OSU Extension has partnered with TAP (Training Achievement Program) Series to offer an on-line version of Level 2 Food Safety training which meets all ODH requirements. You can complete the online course at your own speed and at a time that works for you. Once training is completed, you will take a proctored certification exam at Crawford County Public Health for addition fee of $60. See reverse side of the exam information. ServSafe® Level 2 Online Food Safety Training

Food Safety Level 2 online training is being offered by Ohio State University Extension in partnership with TAP Series. This program meets Ohio Department of Health requirements for Level 2 Certification in Food Protection. Registration fee of $100 (non-refundable) for the online class. The Exam fee of $60 is paid when you register for the exam with the Crawford County Public Health. The ServSafe Manager exam is proctored by Crawford County Public Health Department who is a Certified ServSafe® Proctor. How to Register: 1. and sending the following information to OSU Extension, Crawford County, 112 East Mansfield Street, Bucyrus, OH 44820.

2. Each individual registered will receive 2 email messages (one from TAP and one from OSUE) with username/password and additional class information allowing access to the online class.

3. Student takes the online class (about 2 -4 weeks) and prints the Certificate of Completion/Record of Training.

4. Before or on the exam date, student brings (mails, emails, or delivers prior to taking exam) a copy of the Certificate/Record Of Training indicating successful completion of the online TAP course to Crawford County Public Health.

5. Student takes the ServSafe Manager Exam on pre-selected date at the Crawford County Public Health

Department, 1520 Isaac Beal Rd, Bucyrus, OH 44820. This exam will be proctored by CCPH staff who will submit the exam answer sheet to the National Restaurant Association to be graded.

6. When the student passes the exam, a ServSafe certificate can be printed at www.ServSafe.com 7. Upon passing the exam, student must complete an “Application for Reciprocity of Ohio Department of

Health’s Certification in Food Protection” form found at: http://go.osu.edu/reciprocity_level2foodsafety

• On this form, supply name of course (TAP) & hours spent on course material (minimum is 15hrs). • Supply copy of certificate from ServSafe showing successful completion of exam.
