In order to be eligible to begin practice and participate in the annual Ludy Bowl flag football game, you must:

A. Complete the Registration Form and the Waiver and Release Form and return both to the Office of Student Activities (lower level in Student Union, B101) by 12 pm on Friday, September 19th.

B. Read the statements below and initial at the end of each statement indicating that you have read and understand the information.

If you have any medical problems or a condition that could be complicated by strenuous physical activity you should consult your doctor before you begin to practice. __________

If you have asthma, you are responsible to have your inhaler or emergency asthma medication available at all practices and the game in the event of an asthma attack. __________

Columbia College WAIVER AND RELEASE FORM

Ludy Bowl Flag Football Practices & Games

I, (please print your name) _______________________________________, wish to participate in Ludy Bowl 2014, sponsored by the Columbia College Activities Board, to be conducted from September 22, 2014 through October 11, 2014. I understand that running and physical training may be necessary and are strenuous and potentially hazardous activities. I further understand that I should not participate in Ludy Bowl unless I am physically capable to safely participate. I, knowing this, hereby certify that to the best of my knowledge I am physically capable of participating in the Ludy Bowl and have no medical condition that could worsen by participating in this training.

By signing below, I acknowledge that I am fully aware of and appreciate the potential dangers associated with my participation in Ludy Bowl and assume all risks associated with participating in this training program including, but not limited to personal injury, accident or death, personal medical conditions, contact with other participants, the affect of weather including extreme temperatures, humidity, wind or rain, and all risks associated with the presence of vehicular traffic.

I understand that my participation is voluntary, that I may refuse to participate, withdraw my consent and stop taking part in practice or the Ludy Bowl game at any time. Further I understand that I subjectively understand the risks of my participation in this activity, and knowing and appreciating these risks I voluntarily choose to participate, assuming all risks of injury.

Having read this waiver and knowing the above facts and in consideration of acceptance into Ludy Bowl, I for myself, my personal representatives, my heirs and my assigns, agree to hold harmless, and do hereby waive and release and forever discharge Columbia College, its Board of Visitors and Trustees, officers, employees, agents, students, volunteers and insurers (“Released Parties”) from any and all claims or liability that I may have for any and all personal injuries including death, and property loss or damage, whether caused by my negligence, the negligence of the Released Parties or the negligence of any third party. This waiver and release extends to all claims of any kind or nature whatsoever, including claims or suits at law or in equity, whether foreseen or unforeseen, known or unknown.

Ludy Bowl Etiquette Ludy Bowl is an exciting time on campus for current students, alumnae, faculty, staff and guests. The following guidelines have been established to ensure that everyone is able to enjoy this festive occasion.

1. Window painting is not permitted in the residence halls, unless permission has been granted by the Area Coordinator for the building. If permission is granted, it is the person(s)’ responsibility to remove the paint by 12:00 pm on the Monday after Ludy Bowl. Failure to remove the paint by this time could result in fines.

2. There should be no graffiti or defacing of other people’s property. Failure to adhere to this expectation could result in judicial sanctioning.

3. During the game, fans and participants should not be belligerent or use derogatory language towards the players, coaches, cheerleaders, or referees. Good sportsmanship is an important aspect of the honorable behavior required of all members of the Columbia College community.

4. Only last names and numbers may be included on jerseys.
