Mino Ode L i t t l e T r a v e r s e B a y B a n d s o f O d a w a I n d i a n s S D P I H e a l t h y H e a r t P r o j e c t Dear Tribal Community Member; July 20, 2011 We would like to take this opportunity to introduce Mino Ode (Healthy/good heart) program. This is a new grant called the Special Diabetes Program for Indians Healthy Heart Initiative and its purpose is to determine if a program in our com- munity to prevent cardiovascular disease in people with diabetes is helpful. In 2004, Congress funded the Indian Health Service to give funding to selected Indian Health programs to demonstrate that implementation of activities in Indian communities to reduce the risk of cardiovascular disease in our people. We are now in the enrollment phase, and are looking for people with diabetes who are interested in decreasing their risk of heart disease. You may already be aware that diabetes and heart disease are very serious health problems for native peoples, and diabetes is a major risk factor for heart disease. This includes heart attacks and strokes. Studies have shown that your risk of developing heart disease can be reduced by controlling your blood pressure, weight, cholesterol levels and by not smoking. These are risk factors that are in your control. Mino Ode offers intensive case management of your diabetes that includes education on heart health, weight loss, healthy eating and a physical activity program that is geared to your personal needs, as well as, the needs of your family. It also means that we will do whatever we can or need to do to help you decrease your risk factors. We will support each other on this journey to wellness. This includes a physical exam, lab tests, baseline ECG-checks out your heart to learn as much about your health as we can. You will also receive health education, in a group or as an individual, and participate in program activities. We will work together to develop a wellness plan that is specific for you (and your family). We will get there one step at a time, together! By joining the Mino Ode, the SDPI Healthy Heart Initiative you can: Reduce your chances of getting heart disease Help your family learn new ways to live in balance-healthy eating active living Receive professional medical and nutritional advice from people that truly care about you and your family. Be a part of the study that will set the gold standard for cardiovascular disease prevention all over the world. We will share what we learn with other nations and the rest of the world!. FYI-participation in a program in a facility out side of Mina Mskiki Gumik is estimated to cost $5000! There is no cost to you. We will even help you with food or gas.
