SUMMARY INFORMATION RELATING TO THIS POSITION

POST: CONSULTANT IN GENERAL ADULT PSYCHIATRY (ESTEEM) – 8PA’S

BASE: STOBHILL HOSPITAL

Applications are invited for a 0.8 wte Consultant Psychiatrist for the Early Intervention Psychosis Service (Esteem) within North West Glasgow. North West Glasgow has mixed deprivation, with a mixture of deprived inner city areas and affluent suburbs. Inpatients are currently admitted to Gartnavel Royal Hospital.

NW Esteem and the successful applicant would have a team base at Stobhill Hospital shared with the NE team. The consultant works closely with the multi-disciplinary team and sector Community Mental Health Teams, Social Work and Voluntary Organisations to provide a comprehensive service, including inpatient and outpatient treatment. They will participate in the on-call rota for North Glasgow and cover for colleagues during absences in line with arrangements set out in national Terms & Conditions. It is expected that the successful applicant would be an Approved Medical Practitioner in terms of the Mental Health (Care & Treatment)(Scotland) Act 2003 by the time of appointment. Esteem is a well-established model in Glasgow. It targets all new presentation of psychotic illness and provides continued input for the first 2 years. The aim is to provide maximum functional recovery during this period and improve longer-term outcomes. It is expected that all team members will work closely with carers and provide psychosocial interventions where appropriate. The service model is based on early intervention philosophy and the Glasgow Integrated Care Pathway for Psychosis. The aims are to minimise the duration of untreated psychosis, minimise trauma, promote recovery and functional outcome and provide support to families. It is expected that there will be a 15:1 caseload for key workers and that members of staff provide an active occupational therapy programme and psychosocial interventions, including family work and cognitive behavioural therapy.

Job Description

CONSULTANT GENERAL ADULT PSYCHIATRIST EARLY INTERVENTION IN PSYCHOSIS SERVICE (ESTEEM) – NORTH WEST

GLASGOW

Applications are invited from a dynamic and enthusiastic individual for this exciting 8 session General Adult post. The successful applicant would be expected to lead a multidisciplinary team providing an intensive service to 16-35 year olds with first episode psychosis. The successful applicant will be supported by a multidisciplinary team, including Clinical Psychology sessions, 1 Support Staff and 5 Community Psychiatric Nurses/Occupational Therapists or equivalent. It is expected that key worker caseload will be around 15 patients. The Service will look after patients for first 2 years after diagnosis. The in-patient beds are based in Gartnavel Royal Hospital and the community base is located in Stobhill Hospital. The successful applicant will be expected to work closely with colleagues to provide a full range of services to inpatients and outpatients suffering from their first psychotic episode.

1. Current Psychiatric Services – West Glasgow Area

The current post is based in the North West locality of the Glasgow City Health and Social Care Partnership (HSCP) and the Milngavie and Bearsden area within the East Dunbartonshire HSCP. These are 2 of 6 HSCP across the NHS Greater Glasgow and Clyde (NHS GG&C) Health Board Area which have recently been established to provide health and care services to the population. The Head of Adult Services responsible for mental health services in both localities is Mr Colin McCormack and the Clinical Director in Psychiatry is Dr David Gerber.

The Head Of Service for ESTEEM is Mrs Katrina Phillips and the Clinical Director for ESTEEM is Dr Alex Thom.The Associate Medical Director for Mental Health is Dr Michael Smith.

The North West locality serves a population of around 210,000 based on 2001 census data, with an additional 60,000 at the Lomond area. The weighted population for the Arndale Community Mental Health Team is approximately 54,000 people aged between 18 and 65 years. The Bearsden and Milngavie areas of the East

Dunbartonshire HSCP account for approximately 20,000 adults of this total catchment population. There are established links with both local authority areas with good joint working arrangements. Community services in Greater Glasgow are based in Resource Centres. There are five adult resource centres in the North West locality and West Dunbartonshire: Riverview, Arndale, Goldenhill, Shawpark and Riverside, with an additional team in Helensburgh. The staff within these services constitute the North West Division of Psychiatry. Community services are well established in NHS Greater Glasgow and Clyde and based around community mental health teams. These services, in recent years, have been enhanced by the development of Crisis Services and Primary Care Mental Health Teams.

In-Patient services are based at Gartnavel Royal Hospital. This is a modern facility constructed in 2007, which provides state of the art accommodation for service users. There are 6 wards; 3 acute adult wards, 1 IPCU ward and 2 older adult wards. There are 117 acute beds provided in modern single rooms all with en-suite facilities. In addition there is a rehabilitation service, an Addictions ward and day Unit and a Learning Disability In-Patient facility on the campus. The in-patient service is actively supported by Occupational Therapy Services, Dietetics, Physiotherapy, Advocacy and a well established patient activity service. There is a strong ethos of positive engagement with third sector organisations and service users.

There is an established Liaison Psychiatry service across the Health Board area. Patients in NHS GG&C have access to specialist Psychology, Psychotherapy and Addiction services. The NHS GG&C Perinatal Service, based at Leverndale hospital, provides board wide services and the ESTEEM service provides Early Intervention Psychosis services. There is also a well structured Forensic Psychiatry service providing community, in-patient and Medium Secure services to the Board area. The Community Eating Disorder Service is another Board wide service based in North East Glasgow with admission beds at Stobhill Hospital. There is also a dedicated Out-Of-Hours Service across the city providing psychiatric nursing assessment and support from 8pm-9am.

The service model is based on early intervention philosophy and the Glasgow Integrated Care Pathway for Psychosis. The aims are to minimise the duration of untreated psychosis, minimise trauma, promote recovery and functional outcome and provide support to families. It would be expected that Esteem key workers would assertively follow-up patients in the community and also provide in-reach to inpatients visiting them at least weekly, even when boarding in other hospitals. Esteem care and treatment may be augmented by the Crisis Service, which provides intensive support in order to prevent admission and facilitate discharge. The consultant would be responsible for inpatients and working as part of a broader consultant team.

Patients would not be excluded on grounds of drug use, unless the psychotic symptoms were clearly due to acute intoxication with a drug. There would be discussions with the Perinatal Service to decide which service would most appropriately manage patients, but it would be expected that most patients admitted to the Perinatal Unit would be followed up by the Perinatal team for a year and then transferred directly back to the local CMHT.

Esteem will provide a rapid assessment and it is expected that most patients will be assessed within 2 working days of receipt of referral. GP referrals will usually be made to the local adult CMHT or adolescent team, who would co-ordinate the initial assessment. Wherever possible, assessments will be joint with the locality Community Mental Health/Adolescent Team to provide continuity of care if the patient is not taken on by the Esteem. If the Community Mental Health Team/Adolescent Team are unable to provide staff for the assessment within this period of time, the person will then be assessed by 2 Esteem staff. Responsibility will remain with the local CMHT/Adolescent Team until it is agreed that the Esteem will take over the patient’s care. If the person is accepted by Esteem, it would be expected that their family would be seen within 5 working days. Wherever possible, the key-worker undertaking the initial assessment will provide ongoing care so as to provide continuity. It would be expected that medical assessment would happen within 5 working days of the initial assessment.

The service will involve patients in their care, provide information and choices, and deliver home-based care. These strategies are to empower the individual and minimise trauma. It is expected that the service will actively search for co-morbidity and provide a wide range of psychosocial interventions to promote recovery. Esteem will provide support and information to families, both individually and group sessions. It is expected that patients will be provided with a high level of input during recovery to allow work on the underlying vulnerability to psychosis. The team aims to effectively manage risk through collaboration and knowledge of the patient and their family with the aim of avoiding admission and minimising the use of the Mental Health (Care & Treatment)(Scotland) Act 2003.

It is expected that the medical component of care should be the responsibility of the Consultant Psychiatrist and this should be reflected within the ICP review process. The review process will also involve the families wherever possible. Sharing of information between the team, the young person, their family and their General Practitioner should be given a high priority.

The service has a strong emphasis on multidisciplinary working. All care will be based on multidisciplinary formulation with expert input from medical, psychological, occupational therapy and nursing staff shared with the patient and his family and determining the treatment plan. It is expected that the team will be able to provide social and psychological interventions, particularly to deal with problems caused by social isolation, trauma, anxiety and the misuse of drugs and alcohol.

Esteem services are expected to influence practice out with the service. This would be demonstrated by offering early intervention awareness sessions to Primary Care, Community Mental Health, Adolescent Mental Health Service and the Inpatient Services. It is also

expected that the service will link with Social Work Services when these are not integral to Community Mental Health Teams. Strong links will also have to be developed with Housing, Education and Employment Agencies, as well as other youth-orientated services and organisations.

Teaching/Administration

The post holder will undertake the administrative duties associated with the care of patients and the proper function of the clinical team including appraisal and non-clinical supervision of medical staff and clinical supervision of non-medical staff. He or she will also be expected to participate in the broader aspect of administration and management within the North East Glasgow Division of Psychiatry.

It is expected that the successful candidate will take part in Job Planning, Appraisal, Peer Review and Continuing Professional Development. Participation in audit or research is encouraged. The post holder will be actively involved in the teaching of undergraduate medical students. It is anticipated, in recognition of this, that Glasgow University will grant the status of Honorary Clinical Senior Lecturer in Psychological Medicine. In addition the post holder will be expected to participate in the postgraduate teaching of medical staff and staff of other disciplines.

TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.
