VISITORS’ REGISTERA register book is available for a small charge for visitorsto sign both their name and address. This book provides areference when acknowledging flowers, memorialcontributions or special favors. It also contains funeral servicedata, music selections, pallbearers, family data, memorial andfloral offerings, as well as a section for newspaper notices.

CELEBRANTThere is a celebrant/grief specialist on staff to assist your familywith any needs you may have during and after services. Weoffer grief classes as well as remembrance services throughoutthe year.

PALLBEARERSNormally, six pallbearers are required for each service. Anynumber of Honorary Pallbearers may be selected by thefamily. We suggest that you select the pallbearers and give ustheir names the day before the funeral. We ask that they arriveat the place of the funeral approximately 30 minutes prior tothe time of service. At this time, we will give them instructionsas to their duties. Usually, the pallbearers drive in their owncars unless the family so chooses to order a limousine for thatpurpose.

LIMOUSINESWe include, as part of our service, a limousine to be used bythe family the day of the service. If the family prefersadditional limousines, we will make the necessaryarrangements.

DEATH CERTIFICATESAt the time of making arrangements for the deceased, we askfor vital statistics such as legal name, birth date, city, state, andcountry of birth, social security number, father’s name,mother’s maiden name, etc. This information is transcribedon an original death certificate which is certified by a doctoror medical examiner as to the cause of death. This certificateis then filed with the registrar in the city or county where deathoccurred.

We recommend that you order the maximum number ofcertificates you might need. Certified copies of the deathcertificate are required by financial institutions, insurancecompanies, pension plans, unions, social securityadministration, veterans’ administration, secretary of stateoffices for automobile title transfers and for registering thedeath with the county register of deeds.

DEATH NOTICESAs part of our services, we will arrange to place a death noticein any of the local or out-of-town newspapers. The charge forsuch notice will be billed directly to the family by thenewspaper used. If so requested by the family, we will notifynewspapers of obituary notices which appear at the discretionof the newspaper editor.

SOCIAL SECURITY BENEFITSThe lump-sum death benefit is available only for survivingspouse or eligible dependent children. The deceased musthave paid into Social Security for the surviving spouse tobe eligible for benefits. A Social Security letter, along witha phone number, is provided for the family after theservices. A member of the family should contact the SocialSecurity Office to provide any information needed.The local office must be notified of the death by the funeralhome and we do this by filing a “Funeral DirectorsStatement of Death” with the appropriate office. If thedeceased was receiving a disability or retirement check, heor she is not entitled to it for the month in which deathoccurred.

PRE-PLANNINGOur staff includes funeral pre-planning specialists. Call foran appointment so that all your questions can be answered.Pre-Planning is an important part of arranging ameaningful funeral service. There are also emotional andfinancial benefits to having services arranged in advance.

MEMORIAL CONTRIBUTIONSAt the time of arranging the details for your loved one’sfuneral, you may select a memorial fund to which friends andrelatives may choose to donate. We are supplied withappropriate cards and envelopes for most organizations.At the conclusion of the service, the cards and envelopes willbe placed in a case and given to the family.

CEMETERY CHARGESA cemetery lot may have been arranged for in advance ofneed. If it is necessary to purchase cemetery lot or cryptspace at the time of need, it is best to do so as soon aspossible after the death. In either case, local cemeteriesrequire an opening and closing charge to be paid at thetime of the funeral. Usually the grave or crypt space whichis being used must be paid in full.

CEMETERY MARKERS AND MONUMENTSA cemetery memorial is an important part of our service.We feature fine monuments, and other quality granite andbronze markers of which we have samples in ourshowroom. Each cemetery has its own regulationspertaining to the size and type of marker or monumentthat may be installed. We will be happy to assist you inselecting an appropriate marker.

ACKNOWLEDGEMENT CARDSWe have acknowledgement cards available if the family sochooses to purchase them. There are different styles fromwhich to choose. Proper etiquette dictates that you send anacknowledgement card to those who send flowers,contributions for memorials, masses, etc. and to those whodo a special favor such as bring food to the house, meetfriends or relatives at the airport or any other specialerrand for the family. We also suggest you send a thankyou to the pallbearers and any fraternal order, i.e., MasonicLodge, K.of C., Elks, American Legion, etc. whichconducted a memorial service. It is not necessary to sendacknowledgement cards to those who only pay theirrespects at the funeral home or send sympathy cards.An appropriate time to mail the cards is a week or twoweeks after the service. If the sentiment on the card endswith “By the Family of”, the first name, middle initial andlast name of the deceased should be signed directly below.

INSURANCESWe are prepared to assist you in notifying the life insurancecompanies of the death. All companies require a certifiedcopy of the death certificate along with surrendering thelife insurance policy and completing a death benefit claimform.
