1 Purpose and Scope This document is designed to allow each person in the organisation to understand what they are responsible for when planning, designing, constructing, operating, maintaining or disposing of the processes covered by the standard. The assessment covers all requirements of the standard for Senior Managers, Managers, Team Leaders and Team members in the Operations, Maintenance and Support functions. By using this questionnaire, each person should be able to judge whether they have adequate skills, tools or written procedures to safely carry out their responsibilities for Safe Working at Height. The questionnaire is compiled from the Major Hazard Standard and the audit protocol. This self-assessment has no mandatory requirement to be used and how each site uses this questionnaire is to be decided by the site.

2 How to use the Assessment The questionnaire is divided into separate sections. Each section is designed for a different role and function. These are explained in roles and functions below, together with a selection of examples. Steps:

• First: Select the section of the questionnaire that applies to you; • Next: Read through each question answering with a Yes, No or N/A (Not Applicable); • Finally: Add comments relevant to the question and your circumstances.

3 Roles and Functions The questionnaire is divided into different roles and functions. The aim has been to develop questions that explore how you are complying with the requirements of the standard while taking into account the different responsibilities and activities that are present on each site. Each site will need to define where their management structure fits with the generic roles as listed below. When this has been achieved, all personnel on site will understand which section of the questionnaire applies to them. Roles are defined as follows:
