Why East Harlem?Despite some pockets of gentrification, the level of need in East Harlem remains shockingly high. The neighborhood is still home to some of the most poor and underserved residents of Manhattan.

72%of children in East Harlem are

born into poverty – the highest rate in Manhattan.

1 in 5residents have no health insurance.

73%of emergency room visits are related to asthma –

the highest rate in New York City.

3.5 xmore infants die here than in the

neighboring Upper East Side.

Bringing Wellness HomeLSA services meet the most fundamental needs of families: food, clothing, healthcare, and a safe home. What makes our service model unique is our home-visiting component in tandem with services at our welcoming building on East 115th Street. We provide the immediate care which vulnerable families need in the short term, and a nurturing community that fosters health, strength, and resilience in the long term.

Our programs offer a special focus on families and young children, and emphasize the building of trusting, caring relationships between staff and clients. Home visits by specialized staff, such as skilled nurses, therapists, social workers, and environmental health workers, complement on-site program work.

ADVOCACY AND FOOD PANTRY programs help poor families meet their immediate need for food and address the longer-term issues that keep people in poverty.

PARENTING AND CHILD DEVELOPMENT, formerly called the Home-Based with Socialization program, works with parents to keep very young children (up to age three) developmentally on-track through home visits and on-site socialization groups. The five focus areas of the program are: attachment and healthy separation; language and literacy; exploration and learning; self-regulation and emotional development; and connections to outside resources.

EARLY INTERVENTION offers evaluation and treatment for infants and toddlers with special needs. The program is approved by New York State to provide multidisciplinary evaluations, psychological evaluations, service coordination, special instruction, speech and language therapy, physical therapy, occupational therapy and social work services.

NURSING, through our Certified Home Health Agency (CHHA), provides direct nursing care to patients at home. The program specializes in Maternity Outreach (prenatal and postpartum care and follow-up care to newborns), parenting and breastfeeding classes, and supporting wellness and nurturing relationships between mother and child.

ENVIRONMENTAL HEALTH SERVICES serve asthmatic children in East Harlem whose illness can be improved by mitigating triggers in the indoor air environment. We do this through hands-on home visits by skilled environmental workers, family training and community workshops.

EDUCATION AND YOUTH SERVICES offer after-school tutoring, a reading academy and a summer program to students who are underperforming in English and Math. A Girls Mentoring program builds leadership skills inEastHarlemgirlsentering7th,8th,or9thgrade. We also offer counseling and information about community educational and recreational programming.

PREVENTIVE SERVICES is a support program that strengthens family functioning to avoid placement of children in foster care. Key program components include counseling, referrals for specialized treatment, help with parenting and daily life management skills, and follow-up with education, health care, housing, entitlements, and more, based on the needs of the family.

THE SHARING PLACE THRIFT STORE is East Harlem’s popular destination for those in need of good quality, bargain-priced clothing and housewares. It is a place where anyone with a limited budget can shop with dignity, meet their neighbors, build community, and learn about LSA’s programs.

Programas BásicosLSA Family Health Service

Nuestros programas ofrecen una atención especial a las familias y los niños pequeños, y enfatizan la construcción de relaciones de confianza y cuidado entre el personal y los clientes. Un componente crítico de nuestro trabajo consiste en visitas a domicilio por personal especializado, como enfermeras, terapeutas calificados, trabajadores sociales y trabajadores de salud ambiental, para complementar el trabajo del los programas.

El programa de ABOGACIA Y DESPENSA DE ALIMENTOS ayuda a las familias pobres a satisfacer sus necesidades básicas, como alimentos, y abordar las asuntos que mantienen a las personas viviendo en la pobreza.

El programa CRIANZA Y DESARROLLO INFANTIL trabaja con niños (menores de 3 años) con riesgo de retrasos en el desarrollo. A través de visitas a domicilio y los grupos de socialización, El programa apoya a los padres, que trabajan para mejorar la comunicación entre padres e hijos, el seguimiento del desarrollo de cada niño, y también refiriendo a los niños para servicios adicionales.

INTERVENCIÓN TEMPRANA ofrece evaluaciones y tratamiento por especialistas en el desarrollo, trabajadoressociales,patólogosdelhablay/oterapeutas físicos y ocupacionales para los bebés y niños pequeños (menores de 3 años) con retrasos o sospecha de retrasos en el desarrollo o discapacidades.

SERVICIOS PREVENTIVOS un programa de apoyo a la familia, fortalece el funcionamiento familiar y ayuda a evitar la colocación de niños en hogares de crianza. Los componentes claves del programa incluyen consejería, referidos para tratamiento especializado, ayuda enseñando sobre el cuidado de los hijos y la vida cotidiana, y ayuda con el seguimiento en asuntos sobre la educación, la salud, la vivienda, los derechos y mucho más.

SERVICIOS DE SALUD AMBIENTAL, que forma partedelProgramadeAsmaParaLaFamilia,sirveun elevado número de niños asmáticos en el East Harlem cuya enfermedad puede ser mejorada por la mitigación de los disparadores en el aire del medio ambiente interior mediante expertos trabajadores quienes visitan los hogares, en conjunto con capacitación a las familias y talleres comunitarios.

EDUCACIÓN Y SERVICIOS JUVENILES ofrece tutoría después de clases, una academia de lectura y un programa de verano para estudiantes de bajo rendimiento en Inglés y Matemáticas, así como un programa de mentores para niñas que viven en el EastHarlemyqueentranalgrado7º,8ºo9º.También ofrecemos asesoramiento e información acerca de programas educativos y recreativos en la comunidad.

ENFERMERÍA, nuestra Agencia Certificada de Salud en Casa (CHHA), proporciona atención directa de enfermería a los pacientes en sus casas. El programa se especializa en la Maternidad (atención prenatal y post-parto y los cuidados de seguimiento a los recién nacidos), clases de paternidad y lactancia, y las relaciones que apoyan el bienestar y la crianza.
