We’re excited to honor the connecons that promote quality health care for all. At Alameda Health System (AHS), we live the promise of community health every day, no maer the polical climate. For over 150 years, we’ve connected people to comprehensive health care, and we’re here to stay. Widely known for our capacity to treat the most traumac and life-threatening emergencies, AHS is pleased to announce that Highland was recognized as the East Bay’s first and only Adult Level 1 Trauma Center. As important as this is, we’re even more excited about our growing efforts to advance health equity and overall populaon health in partnership with the diverse neighbor- hoods across Alameda County. You can help. We all benefit from the joint impact of public-private partnerships. Your support of Connect will help enhance pro- grams like Health Advocates that go beyond addressing an illness or injury to help our paents achieve long-term wellness. The Health Advocates volunteer program was created by AHS medical providers and social workers who noced many of our paents would leave the hospital in good condion only to return later with more health issues. Our care team knew that daily economic and social challenges were impeding our paents’ ability to stay well. Health Advocates leverages the skills of community volunteers to assess paent needs for food, housing, transportaon and other public benefits. Health Advocates then connect paents to the vital community resources that they need to stay healthy. This is the power of a collaborave, community effort. Imagine the collecve impact we will have in fulfilling the AHS mission of Caring, Healing, Teaching, Serving All! Thank you for your consideraon. We look forward to your parcipaon at Connect! Presented by Traditions Behavioral Health, this year’s event — Connect — celebrates the partnerships that unite patients, families, health care teams and community services throughout our comprehensive network of hospitals and wellness centers.

We’re excited to honor the connections that promote quality health care for all. At Alameda Health System (AHS), we live the promise of community health every day, no matter the political climate. For over 150 years, we’ve connected people to comprehensive health care, and we’re here to stay.

The Health Advocates volunteer program was created by AHS medical providers and social workers who noticed many of our patients would leave the hospital in good condition only to return later with more health issues.
