    LEFLORE COUNTY HEALTH CENTER OPENS-New Center Offers $10 appointments for Leflore Countys Uninsured-

    Greenwood, MS May 16, 2011 The Leflore County Health Center will celebrate its grand

    opening with a ribbon cutting ceremony and brief open house at 9 a.m. on Thursday, June 2, 2011.This new community-based health center is located at 706 Hwy 82W, Suite A, Greenwood, MS.

    The Leflore County Health Center (LCHC) opens to provide high quality medical services to theuninsured citizens of Leflore County. The new clinic is staffed with certified medical

    professionals committed to providing personalized care in a confidential environment. For a $10administrative fee, patients will receive comprehensive primary care services.

    The opening of this health center was made possible with the support of the Greenwood LefloreHospital, Viking Corporation, Trustmark Bank, Jones Signs, and many other generous donors.Support has also been provided through a partnership with Delta Health Alliance (DHA) initiated

    in November of 2010. DHA is an organization that is committed to increasing access andimproving the quality of health care throughout the Delta. Through the agreement, the LCHCagreed to begin transitioning the clinic from a traditional approach to primary care to a more

    patient-centered approach. Clinical staff, including a nurse practitioner, nurse, patient navigator (social worker) and community health worker, provided through the agreement with DHA, willaddress the needs of the whole person. A registered dietician and clinical pharmacist willsupport the LCHC, along with three additional DHA-affiliated clinics in Humphreys, Sunflower and Washington Counties. Clinic teams are particularly focusing on improving outcomes for chronic illness, specifically diabetes and hypertension.

    The Center is open Mondays and Thursdays from 10 a.m. to 7 p.m., Tuesdays and Wednesdaysfrom 9 a.m. to 5 p.m. and Fridays from 9 a.m. to 3 p.m. Patients are encouraged, but not required,to make appointments in advance in an effort to minimize wait-time. Appointments can be made

    The Leflore County Health Center is a Mississippi-based non-profit health center dedicated toimproving the health outcomes of the people of Leflore County. LCHC strives to be an affordablehealthcare option that increases the ability of an individual to access high quality care, whilesimultaneously educating the citizens of Leflore County on the prevention of disease and illness.LCHC belongs to the people of Leflore County and is run by a board representative of thecommunity and healthcare stakeholders.
