When the entire cost of the Project exceeds $50,000, the School District Board is required by Applicable Law to solicit separate bids for, at least, the following branches of Work:
– General Trades
– Plumbing
– Electrical
– HVAC (heating, ventilating and air conditioning)
School District Board may solicit bids which combine more than one of the branches of Work
Ohio’s Encouraging Diversity, Growth and Equity (“EDGE”) program is applicable
– 5 percent of each Contract award amount
– EDGE is a matter of Bidder responsibility.
Method of Bidding

OSFC Requirements
The successful Bidder must submit the items to the School District Board before executing the Contract Form.
– Bond and current/signed Certificate of Compliance or Certificate of Authority
issued by the Ohio Department of Insurance
– Current Ohio Workers' Compensation Certificate;
– Certificate of Insurance
– If the Bidder is a foreign corporation, i.e., not incorporated under the laws of
Ohio, a Certificate of Good Standing from the Secretary of State showing the
right of the Bidder to do business in the State;

– If the Bidder is an individual or partnership, nonresident of the State, the
Bidder must submit a Power of Attorney
– Affidavit of Delinquent Personal Property Tax (Form DPT).
– Enrollment in good standing in the Ohio BWC Drug-Free Workplace Program
– Commitment of EDGE Business Participation
– Declaration Regarding Material Assistance/Nonassistance to a Terrorist
Organization.
– Certificate of Compliance with Affirmative Action Programs
– Plumbing, electrical, hydronics, refrigeration, and heating, ventilating and air
conditioning Contractors must submit proof of current licensing required by
Applicable Law.8
Contractor Qualifications
Form 26 – Subcontractor’s Declaration;
Form 27 – Manufacturer’s Declaration;
Form 28 – EDGE Declaration of participation
References:
Projects of similar size and type.
Minimum of four (4) references
Each reference to include contact and
phone number for Owner, Architect,
Construction Manager/Contractor, and
Contractor/Subcontractor.

Resume of proposed staff
Projects staff are currently working on
EEO violations over past 3 years. (EEO Certificate)
EPA violations over past 3 years.
OSHA violations over past 3 years.
Any pending claims or lawsuits.
A detailed estimate for contractor reference during post bid interview.
BWC & DFWP Certificate.
For each Subcontractor with an estimated contract value of $100,000+ provide required docs per Special Conditions
