Lil Charmer of the Year will berecognized at the Utah HeadStart Association 2010 - 2011conference which will be heldNovember 2, 2012 in Sandy, UT.
Award Criteria
Note: Failure to meet any of thecriteria below will result inautomatic elimination.
1. Applicant must be the parent orguardian of a child who attendedHead Start or Early Head Start duringthe 2011 - 2012 school year..2. The local program must be acurrent member ofUHSA.
Requirements
1. Include an essay of not more than700 words outlining how this child madeprogress related to his/her experience inHead Start/Early Head Start. This maybe typed or handwritten.2. Include a photo of the child3. Include a letter of support from a staffmember (can be teacher, aide, busdriver, home visitor) that worked directlywith your child.
Ranking
Applications will be ranked on thefollowing:60 points- Essay on how this child hasbenefited from their Head Start/EarlyHead Start experience40 points- Letter from staff member
Applications are due to your local Head Start program by _______________
Winners from each program will be forwarded to the Utah Head Start Association by May 25.
