IRCULAR Group Hospitalization Policy for Executives 04 th April 2012 Dear Colleagues, 1) This is to inform that the Group Hospitalization Policy for Executives has been renewed with an aim to provide the Best of Health Care Services. 2) The Group Hospitalization Policy is renewed with New India Assurance Company Ltd ., for providing hospitalization benefits, which will be effective from 5 th April 2012 to 4 th April 2013 . Medi Assist India TPA Pvt. Ltd will be the Third Party Administrator (TPA) for administering the policy for hospitalization and claims settlement in coordination with respective HR. 3) The policy is applicable to all Executives of Reliance Infrastructure Limited and its associate / group companies. The Hospitalization Policy provides for reimbursement for hospitalization expenses incurred. 4) Executive and his family, including the spouse and parents (when opted specifically, by paying additional premium) would continue to avail the benefits under this policy as long as they are in the Company’s services. In case of resignation of an Executive during tenure of this policy, his name gets automatically deleted from the Hospitalization policy and he ceases to get any further benefits. 5) There is an option to cover the parents under this policy by paying an additional premium by the executive at the rate given in table below. If the option is subscribed by the executive within the specified timelines, the Hospitalization expenses of the parents will also be covered within the above mentioned sum assured. The premium toward parental coverage will be deducted from the salary of the executive in three equal installments. a) Sum insured for the family of the executive (self, spouse, upto 2 dependent children upto age of 24 years and dependent

b) Please note that the parent coverage is NOT automatic and the executive needs to submit the parent’s details in ESS for continuation of the coverage every year.c) Those who are interested in opting for parent cover are requested to submit their request in ESS from 05th April, 2012 to 18th April, 2012. Any request received beyond 18th April 2012 cannot be considered, as the Insurance Company has expressed its inability to include any name

subsequently in respect of existing Executives.6) Benefits to retired executives  / dependant of deceased executives:a) Executives who will retire during  policy tenure, will be continue to be covered in the policy till the expiry of the policy i.e upto 4th April, 2013.This coverage includes Self ,Spouse and Children (upto 24 years), without any additional premium.b) Those executives who are already retired before starting of this policy, if wish to avail the benefit of the hospitalization policy for the year 2012-13 may avail so by paying one time premium at the time of enrollment for the policy at following rate(for Self & Spouse only):

c) In case of death of an executive (while in service) during the tenure of this policy, the policy will cover deceased executive’s spouse and maximum 2 children (upto the age of 24 years) till the policy expires and for the subsequent years if the family wish to avail the benefit they can do so by paying the premium at the rate mentioned above in point 6(b) as applicable to retired executive.d) There will not be any parental coverage in respect of retired executive /OSD and deceased executive.

7) Once the executive leaves the service of the company entire family including parents ceases to be the beneficiary under this policy with immediate effect. 

8) The salient features of the policy are as follows:Focus Area Features

Coverage of the Policy The policy covers the employee, his/ her spouse and two dependent children upto the age of 24 years. The policy covers all illness, Accidents, Pre and Post hospitalization Expenses, Pre-Existing Diseases, Maternity Benefit.

Network Hospitals Medi Assist has covered 4668 hospitals all over India, termed as the Network Hospitals under the policy, given in attached list .

E CARD –ID Card Medi Assist E card could be generated from their website by the concerned employee. The detailed procedure is attached herewith.

Cashless facility at Network Hospitals

Mediassist offers Cashless Service at Network Hospitals listed with them on submission of Pre authorization form. Please note that this is a facility extended by Medi Assist and not part of our insurance policy cover. The cashless facility is given by hospital after AL is authorized by Medi Assist. Mere production of ID card does not entail a member the cashless facility. The cashless facility is given by Medi Assist on case to case basis.

Non-Network Hospitals Executives availing treatment at Non-Network hospitals can send all their original documents pertaining to Hospitalisation along with the claim form to Medi Assist for reimbursement.
