The Community Health Worker serves as a valuable resource to the other network members (primary care and hospital). Patient discharged from hospital with specific instructions. CHW assures compliance.Chronic non-compliant clinic patient. CHW counsels and educate patient on behalf of primary care provider.

The Community Health Worker visit patients in their homes at the request of primary care providers or hospitals who are in the integrated network. These visits often prevent the need for more costly interventions such as emergency room treatments or hospital admissions/readmissions.
