What happened

A floor man (23 year old) Omani lost his balance when exiting the toilet cubicle on the rig. He felt pain as he twisted his left knee but continued to work. The next day the pain was worse so he went to hospital. Initially they allowed him to return to work with light duties but a further examination 4 days later identified tear in his knee joint which required an operation.

What happened A 24 year old Omani roustabout fractured his right middle finger when he trapped his finger. The assistant driller was using a hoist to lift a Kelly hose but the hose snagged on a bolt securing the hand tools board. He reversed the hoist at the same time the roustabout tried to release the hose manually and his middle finger trapped against the hand tools board .

What happened A 40 year old Well Head Technician (WHT) was manually aligning the corrosive insertion ring while instructing the Assistant Driller to lower the tubing hanger. As the hanger was lowered the WHT right index finger became trapped , he reacted by pulling the finger out causing the amputation of the finger tip exposing the bone.

What happened A 32 year old derrick man had just fixed a small leak in a connection between the shale shaker pipe and the flow line (which was also connected to the BOP). He checked the size of the gap between the two pipes by sticking in his finger. Vibrations from the BOP caused a sudden movement in the pipes, the gap closed and his index finger was crushed.

What happened A 53 year old tipper driver driving an articulated tipper truck laden with sand took a short cut up a steep ramp. He was forced to brake when he missed his gear and the trailer rolled back down the ramp and overturned causing the cab to lift off the ground. The cab dropped back down to earth when the pin connecting the tipper to the cab snapped. The impact fractured the drivers pelvis.

What happened A 31 year old operator was laying out 6” flat rubber hoses which were coiled on a large horizontal reel on a hose deployment unit. The reel jammed as the last part of first hose was nearly uncoiled. The operator managed to free the reel but in doing so the motorised reel began rotating again uncoiling the last loop of the hose. This caused the metal coupling on the end of the hose to fly off the reel at speed and it hit the operators head, killing him.

What happened A trainee assistant driller needed to move a hose to the inspection area and so he hooked it to the forklift using two chains connecting the boom to two end of the hose. The forklift drove to the inspection area and as it lowered the hose one of the chain hooks slipped causing the hose to fall and swing striking him on the foot causing a fracture.

What happened A truck driver was using a chain binder to secure a load on his trailer. He pushed the lever down to lock it in place using a cheater bar but the chain binder had not fully locked and as he removed the cheater bar the lever under high tension swung up violently and hit him in the face causing severe injury to his right eye.

What happened During rigging-up the work floor, the support legs were being lifted from the v-door, by winch using a tugger line. The Derrick man was guiding the tugger line with his left hand. What happened After completing rigging down the top section of the scaffolding, a technician began removing a plank (1m section of scaffolding grating). What happened While trying to fill the hoist diesel tank from a diesel drum on a forklift forks the mechanic climbed over the barrier standing with one foot on the forks and the other on the hoist. As he tried to roll the drum he felt dizzy and fell 1.2m to the ground dislocating his right knee.
