Cholesterol has historically been used as the standard indicator for cardiovascular disease, being classified as HDL (good) or LDL (bad). However, it is actually the lipoprotein particles that carry the cholesterol throughout the body, not the cholesterol within them, that are responsible for key steps in plaque production and the resulting development of cardiovascular disease.

Why is it important to know lipoprotein numbers?Cardiovascular risk increases with a higher number of low density lipoprotein (LDL) particles, regardless of how much cholesterol each lipoprotein particle contains. This is because both a higher number of LDL particles or smaller sized LDL particles dramatically increase the probability of lipoproteins penetrating the arterial wall, where it can cause serious damage.

Why measure lipoprotein subgroups?Measuring the lipoprotein subgroups is the only way to evaluate new risk factors, which is crucial for accurate assessment of cardiovascular risk and treatment, according to the National Cholesterol Education Program (NCEP).

Over 20% of the population has cholesterol-depleted LDL. This is a condition in which a patient’s cholesterol may be “normal,” but their lipoprotein particle number and hence their actual risk would be much higher than conventional cholesterol tests would indicate. This is especially common in persons whose triglycerides are high or HDL is low. In a cholesterol-depleted patient, there can be up to a 40 percent error in risk assessment in a standard lipid panel.

Use LPPTM in your practice for accurate cardiovascular risk assessment. With LPPTM, a physician can begin to treat patients with atherogenic lipoprotein profiles before overt dyslipidemia becomes apparent.

NCEP Emerging Risk Factors � Small, dense LDL – these atherogenic particles easily penetrate the arterial endothelium causing plaque.

� RLP (Remnant Lipoprotein) – very atherogenic lipoprotein that doesn’t need to be oxidized to form plaque.

� Lp(a) – a prothrombotic small, dense LDL

� HDL2b – positively correlates with heart health because it is an indicator of how well excess lipids are removed.

Q. Why do I need the Aspirin Response Test ?A. Medical studies indicate that not everyone benefits equally from low dose aspirin. In fact some

people have been reported to be aspirin “resistant” but increasing studies indicate that it may be

a question of aspirin therapy “failure”. Because aspirin is known to reduce the risk of heart attack

and stroke by helping stop platelets from sticking together, the Aspirin Response Test allows the

doctor to determine if you are benefiting optimally from your aspirin regimen.Q. What are the chances that my platelets are not responding to aspirin?

A. According to reports in the medical literature, 10% to 40% of patients taking aspirin were not

receiving the desired result. The fact is that even if aspirin is working on your platelets, there are

other factors that may overcome the benefit of the aspirin. Traditional platelet function assays can

not detect this result.Q. What are the dangers to me if I am not benefiting from my aspirin?

A. Outcome studies have demostrated that patients with a poor response to aspirin therapy have

a higher incidence of second heart attacks, strokes or even death when compared to patients with

a good response. Q. Why shouldn’t I just take more aspirin?A. Increasing the amount of aspirin you take may cause an increase in unwanted side effects such as

stomach discomfort or gastrointestinal bleeding. More importantly, the aspirin may be doing its job

but there is some other factor overcoming its benefit which may require an additional medication.

Only your physician should decide if it is safe to change your aspirin dosage.Q. How often should I be tested?A. The optimum frequency of testing has not been established. Clinical studies indicate that certain

increases in cholesterol. Q. What does the Aspirin Response Test measure?A. When an individual’s platelets become activated they release a chemical called Thromboxane A2

(TxA2) which causes more platelets to become activated and stick together. This may

cause formation of a blood clot. The ART test measures a metabolite of TxA2 called

11-dehydro thromboxane B2 which is excreted in the urine. When an individual is benefiting

from their aspirin therapy the level of 11-dehydroTxB2 in the urine is very low.Q. Is the Aspirin Response Test complicated?A. No. This test is performed on a random urine sample. It is important that you be able to give your

doctor a thorough history of the other drugs you have taken during the past 10 days, especially

other non-steroidal anti-inflammatory drugs. Be sure to mention any dietary supplements and

alcohol intake. Note the amount and frequency of aspirin usage.

Q. Do all NSAID’s interfere with aspirin binding?

A. To date, only Ibuprofen has been proven to interfere with the binding of aspirin to the

platelet which prevents acetylation of the COX-1 enzyme.

Q. How soon after taking NSAID’s is it safe to take aspirin for heart benefit?

A. Again this has been addressed only in regard to Ibuprofen. It is presently recommended that

the individual must wait at least 8 hours after ingesting Ibuprofen before ingesting aspirin or at

least 30 minutes after aspirin ingestion before ingesting Ibuprofen.

Q. How long should I wait after a patient has begun aspirin therapy to test?

A. According to the literature, it takes up to 5 days for low-dose aspirin therapy to achieve

maximum reduction in urinary 11-dehydro thromboxane B2 levels.

Q. Does liver disease affect the results?

A. Yes. Liver disease may prevent the metabolism of thromboxane A2 to

11-dehydro thromboxane B2.

Q. Does renal disease affect the results?

A. Yes. Kidney disease may prevent the metabolism of thromboxane A2 to

11-dehydro thromboxane B2.

Q. Does the platelet count of the patient have an effect on the test?

A. To date no studies have been performed addressing a relationship between platelet count

and levels of 11-dehydro thromboxane B2. If there is sufficient aspirin on board to block the

COX-1 pathway, then the individual is benefiting. Platelet count does not necessarily reflect

functional platelets, therefore a correlation can not be assumed. More importantly, if the

individual does not have a high percentage of platelet turnover, a daily regimen of aspirin

should eventually knock out all of the platelets because the effect of aspirin on the platelet is

irreversible and lasts for the lifetime of the platelet.

Q. How much aspirin should I be recommending and what about adding Plavix?

A. Within the guidelines for treatment with aspirin, the physician may administer doses from 81

to 325 mg. It is completely up to the physician to recommend any change in aspirin dose based

on the individual’s circumstances. Adding Plavix to a patient’s anti-platelet regimen is strictly

up to the physician and depends on the clinical condition of the patient. Certain studies have

demonstrated an added benefit with Plavix in addition to aspirin in symptomatic patients;

however, asymptomatic patients have not always demonstrated an added benefit.
