You need a bankruptcy trustee to file the required documents with the Office of the Superintendent of Bankruptcy.

Assessment Certificate

This form guarantees that you have met with a trustee, gone over your finances, and understand your legal options and

what effect they will have on your financial future.

Form 65

This form is a Monthly Income and Expense Statement detailing your family size and living expenses as well as your discretionary income. It helps determine

if you will need to make a surplus income payment.

Form 79 – Statement of Affairs

This form lists all your assets and liabilities such as your cash on hand,

personal possessions, vehicles, securities and life insurance policies with a cash value, mortgages, credit

card bills, student loans and tax liabilities.

It is also a legal declaration of your identity and address. Your birth date,

address, and marital status are listed as well as your spouse or partner’s

personal information.

Lastly, you will make a statement about any personal property you disposed of

in the last 1 to 5 years.

The Certificate of Appointment

When your trustee e-files the forms, a Certificate of Appointment is transmitted back. This is your legal confirmation that

your bankruptcy is official.

It also has an Estate Number that proves to your creditors and others that

you are bankrupt.
