SBAR: Sepsis Reporting Measures to CMS

S: Reporting of metrics to CMS on Severe Sepsis and Septic Shock went live October 1, 2015

B: Across the Nation, Sepsis is a major cause of Morbidity and Mortality

A: Due to ongoing analysis of the measures and the specific requirements, several areas were identified as needing improvements

R:

1. Antibiotics: Need to be given within three hours of arrival. Broad-spectrum antibiotics are acceptable monotherapy. However, if Vancomycin is ordered, a 2nd antibiotic will need to be given within the first three hours of presentation. Vancomycin is not an acceptable monotherapy in the treatment of severe sepsis and septic shock. It is recommended to give the broad-spectrum antibiotic first followed by Vancomycin.

2. Sepsis exam documentation requires the provider to note the skin color; “normal” is not acceptable, the peripheral pulses and the capillary refill (typically, < 3 seconds or > 3 seconds). We have created a smartext for your convenience to fulfill this documentation requirement. Please incorporate this smart text into your H & P: @SEPSISEXAM@. This needs to be completed within 6 hours of arrival.
