INTRODUCTIONThe 2020 MLHD Excellence Awards celebrate the achievements of individuals and teams for their pursuit of excellence and commitment to improving the lives of others.
Awards will focus on achievements that reflect the MLHD vision and priority areas. The focus is on wellness and seizing every opportunity to work in partnership with consumers to build and maintain wellness. The objective is to enable MLHD to be the best place to work, lead, practise, volunteer, teach, learn and grow.

PHOTOSYou will be asked to provide photos of your nominee or an image that represents your nomination.
SHOWCASEAward winners may be asked to present a showcase of their work at the Annual Public Meeting later in 2020.

If you wish to nominate using a hardcopy nomination or require any assistance in preparing your nomination, please contact:[email protected]
Please consider the following when nominating for these awards.
• Which category does the nomination best fit (you may nominate in more than one category)
• What specifically has the nominee done to address the criteria • How does the nominee meet the NSW Health CORE Values• Do you have examples/evidence that can be attached to support your
nomination• If you are nominating a team/partners – you will need to provide names and
contact details for all the team members/partner organisations• You will need to describe or list the benefits to patients/staff/community etc• Consider what the nominee has done that distinguishes them over and above
others
JUDGINGNominees from the 2020 MLHD Excellence Awards will be reviewed by a panel of judges. Award winners will be announced in September 2020.
The judging panel will shortlist the nominations and only finalists will be invited to attend the Awards Presentation evening.
The judges’ decision will be final.
ELIGIBILITY FOR OTHER AWARDS
Nomination received through the 2020 MLHD Excellence Awards nomination process will be assessed for compatibility against awards criteria and eligible applications will be selected to go forward for the following awards:
• 2020 Executive Director Nursing & Midwifery Leadership Award
• NSW Health 2020 Excellence in Nursing & Midwifery Awards
• NSW Health 2020 Quality Awards
• Premier’s Awards for Public Service
AWARD CATEGORIES
Aboriginal & Torres Strait Islander Leadership Award
In recognition of outstanding achievement by an Indigenous staff member or team
Collaboration Award Awarded to a team in recognition of an outstanding partnership, team spirit, or commitment to developing relationships
Excellence Award Awarded to an individual or team in recognition of their pursuit of excellence in any field
Resilience Award Demonstrated innovation in the face of adversity given the challenges our District has encountered in 2020.
Quality & Safety Award An individual or team who demonstrates commitment to continual improvement and an unyielding focus on quality and safety in caring for patients, residents and/or communities
Focus on Wellness Award In recognition of an individual or team who seizes every opportunity to work in partnership with consumers to build and maintain wellness.
Our People Our Future Award In recognition of an individual or team who showcase MLHD as the best place to work, lead, practise, volunteer, teach, learn and grow.
Diversity Award Awarded to an individual or team who demonstrated best practice or innovation to achieve better health outcomes for people from diverse backgrounds
AWARDS CRITERIA
Mandatory CriteriaAll nominations should • Include at least one member of staff from MLHD• Address the criteria for the award category• Reflect the activities of the previous 12 months• Be for nominees who are role models/ambassadors for MLHD• Demonstrate the CORE Values of NSW Health • Showcase best practice
Aboriginal & Torres Strait Islander Leadership Award In recognition of outstanding achievement by an Indigenous staff member or team
The nominee:• Is passionate about providing and improving health for Aboriginal people• Has achieved outcomes towards closing the gap for Aboriginal people
Collaboration Award In recognition of partnerships that achieve mutual success as a result of collaboration
The partnership / collaboration:• Works with internal or external stakeholders or partners to achieve
mutually beneficial outcomes • Demonstrates outstanding partnership, team spirit, or commitment to
developing relationships • Focuses on addressing priority areas • Contributes to building the awareness of collaboration by sharing
examples of best practice and their results, inspiring similar excellence in others
Excellence Award Awarded to an individual or team in recognition of their pursuit of excellence in any field
The nominee• Demonstrates excellence in their field• Assists MLHD in meeting or exceeding the expectations of consumers in
their care setting• Generates and proactively implements better ways of working
Resilience AwardDemonstrated innovation in the face of adversity given the challenges our District has encountered in 2020.
The nominee: • nominee initiates and leads activities which enable change:• seizes opportunities and tackles challenges
Quality & Safety Award An individual or team who demonstrates commitment to continual improvement and an unyielding focus on quality and safety in caring for patients, residents and/or communities
The nominee• Demonstrates an unyielding focus on quality and safety• Delivers innovative approaches to improving patient safety• Engages patients in approaches to improve safety• Demonstrates behaviour that puts patient safety first • Demonstrates outcomes for improving patient safety
Focus on Wellness Award In recognition of an individual or team who seizes every opportunity to work in partnership with consumers to build and maintain wellness.
The nominee• Partnership supports people and communities to learn about, and
become responsible for improving their own health and wellbeing.• Partnership demonstrates sustainable outcomes
Our People our Future Award In recognition of an individual or team who showcase MLHD as the best place to work, lead, practise, volunteer, teach, learn and grow.
The nominee• Fosters culture that reflects the NSW Health CORE values • Encourages leadership and innovation• Puts the patient at the centre of care• Showcases MLHD as the best place to work, volunteer, teach, learn and
grow
Diversity Award Awarded to an individual or team who demonstrated best practice or innovation to achieve better health outcomes for people from diverse backgrounds
The nominee• Recognises groups in the population, including those who are Aboriginal,
aged, culturally and linguistically diverse (CALD), have a disability or Mental Health condition
• Demonstrates coordinated and integrated care for populations with the greatest needs, recognising that health and social outcomes are interdependent
