“I couldn’t imagine life without my TRIOS
scanner and going back to analogue”
Interview with Dr Andrea Shepperson on using her 3Shape TRIOS®
What are the benefits for a dentist to go Digital? First of all, for the ability to visualise things that you just cannot see from an impression and model. It just allows so much more accuracy. Also it is streamlined and much more comfortable for the patients.
How do you feel using TRIOS daily? The learning curve can be really fast because the software is so intuitive. The tools are easy to use and it’s a very nice scanner to work with. Digital definitely decreased our workload. Because of the accuracy of the system, the number of follow-up visits practically dropped to zero. We have very few adjustments to do and the time saving is fantastic. TRIOS gives really good quality scans.
Why do you prefer scanning with TRIOS versus a conventional impression? The ability to digitize the patient opens the door to the future of dentistry, allowing a view of the patient that we have not seen before. Interdisciplinary care and global collaboration is possible and a 3D view from every angle allows us to plan more accurately. My staff appreciate digital because they are not pouring models and cleaning impressions trays and we are saving so much cost in impression materials. Also, we don’t have to send the impression or model to the lab anymore; we send the scan via the 3Shape cloud and this saves us even more time. I couldn’t imagine life without my TRIOS scanner and going back to analogue.
What do your patients think? The patients perceive what we do as very impressive. They are telling us it is much more comfortable than a conventional impression. They like watching the screen and seeing their mouth built; they find it so high-tech and fascinating and they are engaged and excited. We are explaining this in 3D diagnostic information, that gives us a huge insight into planning the case effectively, in ways we didn’t have before.
What do you think about the full colour and quality of the scan?I like it. I don’t have quite the same need to take photographs as we used to. We are reducing the number of intraoral photographs we take, because we can take a screenshot, bring the case up, or we can go to 3Shape communicate and visualise the case subsequently. We have found this as a very powerful patient record keeping tool. I love the fact I can take a scan and superimpose the restoration on the image.

Tetric® CADThe aesthetic composite blockTetric CAD is an aesthetic composite block for the efficient fabrication of indirect single-tooth restorations using CAD/CAM technology. Tetric CAD is based on the proven Tetric technology and is the digital supplement to the direct restoratives of the Tetric Evo-Line. Due to the pronounced chameleon effect, Tetric CAD restorations blend in with the residual tooth structure for an aesthetic result. The restoration is simply polished after milling and then seated using an adhesive cementation protocol. This processing technique is quick, easy and efficient while providing highly aesthetic clinical outcomes.

Clinical Equipment
Bluephase® Style 20iFast polymerisation at no compromiseBluephase Style 20i is currently one of the most efficient devices for light-curing dental materials. The use of a fast light-curing protocol does not entail any drawbacks in comparison with a conventional protocol, as has been revealed in the in-vitro study below.
An investigation by Ivoclar Vivadent’s R&D department showed that the shrinkage forces occurring at elevated light intensities remain nearly identical to those at lower intensities. In the study, Tetric EvoCeram® Bulk Fill IVA was light-cured as follows:
• Bluephase Style M8 = 800 mW/cm2 for 15 seconds• Bluephase Style = 1,200 mW/cm2 for 10 seconds • Bluephase Style 20i = 2,000 mW/cm2 for 5 seconds
Moreover, an investigation by Prof. F. Rueggeberg on intrapulpal temperature increase showed that the generally accepted threshold value of 5.5 °C is not exceeded when Bluephase Style 20i is used. Therefore, the use of the Bluephase Style 20i (Turbo mode) poses no increased risk for the vitality of the tooth because of a rise in temperature.

Endodontics
XP-Endo® Shaper – 3D-Shaping
“The goal of root canal shaping is to allow an effective irrigation that will facilitate the cleaning and to eliminate the pulp, the bacteria. The desired shape should altogether prepare a cavity specific to the root canal anatomy and allow a three-dimensional obturation.”S.cohen, R. Burns – Pathways of the Pulp
Technological advances and manufacturing processes are allowing the practitioner the ability to get closer to ideal root canal therapy. The “perfect” file should touch all the walls of the canal without changing its shape while still allowing room for disinfecting irrigation solutions. The aim is to achieve optimal disinfection in a minimally invasive fashion. Thus, both aims of root canal therapy can be achieved; a healthy surrounding periodontium and a strong root with maximal resistance to fracture.
FKG aims to develop advanced endodontic instruments that provide dentists with the best shaping ability, even in curved or oval canals.
The XP-endo Shaper is the latest instrument of the FKG’s range of 3D instruments. It is the epitome of what incremental innovation can create for modern dentistry; it features the combination of a dual technology and a unique expertise.
Firstly, the exclusive MaxWire® alloy provides the instrument with an exceptional flexibility and an extreme resistance to cyclic fatigue. It allows the XP-endo Shaper to shape and to progress inside the root canal with agility, whilst expanding and contracting its shape, adapting itself to the specific morphology of each canal.
In addition, the Booster Tip, thanks to its six cutting edges, guides the instrument easily toward the apical terminus and enables the dentist to start the shaping at an ISO diameter of 15, then gradually to increase its working scope to reach an ISO diameter 30.

Preventative
Prophy Pastes: effective and gentle
Fig. 1: Proxyt® in different grit sizes

Fig. 3: Gentle, professional teeth cleaning promotes oral health and pink-and-white aesthetics. (Picture: Dr L. Enggist, Ivoclar Vivadent AG, Schaan, Liechtenstein)
Prophy pastes have to meet stringent quality requirements.
They have to produce maximum cleaning results with minimal wear and roughening of the tooth structure. After polishing, the tooth surfaces should be as smooth as possible.
Various factors influence the polishing and cleaning results produced by prophy pastes. The abrasive particles contained in the paste constitute the most important factor. Their concentration, composition, size and structure significantly influence the product. Furthermore, the pH value, the type and concentration of the binding agent also play an important role. The type of cleaning instrument used (brush or cup) and its characteristics (hard or soft), as well as the pressure applied by the operator and the rpms of the instrument and the treatment time all contribute to the result. In addition, the texture of the surface in need of cleaning should be taken into account.
Roughening of surfaces must be prevented at all costApart from dentine and root cement, composites, compomers and glass ionomers can be worn down significantly under too much pressure. In their case, organic components are removed, leaving inorganic materials exposed. As a result, the surface becomes rough and provides ideal conditions for bacterial plaque to accumulate. The surfaces become more susceptible to staining by pigments from foods, beverages and tobacco products.
Needs-based cleaning and polishing The Proxyt prophy paste system helps to prevent unnecessary wear of the tooth structure as well as any roughening of the dental restorations (Fig. 1). Tough bacterial films and stains can be effectively removed with the pastes containing coarser particles. The fine paste is recommended for the treatment of exposed cervical surfaces, restored teeth and implant-supported restorations. In addition to its gentle cleaning action, the fine Proxyt paste is characterised by its ability to restore the natural shine of dental enamel and maintain the aesthetic gloss of restorations (Figs 2, 3).

What CBCT Can RevealBy Dr Kunal Shah DDS
Discover why 2D imaging sometimes isn’t enoughOverviewA healthy, 52-year-old female patient—with no relevant medical background, moderate previous dental restorations and generally good oral hygiene—had had LL6, LL7 and LR6 extracted 15-20 years ago. For the majority of that time, this loss in dentition did not impact her lifestyle. Recently, however, she was having issues with eating. The adjacent teeth were starting to drift and the opposing teeth on the other side were erupting, creating a functional problem rather than an aesthetic one.
Treatment PlanningWe took a periapical X-ray with a ball bearing, as well as a CBCT scan with the CS 8100 3D. The first scan enabled the surgical team and me to assess the height of the bone, but the 2D image couldn’t gauge the buccal-lingual aspect or the bone density.
CBCT provided the 3D element, allowing us to:
• Track the depth from the crestal bone / bony ridge toward the nerve• Assess the mesial-distal width present for placement of the LL6, LL7
and LR6• Determine the buccal-lingual dimensions• Account for the biological width
We then evaluated the scans with the referring doctor and placed virtual implants on the X-rays. This exercise helped us to determine exactly where the implants should be placed, as well as the angulation, platform, length and type of implants required. We then took impressions using a single-stage wash putty technique and fabricated a study model and surgical stent with mock acrylic teeth. We practiced the surgery on the model, visualizing the implant position and comparing it to the virtual plan.
The goal of this upfront effort was to fine tune the treatment plan to minimize surgery time and associated risks. Based on my experience, I believe that the quicker the surgery, the better the primary stability and integration of the implant. From an aesthetic standpoint, a faster surgery also seems to lead to better gingival and mucosal healing, resulting in less future recession. Soft tissue is a key element for good aesthetics and the long-term success of the implant

Mastering Wax-ups & Diagnostics1-day Hands-on
Monday 5 November - Sydney
Szabolcs Hant7 CPD
Today CAD-CAM is one of the fastest growing areas in Dentistry. It is important to familiarise yourself with which areas need attention to detail to achieve outstanding restorations. On this course, I will share my 20+ years of experience with morphology and ceramic materials.
Understanding tooth morphology is paramount to success and wax-ups are a fundamental tool for diagnosis and communication.
Wax-up is also great if you want to improve your skills about shape, morphology and texture. If you learn with wax how to create a beautiful tooth, then you can complete the same with other materials.
