Case ReportAssessment: • Colostomypostdiverticulitiswithelevatedriskofpost-operativeherniaatcolostomysite

Intraoperative Findings: • Moderatesizedparastomalhernia • Extensiveintra-abdominaladhesionsinvolvingtheabdomenandpelvis • Longcolorectalstumpwithresidualdiverticulosis • Contaminationsecondarytopresenceofcolostomy

Surgical Treatment: • Theendcolostomywasclosedanddissectedfreefromtheabdominalwallandreduced intracorporeally.Theparastomalherniasacwasexcisedduringthetakedown. • Intra-abdominaladhesionswerelysed,thedistalcolonandrectummobilizedandpartiallyresected laparoscopically.Usinga33mmendtoendanastomosisstapler,atransanalanastomosiswas performed. • Thecolostomysitewasclosedinalayeredfashionwitharunningsuturetorepairandclosefascial defect(Fig.1). • A4x6cmsheetofDermaPure®waspreparedforimplantationbyfenestratingwitha#15scalpel(Fig.2). • TheanteriorrectalsheathwasreinforcedwithDermaPure®placedinanoverlayfashion(Fig.3),which wasthenreinforcedwithScarpa’sfascia.Thecolostomysitewaspuckeredwithadermalpurse-stringsuture. Outcome / Conclusion: • Threemonthfollowupwithnopost-operativecomplicationsorevidenceofstomasiteherniation • Patientatfullactivitylevelat4weekspost-op • Allpostoppainresolved • Despitecontinuedtobaccouse,surgicalincisionhealedwithoutcomplication • Releasedtofollowupasneeded
