Domain I: Planning and Preparation Rating Area of Focus Lesson Plans: Teachers must submit 2 sets from bi-weekly submissions of what they believe were their most effective and efficient set of lesson plans during the quarter and submit a short paragraph for each set explaining how the lesson plans were effective as it pertains to student mastery of a particular subject or content area. - Student Assessment: Teachers are to provide 1 copy of a summative, formative and informal assessment used to measure student mastery of a content area or a particular Common Core Standard throughout the quarter. Please label each submitted assessment.- MAPS Scores: If applicable at the time of submission, please submit all student MAPS scores showing data of progress of growth or regression in the content area. Classroom Management: Teachers must submit a copy of all discipline referrals or written documentation

regarding discipline incidents that have occurred within the classroom or on any part of the campus throughout the quarter.

Professional Development: Provide data from any Professional Development that you have received throughout

the quarter. This includes conferences, workshops and internal meetings. Examples include but are not limited to the following: Agendas, Programs, Power Point Presentations, etc.

Staff Meetings: Provide agendas from all staff meetings you have attended throughout the quarter at your building

Leadership: Teachers should provide a written overview of leadership in which you have demonstrated at your

building or in the district during the quarter. ***Note: This could be a bulleted list.
