September 9 , 2014 Dear Parents and Guardians: My name is Beverly Cortez and I am a teacher candidate in the College of Education at the University of Hawaii at Manoa. I am currently working towards my Bachelor’s degree in Elementary Education. Every Monday and Tuesday, I will be working alongside Mrs. Lakalo and her class at Barbers Point Elementary School. Throughout my visits, I will observe, assist and sometimes lead the class in different activities and lessons. These activities and lessons will be monitored and approved by Mrs. Lakalo, to assure that it is beneficial to the students and also meets the standards. To become a more effective teacher, my goals are to increase my confidence in teaching in front of a large group of students, learn time management when implementing different lessons, and learn how to differentiate instruction from Mrs. Lakalo. I have chosen to become a teacher because of my love for working with children driven by the effects that my previous teachers have made in my life throughout my schooling. I want to be able to make a positive impact on my students and have them carry it throughout their entire life. I would like for you to complete the attached Parent or Guardian Media Release form. The College of Education at the University of Hawaii at Manoa requires all teacher candidates to include sample work of elementary students as proof of our academic and professional growth. I would like to request your approval to use your child’s work in my course work, professional digital portfolio, and possibly be shared with other professionals in the field of education. Your child’s image and name will be kept confidential and not be identifiable. Please read and sign the second page and return it to Mrs. Lakalo by September 1 5, 2014.
