access to at least one internal or external formulary for the entire EHR Reporting Period.
• Stage One Threshold: Attestation• Exclusion: Any EP who writes fewer
than 100 prescriptions during the EHRreporting period

• Exclusion: An EP who orders no lab tests whose results are either in a +/- or numeric formatduring the EHR reporting period

• Exclusion: EP who have no such patient data to provide during the reporting period

• Exclusion: EP who neither transfers a patient to another setting nor refers a patient to another provider during the EHR period.

• Exclusion: EP who administers no immunizations during the EHR reporting period or whereno registry has capacity to receiveelectronically

agencies according to applicable law and practice
• Stage One Threshold: Perform at least on test of certified EHR’s capacity to submit ESS (unless no Public Health Agencies have capacity to receive info electronically
• Exclusion: EP who does not collect/submit any reportable syndromic information on theirpatients
