Where are QMP Summary Templates?• In Pantry files (as applicable per Region)• From Regional Materials Staff
Materials Engineer Materials Coordinator or IAP Specialist
Locating a QMP Summary Template in Pantry• Open the current year’s Pantry
– Locate and open the “Statewide Forms” folder– Locate the MS Word documents labeled
“XXXXXXXXTemplate”
(where the X’s represent the type of summary being created -
for example: “HMAMixtureSummaryTemplate”)
– Open the relevant template
Completing/Editing a QMP Summary Template in MS Word• Once the template is open, answer each question by
typing in the requested/needed data• Remove the “Yes / No / N/A” that does not apply• Elaborate on any item that may come in to question
during a materials review or audit• For additional guidance, some regions also have
examples of completed summaries in their region specific folders in Pantry
Transferring the MS Word Template into MIT• Once the information has been entered into the
template, transfer of this information to the MIT/MTS is required
• Highlight the whole body of the text with the cursor/mouse
• Right click on the
highlighted area • Select “copy” from
the pop-up
Transferring to MIT (continued)• Open the MIT program• Click on “Create / Edit Test” from the drop down menu
option
Transferring to MIT (continued)• From the pop-up selection box, double click on “155 -
Miscellaneous Materials” or single click & choose “Continue”
Transferring to MIT (continued)• Complete the sample card• In the “Test Type” box choose “V – Verification”• In the “Material” box enter
“QMP __________Summary - ___________”– The first blank indicates the specification– The second blank indicate the details
• Examples: “QMP Base Aggregate Summary – ¾ inch dense” or “QMP Ancillary Concrete Summary - ABC Contractors”

• Next, click on the “Test Data” tab• First left click then right click on the empty body of
the report below the words “Test Description”• Choose “Paste” from
the pop-up box• Minor editing may
need to be done• Save your report
