Keep safety records of inspections and safety meetings held at the workplace. Keep records of plant and safety inspections of plant and equipment used at the workplace. Carry out risk assessments and review SWMSs for hazardous works Evaluate information on hazardous substances and keep register of MSDSs Provide and facilitate safety training such as Company Induction, safe work method instruction, toolbox talks, manual handling training. Includes all relevant persons when making decisions regarding site safety. Supervisors: Verify identified risks and raise suitable hazard control measures Raise any safety needs of the Company, ensuring that company safety procedures are workable for its employees at the workplace. Sub-contractors: Make sure all employees have the industry induction green card and have been instructed in the job safety analysis as submitted by the sub-contractor Provide a safety management plan to the principal contractor on how safe work procedures will be implemented Provide verification that employees have been issued with and trained in the correct use of appropriate PPE for tasks to be performed Follow any safety instructions given by the principal contractor on site safety Maintain registers or logbooks on electrical plant or other specific hazardous plant as required Supply information on the use of substances and keep a MSDS register Identify a responsibility and communication procedure Cooperate with workplace safety requirements and safety procedures Monitor safe systems of work on a day-to-day basis Carry out hazard identification safety inspections as a routine activity Provide adequate instruction to employees to facilitate safe work Make sure employees are aware of safety hazards Facilitate the implementation of safe work methods Make sure there are adequate resources to carry out work safely Include safety in planning work activities

d) Ketentuan apa yang dibuat perusahaan Anda untuk rapat-rapat komunikasi HSE?

Regular safety meetings, toolbox and line safety meetings at worksite play a significant part in the communication process. Such meetings shall take place informally and within a structured framework. Everyone is required to participate in the process.

A Safety meeting is held for all employees at PT. Aldaberta Indonesia (AI),(frequency of meetings will be appropriate to the location )to brief out any accidents, incidents or near misses. Any issues raised are also discussed to communicate out lessons learned.

A monthly management meeting is held and within this HSE matters are discussed.

Toolbox talks:A toolbox talk is used to communicate OHSE matters to the workers. Aldaberta will hold weekly toolbox talks or when required if there are specific issues to be communicated and submit to the client.

Supervisors usually hold toolbox talks although an external provider such as the OHS consultant may convene a toolbox talk.

Toolbox Talks may include:

Injury close off Safety inspection findings Changes to work practices Development of safe work methods Communicating incident investigation findings Changes to affected legislation

Pre-start Talks:Pre-start talks will be held daily and involve the tasks as described in the workgroups SWMS. It will capture areas of risks involved for the days work and make all workers aware of controls.

Supervisors hold the pre-start talks before work commences for the day.

Talks include: Risks or issues raised from the previous shift Reiteration of high risk task controls Allocation of roles and responsibilities Ensuring checks are carried out

3.3. Kemampuan dan Pelatihan HSE Umum

All new employees of PT. Aldaberta Indonesia (AI) are required to undertake training in Health, Safety and environment and to formally acknowledge their understanding of the safety requirements of the Company which is achieved through PT. Aldaberta Indonesia (AI) regular weekly meetings and risk assessment awareness.

A formal training program of safety subjects has been introduced and is carried out in house.

Formal training in specialized areas is provided through external sources where and when required and control maintained over the need for refresher training as appropriate.

The publication of safety alerts and promotional posters and displays further enhances training.

The Company makes the following commitment to the training of its employees and will ensure that any Sub Contractors of the Company will maintain the same. These requirements include:General Identifying the OHSE training needs of management, supervisors and workers Ensuring that all personnel undergo induction and necessary task training to conform with OHS standards and relevant legislation Maintaining appropriate records of OHSE training Providing refresher courses of OHSE contentManagement Managers and supervisors will participate in OHSE training, specifically Risk Management and Hazard Control. Positive behaviors of all people involved are critical to the success and achievement of HSE goals. It is therefore essential that all personnel are carefully selected and trained, and their skills and competencies are regularly assessed.

Line management is responsible for providing appropriate training, equipments and procedures necessary for employees to perform assigned tasks safely. All personnel employee are responsible for the safety of themselves and others who may be affected by their work activities.Training

Qualifications, Training and Competency RequirementsPrior to commencing any site work, all employees who would be visiting work sites shall successfully complete essential training programs to ensure their competency and safe performance of duties. The training programs shall include general, task-specific and HSE training. The training programs, which will include:

A specification for qualifications, competencies and training requirements for key personnel; HSE Induction for all employees; A comprehensive induction training program Matrix of training requirements covering general task-specific and HSE-related training, training frequencies and intervals between refresher courses.According to the HSE induction, all personnel entering work sites shall have their relevant competency certification with them. Only persons who are in possession of such certification shall be permitted to work.
