When Steve Maggid, MD, obstetrics and gynecology, Holy Cross Hospital, found a lump in Lisa Reich’s breastduring a routine check-up, his patient wasn’t worried. The Silver Spring resident had lived with fibrocysticbreasts, a harmless condition, since her early 20s. Plus, twobiopsies over the years ruled out anything more serious.

But she was a little surprised when her doctor of nearlya decade—who had delivered her baby at Holy CrossHospital and was so familiar with her history—insisted sheget a sonogram in addition to her regular mammogram.

Thankfully, she did. The mammogram, like others before it, was negative.

The sonogram, however, picked up something suspicious,prompting Dr. Maggid to refer Lisa to Eric Oristian, MD,medical director of the Breast Center at Holy Cross Hospital, for a third biopsy.

Dr. Oristian saw Lisa within a few days and scheduled a core biopsy, which removes some breast tissue and asample from the lump.

“Even then, I still fully expected the test to be negative,”says the busy mother and marketing professional. “Butthen Dr. Oristian introduced me to a specialized oncologynurse—a nurse navigator from the hospital’s Breast Center.And it finally dawned on me that, this time, I might be looking at something more serious.”

She was right.

CANCER CARE EXPERTSBack at Holy Cross Hospital, Dr. Oristian was busy conferring with other specialists to decide the best way totreat Lisa’s now-confirmed cancer—standard operating procedure for the hospital’s Breast Center.

“We follow a multidisciplinary team approach—withsurgeons, radiation oncologists, medical oncologists andother specialists reviewing each case—so that patients benefit from the expertise and experience of physiciansfrom different specialties,” he explains.

That approach is one of many positive factors that recently earned the hospital accreditation for excellence bythe American College of Surgeons National AccreditationProgram for Breast Centers. In fact, Holy Cross Hospitalwas the first accredited breast center in our area.

THE LATEST TREATMENT OPTIONSAnother factor in Lisa’s favor was the hospital’s nurse navigator who helps patients understand their diagnosisand treatment options, and serves as an advocate, educatorand guide through the cancer experience. Combined withstate-of-the-art technology, access to clinical trials, a fullrange of services, support groups and other resources,Holy Cross Hospital has emerged as a regional leader incancer care.

For Lisa, that comprehensive care meant a lumpectomy,followed by chemotherapy and then radiation therapy.

In late October 2011, less than two weeks after diagnosis,Lisa underwent a lumpectomy with Dr. Oristian at HolyCross Hospital.

“Right before I went in, both he and Dr. Maggid cameby to visit, which was so kind and reassuring,” she says.

She then started five months of chemotherapy withLinda Burrell, MD, an oncologist at Holy Cross Hospital,followed by 36 radiation treatments. With the exception of her year-long drug regimen, Lisa’s treatment was com-pleted nearly seven months to the day of her diagnosis.

SUPPORTALONG THE WAYLisa has always viewed Holy Cross Hospital as “her hospital,” a sentiment that grew stronger after her good experience as a breast cancer patient.

“Every step of the way, Holy Cross Hospital made mefeel very well taken care of,” she says of the experience. “I never felt like ‘just another patient.’”

Here’s the scenario: You know something’s wrong, but you don’tknow what. Since your teenage years,you’ve had sharp, off-and-on pains inyour lower back that doctors blameon a slight curvature of the spine(scoliosis). But no treatments seem to help and the pain progresses. Bythe time you’re in your late 20s, youcan’t stand for more than 5 to 10 minutes at a time, and have troubleperforming even the most basic activities, including self-care.

That was how Meylinn Martinezfelt before an MRI of her back revealed the real root of her problem: an unusual growth on her lower spinal cord. Her primaryphysician immediately referred her to Zachary Levine, MD, medicaldirector of neurosurgery for HolyCross Hospital, who refined the diagnosis even further.

AN EXTREMELY RARE CYST“Through neuro-imaging techniquesand a biopsy, we discovered Meylinn,then 29, had an extremely rare cyst,”he explains. “It was a type of tumorthat arises before birth, but she probably wasn’t aware of it until itstarted growing and giving her problems.”

While they are not cancerous, andthus not dangerous in and of them-selves, cysts like Meylinn’s can wreakhavoc if they lodge in the wrong spot.Meylinn’s was pressing upon vitalnerves, causing unrelenting pain inher back and waist. She needed surgery as soon as possible.

A DIFFICULT AND DELICATE SURGERYThe location of Meylinn’s cyst, how-ever, made surgery precarious. Onemisstep in the operating room couldhave affected her bowels, bladder orlegs, leaving her incontinent or evenparalyzed from the waist down.

Fortunately, Dr. Levine is experienced with the most difficultneurosurgeries and had the expertise,state-of-the-art technology and other

resources to tackle Meylinn’s tumor(see box below).

“This type of cyst is so unusualthat, in my entire career, I had onlyoperated on two patients with thesame condition before Meylinn,” hesays. “And their cysts were in thebrain, not spine, so it’s a different surgery.”

her spinal cord to protect criticalnerves from damage. The latterhelped guide Dr. Levine’s approachand preserved her ability to feel andfunction.

During the three-hour procedure,Dr. Levine first removed bone fromthe spinal column so he could reachthe spinal cord. Next, he opened upthe protective sheath covering thespinal cord and then used a special-ized piece of equipment to carve the cyst off.

Only five days later, Meylinn went home.

NO COMPARISONToday, Meylinn is walking on her own, and once again able to take care of herself. While she still receives physical therapy, she credits Dr. Levine and Holy Cross Hospital for “allowing me tohave a better quality of life.”

“After my experience, I can attestthat Holy Cross Hospital can take care of the most high-risk surgerieslike mine,” she says. “My cyst was in a very critical position, and Dr. Levine and his staff were able to remove it safely. He’s an extraordinary neurosurgeon.”
