MLS Form Instructions 1. You should have received “Instructions”, RENTAL profile

sheets and “Final Questions”.

2. Complete RENTAL profile sheets completely and accurately.

All owners must sign and date where indicated on the forms.

cannot upload your property to the MLS!)

and “Final Questions”.

4. A referral fee is required to be offered on the profile sheets to a REALTOR who refers a tenant to

you. This fee is most often a one time flat fee but can be a percentage of the lease price. (If you offer a

percentage of the lease price, the fee must be paid each month to the referring REALTOR for the

duration of the lease.) By signing the RENTAL profile sheets and “Final Questions” you are agreeing to

pay the referral fee mentioned above.

5. MAIL, FAX or SCAN & E-MAIL the RENTAL profile sheets and “Final Questions”, to the

address, fax number, or e-mail address listed at the bottom of the page.

6. SEND AT LEAST 1 PHOTO. MLS rules REQUIRE at least 1 exterior photo of the front of the

property to be loaded at the time we place your property in the MLS system. DUE TO THIS WE WILL

NOT BE ABLE TO COMPLETE YOUR MLS LISTING UNTIL WE RECEIVE AT LEAST 1

PHOTO.

7. Payment of our $150 listing fee is REQUIRED in advance to

list a rental property. We will not upload your property until

payment is made in full.

When Jack Keller Inc., REALTORS, receives all completed, signed forms and payment, your listing

will be processed and uploaded into the MLS system. After your property is uploaded we will send you

a copy of the actual complete listing from the MLS system. Please complete the page at the end of these

forms to notify us how and where you would like to receive a copy of your listing.
