4.5.3 Failure. One or more failures in any of the application examina-tions or tes=l be cause for refusal to grant qualification.

4.5.4 Retention of qualification. To retain qualification, the con-tractor shal1 forward a report at sanpling intervals specified in 4.6.3.1.1 tothe qualifying activity. The qualifying activity shall establish the initialreporting date. The report shall consist of:

a. A sunsnaryof the results of the tests performed for inspectionof product for delivery, group A, indicatingas a minimum thenumber of lots that have passed and the number of lots thathave failed, shall be submftted every 12 months. The resultsof tests of all reworked lots shal1 be identified and accountedfor.

----- b. The results of testsperformed for periodic inspection group B, ,including the number and mode of failures, shal1 be reported. The test report shall, include results of all periodic testsperformed and completed during the sampling intervals of4.6.3.1.1. If the test results indicate nonconformancewithspecificationrequirements, and corrective action acceptable tothe qualffying activity has not been taken, action may be takento remove the failing product from the .qualified products 1ist.

Failure to submit the report within 30 days after the end of each samplingintervalmay result in loss of qualificationfor the product. In addition tothe periodic submission of inspection data, the supplier shal1 intnediatelynotify the qualifying activity at any time during the sampling interval thatthe inspection data ‘fndicatesfailure of the qualified product to meet therequirements of this specification.

In the event that no production occurred during the reporting period, a reportshal1 be submitted certifying that the company still has the capabilities andfacilities necessary to produce the item. If during three consecutivereporting periods there has been no production, the manufacturer may bereQuired. at the discretion of the aualifyfng actfvity, to submit a repre-seritativeproduct ofification inspection
