R: Mitch, please take a minute to tell us more about your back-ground, how you became interested in the connection betweenhormones and menopause, and what inspired you to founded Balance 3H Plus® Medical Weight Loss Centers, Inc.

M: Flashback to late Fall 2012. My daily practice was toattend lectures by leading weight loss specialists. That experience presented me with an early understanding ofthe drastic changes that a lot of women go throughanatomically during menopause, and in particular, theweight gain women experience during their midlife.

R: So how did you take this important issue and yourideas and turn it into a business reality?

M: The facts I gleaned from the lectures really made an impression.There is so much misinformation about menopause, much of itcoming from less informed practitioners, the press, and drug com-panies. This group of women needed help. It’s estimated that 6,000American women enter menopause every day. It occurred to methat the most underserved area of medicine is the realm ofmenopause management, and I sensed a strong entrepreneurial opportunity. And inevitably, my natural inclination was to developa system targeting this demographic. This is where the concept ofBalance 3H Plus® Medical Weight Loss Centers, Inc. was first born. I continued my research on menopause from the integrative

perspective and it became clear that the symptoms and physicalchanges women experience in relation to perimenopause andmenopause are often distressing, and frequently have a negativeimpact on their quality of life. I ascertained there was a demand in the marketplace for natural

supplements that would alleviate symptoms and deliver targetedmicronutrients that a woman was no longer getting exclusivelyfrom her diet during menopause. That was my admission ticket into the brave new world of

health and wellness. It was at that moment I decided to move for-ward and open an integrative wellness center that focused onwomen’s health during menopause and specifically address theweight gain associated with menopause.

R: It is known among weight loss and hormone specialists thatyou have quoted "Your eyes are the windows of your soul: YourHormones are the doorway to your health." Can you please ex-pound?

M: Of course. Balance 3H Plus® Medical Weight Loss Centers, Inc.receives hundreds of calls a week from women going throughmenopause asking, “how can I lose weight and keep it off?" Our response cannot be accurate unless we know what yourhormone levels are. The completion of our exclusive and mostcomprehensive female hormone panel, "Health 365" allows us tocorrectly identify and reset "naturally" the hormones that are mak-ing you fat. When it happens, thisweight gain usually occurs around our midsection and can there-fore elevate our risks for various undesirable health conditions suchas metabolic syndrome, Type 2 diabetes, and heart disease. As a functional nutritionist, I know for a fact that weight gainneed not be a natural aspect of aging and there are many differentways to maintain a healthy weight as you age. Leptinhas the ability to shut down your appetite and increase your calo-rie-burning process. Leptin decreases during menopause, and thedecline in leptin production is the biggest predictor of weight gain.CORTISOL is the stress hormone that is elevated when you becomeparticularly sensitive to stress. Estrogen and progesterone areANTI-CORTISOL hormones, without which, the menopausal fe-male becomes acutely susceptible to stressors.

When these hormones are working well, you’ll eat until youdon’t require any more food, and you naturally stop. Your brainwill hear your hormones signaling loudly and clearly. Why does this happen? Well, there are a number of reasons.One of the biggest is that people often view adiet as a short-term solution, and don't really changetheir behaviors. That’s why our Balance 3H Plus®hormone balancing and weight loss program focuses

Another reason is that our bodies have appetite-and weight-regu-lating mechanisms that try to maintain homeostasis––in other words,to keep things the same over the long haul. HORMONAL CONTROL OF APPETITE AND FATThree important hormones that shape our appetite, hunger, and fatstorage are leptin, ghrelin and cortisol. Leptin and ghrelin are believed to play a significant role in regulating appetite, which consequently influences weight and bodyfat. When we get hungrier, we tend to eat more. When we eat more,obviously, we maintain body weight or gain that weight back. Both leptin and ghrelin are peripheral signals with central effects.In other words, they're secreted in other parts of the body (peripheral)but affect the brain (central). Both hormones respond to how well-fedyou are by sending signals to your hypothalamus––a part of the brainabout the size of an almond. Cortisol is known as the stress hormone. It is part of a complexstress-response system stemming from the human body’s early evolutionary needs––one of which was to store extra fat in times ofstress, often associated with periods of hunger. That response has outlived its usefulness, and in modern times, it can get in the way ofour weight loss efforts. Obesity can interfere with these hormones andtheir signals.

LEPTINBack in 1994, researchers noticed that one genetically alteredstrain of gene was responsible for appetite control. Soon afterwards,nearly everybody who was interested in fat research was doing research on leptin. At the time, this appeared to be the holy grail of obesity research: a protein that made really, really fat mice into skinnymice. There was a lot of excitement surrounding the idea that bysimply boosting leptin levels with pills or injections, people wouldbe able to eliminate their excess fat, just like the mice. Well, like most things in biology, leptin is more complicatedthan that. As it turned out, leptin injections only worked on mice––and people––who were genetically leptin-deficient, which is onlyabout five to ten percent of obese subjects. The other ninety toninety-five percent were out of luck.

HOW DOES LEPTIN WORK?Leptin is made by adipose tissue or fat. Generally, the more fat youhave, the more leptin you make. Leptin is secreted from the fatcells into the circulatory system, where it travels to the hypothal-amus. Leptin decreases hunger by telling the hypothalamus thatwe have enough fat, so we can eat less, or stop eating. Leptin mayalso increase metabolism and help burn calories, because the lessfood you eat, the higher your metabolic rate. Basically, for weight loss, the more leptin the better.

LEPTIN RESISTANCEYou'd think, then, that people with higher body fat would just nat-urally stop eating and start losing weight once their leptin levelswere high enough to signal the brain that the body has stored a sufficient amount of fat. Unfortunately, you can become leptin re-sistant. When this happens, you can have a lot of fat, making a lot ofleptin, but it doesn't work. The brain isn't listening. No drop in appetite. No increase in metabolism. Your brain might even thinkyou’re starving, because as far as it is concerned, there’s not enoughleptin. So it makes you even hungrier. It's a vicious cycle. Leptin resistance is similar to insulin resistance, and they alsoshare common signaling pathways. Insulin resistance occurs whenthere is a lot of insulin being produced, for example, with a diethigh in sugar and simple carbohydrates, but the body and brainhave stopped listening to insulin’s signals. Similarly, fructose seemsto induce leptin resistance. Interestingly, both types of resistance seem to occur togetherin obese people, though obese men who tend to have more internalbelly fat, or visceral fat, have higher insulin levels, while obesewomen, who tend to have more fat under their skin, have higherleptin levels. There are a few possible explanations for how leptinresistance actually works. One theory is that leptin can't get to thehypothalamus, because proteins that are needed to transport itacross the blood brain barrier aren't present. Regardless of the mechanics, the important point is that past acertain level, having more body fat can interfere with those signalsand actually make you hungrier.

GHRELINGhrelin was discovered seven years after leptin. But after the leptinletdown, there was much less fanfare. Both hormones help to regulate appetite, but in different ways. Like leptin, ghrelin is released into the blood, crosses the blood-brain barrier, and arrives at the hypothalamus, where it tells thebrain you're hungry. But there’s a difference: Leptin is a hormonethat is a result of stored body fat, so it's a long-term regulator.Meanwhile, ghrelin regulates appetite in the short term: Your stomach makes ghrelin when it's empty. Ghrelin is high before youeat and low after you eat. If you want to lose weight, you want less ghrelin, so you don'tget hungry. When these hormones aren’t getting the right signals to the hypothalamus, this obviously presents a challenge for folks tryingto lose weight and keep it off. Faulty appetite signals can lead tothe dreaded yo-yo dieting phenomenon.

CORTISOLCortisol is known as the stress hormone. It is released along withnorepinephrine and epinephrine as part of a stress-response thatprotected early humans during times of danger or scarcity. When the stress response is triggered, these hormones work inconcert to give the body fast fuel, by halting insulin production, relaxing the digestive muscles and reducing the blood flow to thedigestive organs. It also directs the body to store extra visceral fat,or belly fat. That primitive response, which worked well to protectour ancient ancestors in fight-or-flight situations, is no longer appropriate for modern stressors. Instead, it can interfere with ourefforts to maintain a healthy body weight. In fact, excess visceral fat, which is concentrated around theabdominal organs, can cause increases in inflammation and insulinresistance, and has been associated with cardiovascular disease,diabetes, and certain types of cancer. Moreover, an enzyme presentin visceral fat causes you to produce even more cortisol, leading toan unhealthy cycle. Managing your cortisol levels can help keep this now unfavor-able stress response in check.

Asyou age, your metabolism tends to decelerate by about 5% for everydecade past age 40. So if your resting metabolic rate was 1,200 calories per day at age 40, it will be around 1,140 at age 50. At age40, to maintain your weight, that is, to not gain weight, you're goingto have to eat 100 calories less a day. If you are over consumingjust 100 calories a day, you can gain 10 pounds in a year. The mathematical equation is 100 calories a day times 365 daysin a year equals 36,500 calories. Now divide that number by 3500calories––which is one pound of fat––and you get 10.42 poundsgained in a year. The point is, just by adding an extra 100 caloriesa day, or lowering your activity by 100 calories, or losing muscle,can add a 10-pound gain. Combining that with metabolic syndrome, menopausal symptoms, and an imbalance of naturallyoccurring hormones––cortisol, ghrelin and leptin––will certainlylead to weight gain.

But metabolism is really only a small part of the story. As weage, our lives become more complicated, whether it's with children,work, or aging parents, and so we have less time to be more physically active and pay attention to what we're eating. Food isavailable 24 hours a day, seven days a week, in large portions thatare relatively economical. So food is always around, and we tendto have more mindless eating and cut down on activities. Recentresearch suggests that women, on average, will lose muscle masstwice as fast as men the same age, and that can make a huge difference in their ability to lose or at least maintain weight.

Acommon question I hear from women coming to me forhelp with weight loss is, “what does it mean when yourbody is suffering from inflammation?” Inflammation is acondition that I believe deserves explanation, as it is often

an overlooked factor in women’s struggle to lose weight. It seems as though everyone today is tossing around the word“inflammation”, but few are clear on what it means. I define it asan intense immune response in your body that causes an increasein systemic inflammation. The primary and classic responses tothis are redness, heat, pain and swelling inside or visibly on theoutside. For example, when you get stung by a bee you swell andturn red at the site. This is a natural and helpful inflammatory response. Your body releases a cascade of immune protectingchemicals, which rush to the site to rescue you from further damage. This type of inflammatory response is supposed to helpyour body heal from invaders such as toxins, bacteria or a virus. But, when your immune system is working overtime because ofexposure to chronic stressors, such as lack of sleep, over exercise,foods which contain inflammatory compounds and fats (i.e. dairy,sugar, gluten and corn) – you’ll likely experience the negative sideaffects of low-grade inflammation. One side effect of unmanagedinflammation is unwanted weight gain and weight-loss resistance. Inflammation can pack on the pounds several different ways. Itmakes your body resistant to key chemical messengers that helpyou manage stress, burn body fat and normalize your hunger andcravings (i.e. ghrelin and leptin hormone levels). A great example of inflammation causing chemical resistanceis with your key stress hormone, cortisol. When your body doesnot hear its signal, your levels will continue to rise, stressing youout even more, storing belly fat and causing crushing cravings. Elevated levels in cortisol eventually lead to drops in serotonin (afeel good nuero-hormone), so you end up feeling more anxious, less optimistic and restless in bed. Over a prolonged period or duringan extremely stressful event your body may quit pumping out cortisol, which may lead to intense feelings of fatigue, apathy andsluggishness.

Inflammation also causes a chemical resistance to your key satiety hormone, leptin. Meaning your cells aren’t hearing leptin’ssignal that you are full, and it’s time to stop eating. My experience with Balance 3H Plus® Medical Weight Loss Centers, Inc. has been nothing but positive. The Balance 3H Plus® program has dramatically changed the way I feel about myself and given me tools to look

and feel better, and stay healthy. I have lost 8 lbs, but surprisingly my belly went down. My biggest concern was that I be able to feel full, but Balance 3H Plus® was able to do that. I believe the ingredients in the shakes and the vitaminshave reset my metabolism to the point that I do not experience the hunger that I used to have on other diets. My firstmeeting with Jen Dorf, the nutritionist, was amazing. She made me feel comfortable and it was easy to tell her about

my struggles with weight loss and my relationship with food. She gave me a book called The Slow Down Diet that made a big impact on how I eat a meal. The first week was tough. The focus is on cleansing but with the help of the nutritionist, I was able to eat the right foods and snacks to keep me on track. I did not lose any weight the first week.

The second week I began drinking two shakes a day. They were tasty and I substituted almond milk and other non-dairymilks to add to the shakes. My evening meals consisted of lots of vegetables. Cooking vegetables on the weekend,

ahead of time, made it easier to follow the plan during the week. I did not give up coffee but limited myself to two cups a day. The weight started to drop. The lab work revealed a lot of deficiencies that I was not aware of, even my lack

of drinking water showed up in the results. Jen gave me information on the foods that would help me get the vitamins I was lacking. Now I make a conscious effort to drink water and, with a suggestion from Jen, make it tastier by

simply adding cucumbers. I am also enjoying my workouts with Lisa, the personal trainer. She gives me customized workouts that are specifically geared toward my problem areas and pain.
