Being an athlete or activity participant at PHS is a privilege and requires a
commitment to being a better version of yourself through academics, attendance, and
character.
Through COMMITTED PARTICIPATION, athletics and activities serve as an
extension of the classroom, focusing on:
- Hard work, TEAMwork, Determination, Leadership, Confidence, Accountability
Athletics and activities provide training that promote lifelong leadership and
community building.

REFUND POLICY:a. A full refund will be given if a student quits the team within 2 weeks of the start date.
b. No refunds are given for students who leave the team after 2 weeks of the start date.
c. There will be refunds given to those students who terminate participation in an activity due to
injury, illness, or those not making the team. The student may be asked to provide a Doctor’s note
stating that the student is no longer able to participate for the remainder of the season. The
amount of the refund will be according to the following time schedule.
- Up to the first public performance/competition—full refund.
- First performance/competition through midpoint—half refund.
- Midpoint through season’s end—no refund.
d. No refunds will be given without verification, in writing, by the activities director.
e. Any refunds of participation fees must be requested through the activities director by the
participant within 1 week of the students last practice or contest.
f. Students that are dismissed from participation due to disciplinary issues will not receive refunds.
g. Students who leave a team after the first two weeks due to academic issues shall not receive a
refund.
ACADEMIC ELIGIBILITY PROCEDURE:IF... THEN...
Student’s GPA is below 1.75 from previous semester Student is on a weekly tracking sheet for all classes to start the new semester until Checkpoint #1
- If “F” on tracking sheet = ineligible for practice or games until proof of passing grade
- Failure to turn in tracking sheet = ineligible for 1 week for practice or games
Student has an “F” in any class(es) at a checkpoint for first time
Immediately ineligible for practice or games until proof of passing grade AND weekly tracking sheet until the next checkpoint.
- Tracking sheet for only classes with “F” at checkpoint
- If “F” on tracking sheet = ineligible for practice or games until proof of passing grade
- Failure to turn in tracking sheet = ineligible for 1 week for practice or games
ACADEMIC ELIGIBILITY PROCEDURE (CONT.)
Progress Reports must be turned into Activities Director by noon on Friday
or noon on the last day of the school week.
IF... THEN...
Student has an “F” in a class or classes for second consecutive checkpoint
Student is immediately ineligible for games and practices until the next checkpoint
- Counselors and school staff will work with student to address any academic barriers and formulate a plan for achieving success)
A student-athlete is absent for 4 or more class periods in a single day.
Student is ineligible to participate in games or practices on that day
- Exceptions may be made for excused absences or by administrative discretion
A student is suspended from school Student is ineligible to participate in any practices or games for the duration of the suspension. Student may not attend any school sponsored events during the suspension.

Buying a ticket does not entitle you to behave in a way that takes
away from the enjoyment of the participants or other spectators.
CHEER FOR THE RAILS...NOT AGAINST THE OTHER TEAM!!
Act as though you are on camera because the chances are you are
being recorded in some way, and young eyes are always watching.
SOCIAL MEDIA GUIDELINESCell Phones and other electronic devices that can capture photos are prohibited in
locker room areas unless cleared by the head coach of the sport or activity.
Before posting items on social media, keep in mind that what is posted could have
school and athletic consequences per school policy and the code of ethics in the
MSHSL bylaws.
Before you post, think about…
- Who will see what I post?
- Will anyone be offended, embarrassed, or hurt by it?
- Am I proud of what I am posting?
- How would I feel if someone posted this about me?

Athletic Training ServicesAthletic Trainers (ATs) are licensed health care professionals who collaborate with
physicians to provide preventative services, emergency care, clinical diagnosis,
therapeutic intervention and rehabilitation of injuries and medical conditions.

Athletic Training● Our goal is comprehensive care of all Proctor High School student
athletes○ Regardless of where you receive care○ We have communication with physicians at both Essentia and St Luke’s
● Athletic Training Rooms at the high school and hockey rink

○ Return to play; must have written documentation from providers■ If your student athlete is seen by a medical provider, they must
provide the AT staff with written documentation from that provider stating any restrictions in participation● This can be hand carried, or faxed (218.628.4931) Attn: Athletic
Training Staff■ This includes general medical issues such as flu, pneumonia,
bronchitis and so on■ If the note is not provided your student athlete will be unable to
resume participation until provided to AT staff

Athletic Training -- How ImPACT® Works
• Baseline tests obtained– Typically during Freshman and Junior year,however, we will ensure a
baseline has been collected within the last two years
• If a concussion is sustained– Team approach between ATs, school nurse, guidance counselors,
teachers, coaches, and physicians as needed– Athlete will work through the return to play progression– ImPACT® test may be repeated before he/she returns to full play
Locker Rooms● Locker rooms need to be safe, clean and a healthy environment for everyone (MSHSL policies)
● No audio or visual recording devices, including cell phone cameras are allowed to be used in a locker
room
● Athletic Locks need to be purchased through the athletic office (athletic locks cost $10 and will be used
in the varsity locker room only)
● Students are responsible for making sure their locker is closed and locked.
● Students are expected to keep the locker room and their locker clean at all times
● The Boys and Girls physical education locker room will be used for visiting teams during events.
● Lockers must be cleaned out before the last day of school(all clothing etc. left in lockers will be
removed and donated to charity)
Lettering● Each team will have its own standard for lettering.
● A MSHSL violation of bylaw 205.00, 206.00, or 209.00 during a specific
season will disqualify any individual from lettering in that season.
● If the violation occurs in the senior year, the individual will not be
recognized as a multi sport athlete and potentially athlete of the year.

Goal/Mission = to create an opportunity for students to lead and be
role models within our school.
