the Consent to Establish (CTE) from OSPCB for Batching Plants of SH-53 Camp and Hot Mix Plant from ÓSPCB’ to Engineer’s Office. Contractor has not submitted these yet.
4 Construction Camp(s) – Selection, Design and Lay-out
Contractor has to submit his proposed location plan of base camp at SH-53 and approval by the Engineer. Contractor has not submitted this yet.

Construction Vehicles, Equipment and Machinery
The Contractor shall maintain a record of Pollution under control (PUC) certificates for all vehicles at site office:- PUC certificates of the construction vehicles, equipment and machin-eries have been not submitted to Engineer.

Identification, Operation and Rehabilitation of Bor-row Areas
The Contractor has identified soil borrow area no.s 1, 2, 3 & 4 at Ch.10+800 (LHS) , 11+300 (RHS), 4+100 (LHS) & 17+114 (LHS) re-spectively of SH-53 and submitted documents to engineer for approv-al of borrow areas.
7 Identification, Operation and Rehabilitation of Stone Quarry
The Contractor has not obtained lease of stone quarry. Aggregate is being procured through private suppliers from stone quarries at Chandikhole.
8 Identification, and Opera-tion of Sand Quarry
The Contractor has not obtained lease of sand quarry. Sand is being procured from third party supplier.

Arrangement for Construc-tion Water
As per the Central Ground Water Authority (CGWA) guidelines, there are two categories of area. (i) Notified area and (ii) Non-notified area. For notified areas, permission/approval is required while for non-notified areas only NOC is required for abstraction of ground water more than 100 m3/day. Now this location comes under non-notified area. Therefore permission/approval for extraction may not be re-quired. Also as the extraction quantity is less than 100 m3/day. There-fore NOC is also not required as per previous notification. But as per new notification published by CGWA, NOC is required also as the extraction quantity is less than 100 m3/day. The Contractor has yet to obtained NOC.
10 Clearing and Grubbing Only ground cover necessary for works is being removed.
11 Stripping, Stacking and Preservation of Top Soil
Top soil is being stacked and preserved at identified construction and borrow area sites.

The potable water is being providing by Contractor in office, laborato-ry and proposed camp area. Provision of toilets/mobile toilets separate for women with sewage system and adequate water supply:- Toilets have been constructed at proposed base camp but not at site. These are being cleaned up regularly.

Transportation of Construc-tion Materials and Haul Road Management
Vehicles shall be covered during transportation of materials and main-tained haul roads:- Vehicles are not covered most of the times during transportation of construction materials. Though haul roads is being maintained during transportation.

Worksite Safety Manage-ment
Work site proper safety management shall be arranged:- Though safety arrangements have been done at site, it needs to be improved further. Temporary diversion has to be arranged: - Diversion has been ar-ranged at site wherever it is required.
Traffic safety has to be arranged as per Traffic Management Plan:- The “Traffic Management Plan” have to be submitted by the contrac-tor. Adequate PPEs to be provided to labourers and warning signage to be installed in high risk area:- PPEs have been provided to labourers and implemented but its im-plementation needs to be improved, warning signages are inadequate in high risk area. Precautions for electrical cables, wires and equipment:- At present, The Contractor has taken precautions for electrical cables, wires and equipment at base camp SH-53. Readily available first aid kits:- S The contractor has arranged first aid box at base camp‟s office, and at store room of SH-53 (Camp for P02). The Contractor shall take precautions to prevent danger due to mis-hap:- The Contractor has yet to be submitted the Construction Safety Plan for its implementation at site.
15 Accessibility The Contractor has to provide safe convenient passage for affected
road side users:- No obstruction reported from road side users.
16 Disruption to Other Users of Water
The Contractor shall not obstruct water bodies:- No obstruction reported yet.
17 Labour Requirements Local labours shall be employed preferably:-
Local labourers are engaged for construction work.

Pollution Management Pollution management has to be done in conformance with EMP:- Dust suppression measures needs to be applied during the construc-tion activities; Water sprinkling is being done at site regularly two to three times in a day. No dust emission has been reported in this month during con-struction activities. Pollution monitoring has yet to be scheduled half yearly by Contractor

through a NABL accredited agency. Waste will not be released into water bodies and drainage systems:- No waste water is being discharged directly into the water bodies. Dispose the fuel, lubricants and chemical wastes as per guidelines of SPCB/MoEF:- Used and waste oil has to be disposed off through authorised recy-cler. The used and waste oil generates in very less quantity. Though the Contractor has identified the authorised recycler. The Contractor so far has stocked the waste oil at base camp at SH53. Noise level shall be maintained of all construction vehicles, machiner-ies and equipment:- The Contractor has maintained vehicles, machineries and equipments and obtained PUC certificates of some of the vehicles and machiner-ies.

Drainage and Flood Con-trol.
Pre monsoon activities:- Contractor had cleaned up the channels wherever it is required before onset of monsoon. The clearing operation still being carried out whenever it is required to maintain the proper drainage.
20 Slope Protection and Con-trol of Soil Erosion
Slopes have to be protected with stone pitching and grass:- Slope protection work has not started yet.

Waste Management
Contractor has yet to identify waste disposal sites:- Separate garbage bins for garbage collection in camp area, office and laboratory have to be provided. Contractor has yet to identify site for bio-degradable waste disposal at SH-53 camp area. For non-bio-degradable waste disposal, Contractor has identified the authorised recycler. The Contractor so far has stocked the waste oil at base camp at SH53.
22 Chance Found Archaeo-logical Property.
No archaeological property has been found at site.

Demobilisation and De-commissioning.
The Contractor shall clean up the area or temporary structures after completion of work as per the approved waste management plan:- Demobilisation and Decommissioning has not started as project is in progress.
5.2 ROAD SAFETY
Though, barricading with bamboos and sand bags with reflective sticker and caution tape at site is being done, but hard barricading and more sign boards are required in construction zones in con-formance with codes & specifications.
Helmets, safety jackets, safety shoes, gumboot, nose mask and goggles etc. have been provided to the workers but, these are required to be used regularly and adequately during construction activities.

Chapter 9. Visits and Meetings
On 01.12.2018, Executive Engineer, (PMU) and CSC‟s Team Leader visited the site. Resident Engineer (CSC) and Contractor‟s Project Manager were also present during the visit.
A routine review meeting was held on 14.12.2018 by Resident Engineer at base Camp of the project with the Project Manager of Contractor.
On 23.12.2018, Executive Engineer, (PMU) and CSC‟s Team Leader visited the site. t Resident Engineer (CSC) and, Contractor‟s Project Manager were also present during the visit.
A routine review meeting was held on 27.12.2018 by Resident Engineer at base Camp of the project with the Project Manager of Contractor.
