on 3/02/93, a review identified a discrepancy in the QualityAssurance (QA) Categorization of the solenoid valve,SOV-CH-101B, used to control the cooling water flow to theLubricating (Lube) Oil Cooler for the IB Charging Pump(CH-P-1B). The SOV was identified as QA Category 2 (QA-2), whenin fact the SOV is used in a QA-1 application. The original SOVwas replaced with a commercial grade SOV in August 1986 as aresult of maintenance activities. The stock number /part numberfor the SOV, in 1986, was listed as QA-2 (commercial grade).Throughout the time period from August 1986 to the present,CH-P-1B was utilized as the running or standby pump. Failure ofthe QA-2 SOV could result in a failure of CH-P-1B due toimproper temperature control of the lube oil cooler. The causefor the discrepancy in the QA Category could not be determined.CH-P-1B was taken out of service on 3/02/93 followingidentification of this condition. The SOV was replaced with aQA-1 SOV and the specifications were revised. CH-P-1B wasreturned to service on 3/13/93. Safety implications as a resultof this event were minimal. A review of surveillance testsperformed showed no indication of improper lube oil cooleroperation.

DESCRIPTION OF EVENT

on 2/24/93, an engineering review identified a potentialdiscrepancy in the Quality Assurance (QA) Categorization of thesolenoid valve, SOV-CH-101B, used to control the cooling waterflow to the Lubricating (Lube) Oil Cooler for the 1B ChargingPump. On 3/02/93, the completed evaluation determined that thesolenoid valve (SOV) was identified as QA Category 2, when infact the solenoid is used in a QA Category 1 application. Theoriginal solenoid valve was replaced with a commercial grade SOVin August 1986 as a result of maintenance activities. At thattime, the component specifications, (ie. stock number /partnumber), for the SOV were listed as QA category 2 (commercialgrade). Throughout the time period from 1986 to the present,the 1B Charging Pump was utilized as the running or standbypump. Failure of the QA Category 2 SOV could theoreticallyresult in a failure of the IB Charging Pump due to impropertemperature control of the lube oil cooler.

CAUSE OF THE EVENT

The original QA Category 1 SOV was installed as part of a designchange for the charging pump lubricating oil system in 1986.Following installation of the design change, the QA requirementsfor the SOV vere not incorporated into station documents. Thecomponent specifications, (ie. stock number /part number), for

I the SOVs on all three charging pumps, as they existed in August| of 1986, were listed as QA Category 2 (commercial grade). Thus

the original QA Category 1 solenoid valve on the 1B chargingpump was inappropriately replaced with a commercial grade SOV inAugust 1986 during maintenance activities.

CORRECTIVE ACTIONS,

I

The following corrective actions have been or will be taken as al result of this event:

1. The 1B Charging Pump was taken out of service on3/02/93, the SOV was replaced with a QA Category 1 SOV,and the pump was returned to service on 3/13/93.

2. The SOV component specifications for all three chargingpumps were revised to reflect the change in QACategory.

3. The installed solenoid valves on the 1A and 1C Charging- j

Pumps have been verified to be the. correct QA Category 1i classification.

4. The present design change process includes increased,

administrative guidance / procedural controls versus the !process in use in 1986. These increased controls were !instituted to prevent recurrence of this type of event. ;

SAFETY IMPLICATIONS .;

Safety implications were minimal as a result of this event. A j,

review of surveillance tests performed showed no indication of ;,

1 improper lube oil cooler operation. In the event of QA-2 ,

solenoid failure, the most likely failure mechanism would result :in overcooling of the charging pump lubricating oil. An 1

4 - analysis conducted by the vendor determined that overcooling is l

not- a concern. If the QA-2 solenoid were to mechanically bindin the open position, all of the lube oil would bypass the lube. j

oil cooler and overheating could occur, which may result in ;

short term pump failure. Although the IB Charging. Pump was !

functional, the QA. Category 2 solenoid valve could not satisfy !-

' seismic requirements. The solenoid' valve for the lubricating ;

oil system for the charging pumps is not ~ required to be,

Environmentally Qualified. j

REPORTABILITY f!

This written report is being submitted in accordance with |10CFR50.73.a.2.vii.D, as a single condition- that could havecaused at least one independent train to become degraded in asystem designed to mitigate the consequences of an accident.
