As a participant in the Spiritual Growth Retreat, March 23-24, 2018, I take seriously my responsibility and affirm my commitment to remain
on the site of the event. I will attend all scheduled activities, sessions, and meals. I will observe curfew by being in my room, being quiet,
and not disturbing others. I understand that anything considered illegal for minors (persons under 18) under civil and criminal law applies
to this event as well. Therefore, I will obey all laws. I will not use tobacco, illegal substances, or alcohol, nor will I possess any firearms,
weapons, or fireworks. I will respect the equipment and property of others and care for the facility which we share. Any damage to the
facility will be the sole responsibility of the person(s) who cause the damage and the parent or legal guardian of each person responsible.
This covenant is made between each teen and the entire group. I understand that if I break the covenant and if the brokenness cannot be
reconciled, that my parent or legal guardian will be contacted and my participation in the event may be terminated.
Parental Consent
I give my daughter, _______________________________________, permission to attend and participate in the Spiritual Growth Retreat,
sponsored by the South Georgia Conference UMW, at Epworth-By-The-Sea, March 23-24, 2018. I am aware of the purpose of the event and

which my daughter has signed, and I understand the responsibilities she has agreed to. I will support her in fulfilling this covenant.
I designate South Georgia UMW as chaperone for my daughter during the South Georgia Conference UMW Spiritual Growth Retreat. This
covers all events on campus and/or off campus.
