What tests are available?

YorkTest Laboratories was set up in 1982 and has designed, developed and manufactured their food - specific IgG enzyme linked immunosorbent assay (ELISA) test since 1998.

Lorisian results and test methods are overseen by our Medical and Scientific Directors.

There is a lot of confusion about the terms food intolerance and food allergy, and the differences betweenthem. Many people think they have a food allergy when their symptoms can sometimes indicate food intolerance instead. Up to 45% of the population suffer from food intolerance* which, whilst not life threatening, can have a negative impact on many aspects of daily life.

When foods and drinks are digested the proteins within them are broken down into smaller fragments foreasy absorption into the body. Larger fragments can pass through without breaking down, and sometimesthe body reacts to them as invaders, attacking them using antibodies called Immunoglobulin (IgG). This cancause inflammation and a wide range of symptoms including headaches, digestive problems, IBS, bloating,itchy skin, joint pain, weight gain and low mood to name a few. *According to the leading medical charity Allergy UK

How do Lorisian food intolerance tests work?

What are food intolerances?

First of all a few drops of blood are taken using our simple finger prick blood collection wick system. Because the finger prick system is so easy to use, the sample can be taken anywhere – at your premises, at the clients’ home, at the gym etc.

The sample is then posted back to our laboratory (in a pre-paid envelope), tested and analysed for IgG antibody reactions to more than 150 food and drink ingredients.

Clear colour coded test results and support pack are delivered back to you that explain your clients’ foodand drink intolerances. This allows you to discuss in detail recommended dietary and nutritional changes - a fast track to an elimination diet that optimises health and wellbeing.

What sets Lorisian apart from the competition?

There are no international standards available for tests that measure food-specific IgG antibodies. The onlyway that a particular food specific IgG test can be shown to be valid is by carrying out clinical trials.

Our food-specific IgG method has had more scientific papers published about its performance than anyother food intolerance test on the market.

The studies include two independent randomised controlled trials; including a unique trial in IBS. In additionthe largest study of its kind, commissioned by the UK medical charity Allergy UK, with over 5,000 participants,76% of which reported moderate to high benefit in their chronic ill health symptoms after using the service.This means that we can make claims about test accuracy and effectiveness using substantive evidence; something that other suppliers cannot do; there are many tests and programmes that are offeredthat do not have any published data available to show how they perform.

False positive results remain the biggest challenge for test providers. We use a proprietary formulation duringthe ELISA (enzyme linked immunosorbent assay) test manufacturing process in order to block non-specific binding (false positive results).

We can therefore provide the maximum benefit and support for your customers by accurately advising minimal dietary changes. We have data sets supporting a reproducibility of greater than 98%. This isachieved by using consistent and controlled manufacturing processes and operating procedures.

Reproducibility performance is supported with regular routine quality control checks and includes ELISAplate to plate checks and sample to sample data. Sample stability claims are also supported.

Support for Dietary Changes

Lorisian results provide a starting point for an elimination diet which can be carried outonce any serious cause of your clients’ symptoms have been ruled out. You can provide advice to your client about making dietary changes; however, it will be up to theclient to make the changes themselves. You can also provide advice about other productsthat may help their symptoms.

Ideally the client will start by removing all the “reaction” foods from their diet but this may not be practical. They should also try and reduce the intake of their “limit” foods.

If it is too difficult to remove all the foods from the “reaction” list then the client should prioritise first those foods that are easy to remove, and those that are nearest to the top of the list.

STEP 3

Identify foods that the client may find harder to remove and substitute.In this results example:-

Remember that these four food groups play a special role in nourishment. Details and alternatives for your client can be found in the enclosed “Guide to Complex Intolerancesand their Alternatives”.

STEP 4

Advise your client to avoid reactive foods and reduce “borderline” foods for at least 3 months and longer if possible.

Remember that many ingredients can be hidden in more complex foods. It only takes a very small amount of a food to trigger an immune response which will prolong inflammation and symptoms.

STEP 5

Reintroduction of foods

Warning

Lorisian is a food intolerance test and NOT an allergy test. If your client suffers classical allergic (IgE) reactions to any of these foods then they MUST NOT eat them. The test willnot identify all the different types of food intolerance, for example it will not diagnose lactose intolerance or coeliac disease.

| It is up to your customer to decide when to try and reintroduce foods back into their diet. If they are feeling a lot better and are used to their new diet then they may want to wait longer than 3 months

| When they do reintroduce foods they should introduce them back one at a time and at intervals of at least a week to see whether symptoms return

| Sometimes foods will need to be limited long term otherwise symptoms return

| Sometimes foods can be reintroduced and symptoms do not return

| Sometimes foods can be eaten in small quantities and not cause problems

Remember that each customer and each set of symptoms is unique. With the tools ofknowing which foods are causing a reaction the customer can start to determine for themselves the best balance of removing the foods against the degree of their symptoms.

Wheat / Gluten Intolerance

Gluten is a protein found in foods processed from wheat and related grains. Gluten gives elasticity to dough helping it rise and keep its shape and often gives the final product a chewy texture.

| Gluten is found in wheat, spelt, kamut, rye and barley.

| Oats contain a similar substance to gluten which is not alwaystolerated by those with gluten intolerance.

| Gluten intolerance is different from coeliac disease.

| Those with wheat intolerance should avoid all products containing wheat, not just gluten-free products.

Wheat / gluten are often used to bulk up and thicken processed foods. They are often found in foods such asmeat products, sauces and processed foods. It is important for your client to check labels for ingredients.

Suitable alternatives which may be bought as flour, grain, seeds, meal or flakes include:-

| Guar gum; a polysaccharide thickening agent made from the seeds of the Guar plant which are taken out of their pods and ground into flour.

| Carob flour; the ground kernels of the carob fruit which can be used as a thickening agent.

| Potato flour; this has a high starch concentration and can be used to thickensoups and sauces, and to make dumplings.

Gluten-free and wheat-free foods are readily available in supermarkets and online and can be recommended toyour client.

Removing wheat / gluten: Keeping a balanced diet

Grains provide carbohydrates, B vitamins, calcium and other vitamins. Whole grains are a good source of fibre andare therefore valuable components of a healthy diet. Your client should try using the alternatives mentioned above.It is possible to have a healthy diet without wheat / gluten but fibre and vitamins should be replaced by eating more fruit and vegetables. A vitamin B supplement could be recommended.

Egg IntoleranceIf your client has an intolerance to eggs this means that they need to avoid not only chicken’s eggs but other eggssuch as those from duck, quail and goose too. This is because there are similar proteins in all eggs. There are manyproducts that may contain eggs. These include bread, pancakes, quiches, desserts, sauces, spreads, cakes, mayonnaise, ice cream and meat products. It is really important for the client to read the ingredients labels tocheck that egg is not present. Usually egg is listed as an allergen if present and so it is usually easy to see from the label.

Removing eggs: Keeping a balanced diet

1 egg equates to about 15% of the daily requirement for protein and so eggs are important particularly for thosethat are vegetarian. Eggs contain vitamins A, B, D and E and also calcium, phosphorous and iron. Other proteinsources need to be eaten if eggs are removed from the diet. These include milk, meat and fish based proteinsand also vegetable proteins such as beans, legumes, nuts and seeds. A vitamin supplement may be recommended.

Milk/dairy intoleranceIf your client has a cows’ milk specific IgG intolerance to milk, their body reacts negatively to dairy proteins. Thisdoes not mean however that they will automatically have the same reaction to sheep or goat’s milk. Lorisian150plus tests for reactions to sheep and goats’ milk as well as cows’ milk to give a fuller understanding of theirreaction to dairy as a whole.

Milk intolerance is different to Lactose Intolerance. If your client has a positive reaction to animal milks, theyshould avoid lactose free milk as well. This is because the reaction involves the milk protein, not the milk sugar(lactose).

Yeast IntoleranceYeast is a living fungus which is used as an active ingredient in many products especially baked products and alcoholic drinks. Bakers use yeast to make dough rise and brewers use yeast to ferment sugar to alcohol.

Yeast can also be present on foods naturally. They often grow on the skins of berries and soft fruits. If they have ayeast intolerance your client may also react to fungus containing foods such as mushrooms, truffles, bluecheeses and malt. If your client has a positive reaction to yeast they should remove yeast and fungus containing foods from their diet and also cut down on sugary foods which can fuel the growth of natural yeastsin the body such as Candida.

Yeast can be found in Marmite, bread, vinegar, ginger beer, gravy cubes, fermented drinks and some vitamin supplements. All alcoholic drinks depend on yeasts to produce the alcohol; however, distilling and filtering will remove most of the yeast. Spirits such as vodka or gin will have the lowest amount of yeast and champagnemethod wines will also be low in yeast. Beers and wines and ciders will need to be avoided.

1 | What is the difference between food intolerance and food allergy?

45% of the population suffer from food intolerance which, whilst not life threatening can cause many different symptoms. Digested foods are usually broken down into smaller fragments for easy absorption in thebody. Sometimes larger fragments can pass through into the blood stream, causing the body to react to them asinvaders, attacking them using antibodies called immunoglobulin G (IgG). IgG antibodies to foods are linked to inflammation in the body. Symptoms can be slow to develop and can take hours or days to appear. Food intolerance can be reversible.

Food allergy is very different; it produces reactions very quickly and can be life-threatening. Food allergies are life-long.

2 | My doctor has carried out an allergy test, why don’t theresults correspond to the results of Lorisian?

The Lorisian test measures the delayed food intolerance reactions in the form of IgG antibodies. Allergy testsmeasure the rapid reactions caused by raised IgE antibodies. Therefore the results do not correspond.

3 | What are the most common allergens?Everyone’s “fingerprint” of reactive IgG foods is different and so it is not possible to generalise. The exact foodtypes will be unique to each individual.

4 | Why can’t the Lorisian test be used to test childrenunder the age of 2?

The immune system is not fully developed under the age of 2. Changes in diet at that age must be supervised asthey may leave the child deficient of nutrients. Lorisian can accept samples from children under 2 if they have areferral from a Medical Practitioner.

5 | Why can’t women take the Lorisian test if they are pregnant or breastfeeding?

Optimum nutrition during pregnancy and breastfeeding is vital for mother and child. To reduce any possible riskto the mother or baby, it is Lorisian protocol not to offer tests or treatments during pregnancy and breastfeeding.In addition the test has not been validated in pregnancy.

6 | Why can’t the Lorisian test be taken if the client is taking steroids or immunosuppressant medicines?

Steroids and immunosuppressants suppress antibody production. Hence they may give false negative test results. The extent of this depends on the dose taken and the length of time for which the drug is used. The testcan be carried out if steroids are used for greater than 6 months and at a stable dose as the body’s immune response will be “reset” at a lower level than usual and this becomes the “normal” for that person.

7 | Why do you have to wait to take the Lorisian test if you are taking steroids but not if you are taking antihistamines?

Antihistamines do not affect IgG antibody production.

8 | If a client has been eliminating a food already, should it be reintroduced before taking the Lorisian test?

If a client has been excluding foods prior to the test then, even if the client has an obvious reaction to them, theymay not show up as a positive result in the test. We would NEVER recommend that the client introduces any foodback into their diet that they know they react to, or to change their diet prior to taking the test. It may be worththem noting what their typical diet is prior to the test, and after the test ensure they do not add foods to their dietthat they don’t normally eat.

9 | How long should foods be eliminated for?This depends on the severity of symptoms. Normally foods would be eliminated for around 3-6 months; with aparticularly bad reaction this may be for longer and some people have to eliminate their problem foods for manyyears. The vital thing to consider when reintroducing foods that have been excluded from the diet for any significant amount of time is that foods should be reintroduced carefully and in a planned manner. Make sure theclient has plenty of time to do this and is aware to leave at least one week between each food reintroduced andthe next.

10 | Should the client repeat the test after they have changed their diet?

Your client should repeat the test; if they fit the following criteria:

| If they are reacting to any new foods and want to identify and eliminate these from their diet| If they have started to reintroduce food to their diet and are still reacting to them| Regular testing can help you keep a check on reactive foods.

11 | The test shows reactions to foods that have not been eaten. How is this possible?

Some types of proteins are present in more than one food type. In addition many foods contain “hidden” ingredients that may be eaten without realising it.

12 | What should the client do if they crave one of their “Reaction” foods?

Those with food intolerance often crave the foods that they are intolerant to. If they can resist the temptation for afew days then the craving should reduce and they will find it easier to avoid the food in future.

13 | What happens if the client reacts to citrus fruits? Do they need to avoid citric acid?

IgG antibodies can only be formed by reactions to proteins. Citric acid is a chemical and not a protein and so whilethey will need to remove the particular fruit(s) from their diet they will not need to avoid citric acid.

14 | There has been a request to provide another blood sample. Why is this?

Every laboratory test that is processed by Lorisian includes individual checks (quality controls) to ensure that theresults that are provided are reliable. On rare occasions these fail to pass Lorisian’s stringent standards. In thesecases Lorisian has to ask for the client to provide another sample.

15 | What is the sensitivity and specificity of the Lorisian tests?Sensitivity and specificity are measures that show how close a diagnostic test is performing compared with the“True” result. This “True” result is usually determined by a Gold Standard method. For food-specific IgG tests there isNO Gold Standard method. Lorisian have shown that their food-specific IgG tests are effective by carrying out clinical trials.

16 | The test shows the presence of antibodies against yeast. Does it mean that the client has a Candidainfection?

The test is not a test for Candida or Candida antibodies. People who react to yeast are however advised to cutdown on foods that contain yeasts and also foods that can stimulate yeast and Candida overgrowth, such as sugary foods.

17 | Does a diet to lose weight work based on the IgG results?When your body has an immune response it acts as if it is “under siege” hoarding supplies and carrying weight andfluid. Removing foods that have caused an IgG reaction have been shown in a clinical study to help people to loseweight, feel better and improve body composition.

18 | The Lorisian test is carried out in a laboratory. What accreditations does the laboratory have?

Lorisian has certification from the MHRA (the Medical Health Authority in the UK). The laboratory tests and products meet all the requirements of the European Medical Device and In Vitro Diagnostic Device Directives.

19 | Can those positive to cow’s milk use lactose free milk?No, the lactose is not the problem it is the proteins that are present in cow’s milk that cause the intolerance, theseare still present in lactose free milk. Alternatives such as soya, coconut, almond, hazelnut, rice, or oat milk should be used.

20 | Can those positive to cows’ milk use goats’ or sheep’s milk?

It is possible to react to the proteins in cows’ milk and not those found in goats’ or sheep’s milks. The Lorisian150plus test includes these three ingredients to help give a fuller understanding of reactions to animal milks andhelp make appropriate dietary changes.

21 | Can those intolerant to hen’s eggs eat duck, quail or goose eggs?

It is probably best to avoid all eggs.

22 | Is gluten-free the same as wheat-free? A product can be wheat-free and not gluten-free. A product can be gluten-free and not wheat-free. There are however, products which are both gluten-free and wheat-free. We advise clients to read labels carefully.

Many people wish to start re-introducing ingredients into their diet following a period of exclusion. This must be done with care and slowly. Introduce one food or drink ingredient at a time and leave one week between the introduction of that ingredient and the next. If there are no symptoms after a few days then use that ingredient on a four day rotation to maintain increased tolerance. If at any time your symptoms start to return or you start to feel ill DO NOT continue with the re-introduction of that ingredient.
