2. EMERGENCY PREPAREDNESS AND RESPONSE (EPR)
TEAM LEADER
Roles and Responsibilities:
1. Shall be mainly responsible for building-wide emergency
planning and evaluation; assists the President and heads the
team in the absence of the president.

4. Sees to it that fire fighting, first aid and all other emergency

6. Exercises full responsibility during fire fighting and disastercontrol, rescue and evacuation of personnel, and first aidoperations.
7. Assesses the nature and extent of damage resulting from thedisaster and reports to the President.

3. DEPUTY MERGENCY PREPAREDNESS AND RESPONSE(EPR) TEAM LEADER
Roles and Responsibilities: 1. Assists the EPR Leader and acts for her/him in her/his absence
when emergencies occur.
2. Assists in the organization of the various EmergencyPreparedness and Response Teams and subsequently theirtraining which include making arrangement for assistance in getting
resource speakers, scheduling, and obtaining appropriate trainingmaterials.
3. Reports incidents or accidents and provides recommendations tothe EPR Leader and concerned Manager.

4. Arranges periodic emergency drills and reports to the EPRLeader who secures approval from the President and otherManagement Team.

Roles and Responsibilities:
1. Responsible for the provision and control of emergency medicalsupply and equipment stored at the clinic and to safeguard against
deterioration of drugs and medicines including the supply of oxygenand the breathing apparatus.
2. Treats the patient or victim in his/her full capacity as a physician.
Company Nurse
Roles and Responsibilities:
1. Assumes responsibilities of the physician/doctor in the lattersabsence.
2. Assists and coordinates with the First Aiders in conducting first-aid treatment.

3. Promptly notifies Safety Manager or Safety Engineer, shouldanybody have been treated for his/her injury arising fromincident/accident that occurred within the premises for the former tobe able to conduct investigation.
4. Coordinates with General Affairs and/or Security for the use ofemergency vehicle should a patient/victim needs to be immediatelytransported to the hospital.
5. Monitors condition of patients confined in the hospital as a resultof injury or illness acquired while on-duty.

5. COMMUNICATION TEAM (Security CMS)
Roles and Responsibilities:
1. Mainly acts as the central window for communication duringemergency; gather necessary information about an emergency
and relay to the EPR Leader and Department EPR Leaders for
appropriate decisions and actions; and responsible for
protecting company properties against trespassing, vandalism,
theft and other forms of malicious mischief by employees oroutsiders during emergencies.
2. Conducts patrols, security checks and enforcement of plant
rules and regulations.

3. Maintains operation of the teams communication equipment.
4. Monitors CCTV for suspicious activities concerning safety andreports to the Safety Manager and the concerned ManagementTeam.
5. Investigates accidents/incidents of which he/she is a witnessand other incidents he/she is instructed to investigate andprepares report for submission to management.
6. Must be familiar with the operation of fire alarm control panel.
7. Answers Emergency hotline telephone located in the Security
Office and relays immediately to the SecurityManager/Supervisor, Security-In-Charge/DetachmentCommander for coordination with the OMR and/or Deputy OMRwhoever is easily accessible.

10. FIRST AID TEAM

injured victim(s) or sudden illness before service of
physician/doctor can be obtained.
2. Assists rescuers in locating trapped or injured persons and
moving them to a safer place and administer on the spot first aidtreatment.
3. Bring injured/patient to the Clinic or hospital, if necessary.

11. SEARCH AND RESCUE TEAM
Roles and Responsibilities:
1. Responsible for searching people in areas like toilets or locker rooms
where they might not have heard the alarm during emergency drills oractual emergencies.
2. During actual emergencies, he/she looks for people who may havepanicked and became hysterical and fainted, injured, or trapped and movesthem to a safer place where they can be taken cared of by the first aiders.
3. Coordinates closely with the first aiders.

12. EVACUATION AND CROWD CONTROL TEAM
Roles and Responsibilities:
1. Shall be mainly responsible for the orderly exit, assembly and
accounting of all persons in their area.
2. Saves as much valuable company properties as possible
during the emergency, without putting himself/herself in any
imminent danger.
3. After the emergency, salvages other equipment that may still
be of use.

Roles and Responsibilities:
1. Directs the evacuation of all personnel in his assigned shift andarea.
2. Receives instructions from the Dept EPR Leader or Deputy DeptEPR Leader or Dept Safety Officer and relays such instructions tothe First and Last Man Out in case of drills or actual emergencies.In the absence of the Dept EPR Leader or Deputy Dept EPRLeader or Dept Safety Officer and Superiors, he makes decisionrelevant to the safety of affected employees within his/her area ofcontrol.
3. In actual evacuation, sees to it that the instructions are beingexecuted effectively by his team.

Captain
Roles and Responsibilities:
4. Sees to it that emergency exit doors in his/her area includingwalkways, doors, stairs, and other escape routes in his/her floorarea are kept free from obstructions at all times and if there besuch, be relayed to the concerned personnel.
5. He is responsible for the evacuation of all occupants in hisassigned area and shift.
6. Waits for the instructions from the Dept EPR Leader or DeputyDept EPR Leader or Dept Safety Officer or Superiors beforeexecuting the planned evacuation procedures and upon receivingthe signal for evacuation, immediately instructs the First Man Outand Last Man Out to execute.

Roles and Responsibilities
First Man Out and Last Man Out
Roles and Responsibilities:
1. Leads his group to a safe place or to the evacuation center or to anassembly area as instructed by the Captain.
2. Sees to it that the movement/march from the area to the stairways/exitdoors is orderly, fast without crowding and at uniform pace.
3. He shall be familiar with the location of all emergency exits in his area ofresponsibility
4. Count-off his/her group and check out if any of his/her colleagues ismissing and inform the EPR Team accordingly.
5. Leads back his group to the building once return to work has beendeclared by the President.
6. He shall be responsible to open exit door nearest his area promptly afterthe first signal and keep open until all occupants have left the area andthen it can already be closed.
7. Should discourage occupants to go back to retrieve things duringevacuation.

13. FIRE BRIGADE TEAM
Roles and Responsibilities:
1. Responds immediately to alarm signals and deploys to fireareas to extinguish or contain fire pending the arrival of otherfire fighting forces and cooperates with such forces upon theirarrival.
2. Responsible not only for fighting and putting out fire but alsoassists in rescue and first aid operations, if qualified to do so, in
case of emergency other than fire. 3. Members should know the exact location of all fire fighting
equipment, fire hoses, fire extinguishers, SCBA and otherrescue equipment.

1. Mainly responsible in containing and collecting spill chemicals andprevent further health and safety hazards.

4. Handle and dispose collected spill and contaminated materials properly.
