A person has a problem, for which she/he is seeking help from social and health services

The person has any kind of need to seek services or she/he is already getting services.

She/he does not know about the ICanFunction solution.

A person does not always recognize her/his service needs. Then her/his family members or friends can help and act as a proxy for her/him.

Social and health care worker tells that there is an ICanFunction app.

A social or health worker suggests the person to use the ICanFunction app. She/he can also find it her/himself.

The ICanFunction app helps the person to describe her/his own life situation and use it at the encounter. It is also helpful when the person needs to tell the story again for other workers.

Marketing should be focused to users, so that they find the solution and feel that it’s worth it.

The user downloads the ICanFunction app

This phase requires activity from the user.

It is easy to find and take into use.

The user downloads it from Appstore, Play shop or via a QR code.

The user registrates to the ICan Function app

The app allows for the choice of a self report or a proxy report.

The user or her/his family members or friends register or sign in the user.

Only by signing a consent, user can authorize another person to act on her/his behalf.

The person starts to use the Ican Function app.

The person defines her/his needs and selects issues that are important for her/his current life situation.

The ICanFunction app helps the person to tell about meaningful issues, add photos and do simple self-tests.

User will be motivated to use it only if the app is reliable and easy to use.

The person adjusts from settings, how she/he wants to use it

From settings the user can adjust timing and frequency of pop-up messages. She/he can also select how an assessment scale is visualized.

Remind messages sould be easily switched off.

Pop-up messages remind the person to continue self-report. Vizualizations of assessment scales include e.g. of scales with number or text and smileys.

The person continues self-reporting

The person returns to the app after getting a remind message or remembers her/himself to continue self-reporting.

The person can continue to do the reporting in several days from the same point she/he left it. New assessments of the same issue make it possible to monitor progress over time.

The continuing self-reporting should be easy and simple. A person should be able to do it with a little effort only.

The person views her/his own data

The person can see the self-reports on her/his phone.

The app allows the user to view and modify his/her own data on the phone.

In future, the person could send her/his self-reports directly to national PHR.

The person shares self-reports where she/he wants to share them

The user can share a summary as she/he wishes in preferred sharing channels.

Now she/he can print a pdf file or show a self-report summary on her/his phone to social and health service providers.

The ICan Function App isn’t yet connected to the national information systems. The sharing modalities will be reliable and so clear, that the person is sure to avoid sending anything to the wrong place.

The person discusses her/his self-reports with a social or health worker

The self-report can empower the person to discuss issues that are meaningful. She/he does not need to tell the same issues over and over again. The worker needs to document less data.

It is possible for both the person and the worker to utilize the saved information from a self-report.

There is a risk that the social and health care worker cannot or will not utilize person’s self-reports.

The person continues to use the ICanFunction app

The person continues to describe her/his life situation on issues important to her/him according to her/his needs.

She/he can make new self-reports to follow up, if she/he needs to visit the social or health worker more than once.

The person needs to feel that it is useful to make self-reports. That motivates her/him to continue doing it.
