As we kick off the 2014-15 school year at La Crescent-Hokah Public Schools, we invite you to get involved with some of the many amazing opportunities we have for students, parents, and community members alike. Whether it is volunteering in the classroom, participating in or spectating at extra-curricular programs, or enrolling in a Community Education class, our district has something to offer to all of our community members! I encourage you to take a moment to review this booklet, as it contains important re-sources including the application for the Free and Reduced Meal Program, general Food Service information, volunteering information, parental rights to their student’s educa-tional record, other required District notifications, and other upcoming opportunities.
As stated in our Mission, the La Crescent-Hokah Public School District is committed to prepare all learners to thrive in an ever-changing, global society. This commitment is to learners from birth through senior citizens. The District recently convened a group of stakeholders from across the community to complete a strategic planning process in support of its Mission. That process led to the development of the following three Bold Steps from which to prioritize work in the year ahead:
• Assess student needs (academic, behavioral, social, emotional) and implement plans to meet identified needs.• Pass a bond referendum to address facility needs. • Implement a positive communication and marketing plan.

Three areas of focus for the year in support of the District’s Bold Steps include the advancement of the District’s LancerLearningHD 1:1 technology integration initiative, the School Board’s work to bring a facilities bond referendum to the voters, and the work of the District’s Marketing and Outreach Committee to develop and improve communication and marketing across the district.
LancerLearningHD redefines learning through the continued development of innovative and individualized educa-tional experiences that seek to inspire and ensure success for all learners. An integral element of LancerLearningHD includes 1:1 student computing. The teacher, student, and parent feedback after year one of the initiative has been very positive and we look forward to the further expansion of the program as it helps to transform learning. The academic success of all learners is strongly supported through safe and appropriate, quality space for student learning to occur. In addition, our District’s academic and extracurricular offerings, paired with our high level of student performance attracts new families to our communities. After the near decade of work and analysis led by task forces and work groups comprised of community members, administration, school board members and staff, our School Board has taken action to address our aging facilities. Our taxpayers will be called to the polls in January 2015 to vote on a facilities improvement plan that includes the construction of a new elementary school on District-owned Abnet field and updates to some of the oldest sections of the high school building. Please continue to follow our website or attend school board meetings for the latest updates, and feel free to contact any school board member or myself with comments or questions.
In addition to the above, work is underway in the District’s Marketing and Outreach Committee to develop a comprehensive plan from which to guide and support communication and marketing of the wonderful learning opportunities and programs across the District. We encourage your child to eat nutritious school meals. Our meals include low-fat milk, locally grown fruits and vegetables, meat/meat alternates, whole grains and local products. Meals have limits both for fat and calories. New Portion size at Breakfast! Lunch is Healthier than Ever. Breakfast consists of grains and/or meat alternate, fruit, and one cup of low-fat milk. The portion size for fruit has increased to a full cup this year! Students must take at least ½ cup of fruit or vegetable at both breakfast and lunch. Lunches contain a large serving and wide variety of fruits and vegetables including dark green & red/orange vegetables, beans and legumes. There is less sodium and no trans-fat! Milk Break Help your child's learning by providing a milk or juice break every morning. The cost is only $28.00, payable the first week of each semester. This will include one milk or juice per day. No refunds will be given after the beginning of the first semester and the beginning of the second semester. Farm-to-school continues this year so students can enjoy locally grown produce. The “Harvest of the Month” gives students the opportunity to try new fruits and vegetables such as roasted potatoes, parsnips, squash, tomatoes, blueberries, kohlrabi, cranberries, sweet potatoes, spinach, and dried beans. Students will also be able to sample new vegetables with a taste test in the cafeteria before the item is served. Easy System to Receive Free or Reduced-Price Meals It is our goal to make qualifying for free or reduced-price meals as easy as possible. We include an application in every packet we send out; however, many households will automatically qualify without completing an application. This depends on whether you receive assistance: • Don’t fill out an application if you receive Minnesota Family Investment Program (MFIP) or Food Support (SNAP) benefits. You automatically
qualify! We receive your child’s information electronically and don’t need the paper application. • Do apply in any other circumstance. The application takes effect on the day it is received in the school nutrition office. The previous year’s
approval is only valid for the first 30 days of school. After this time, if a new application has not been received, you must pay for any food your child consumes. If your application is incomplete or does not arrive in time, you are responsible for the cost of any meals taken by your child.
We Request Prepayment on All Accounts We regret that if there is not sufficient money in your child’s account we will not be able to provide meal service. Outstanding balances are your responsibility. Meal payments made in advance will be used for breakfast, lunch, or ala carte items. Checks or cash will be accepted, although paying by cash slows down the lines. If a child's account reaches (3) three meals with no payments, the parent will be notified and the student will not be allowed to eat until payment is made. Please direct questions to Deb Prath-Schute at 895-5143. Pay Conveniently On-line with RevTrak Meal payments are accepted through our district’s website via Family Access through RevTrak. From the main district web page select “Lunch Menus/Nutrition” under the Quick Links section on the left side. That will direct you to the nutrition page where there is an option to “’Click here’ to access your student’s account/pay lunch fees.” When utilizing Family Access, you can receive an e-mail notification for account balances and check the status of your student’s account. Dear Parent/Guardian:
Our school provides healthy meals each day. Breakfast costs $1.30 for all students; lunch costs are $2.30 at the elementary school and $2.55 at the middle and high school. All kindergarten students will receive free breakfasts this year.
Your children may qualify for free meals. New this year:
- Students who qualify for reduced-‐price school meals will receive free lunches and breakfasts. - All kindergarten students qualify for free breakfasts.
To apply for free school meals, complete the enclosed Application for Educational Benefits following the instructions. A new application must be submitted each year. If you don’t qualify now, you may apply at any time during the school year. Your application also helps our school qualify for education funds and discounts.
Return your completed Application for Educational Benefits to your school office.
Who can get free school meals? Children in households participating in the Supplemental Nutrition Assistance Program (SNAP), Minnesota Family Investment Program (MFIP) or Food Distribution Program on Indian Reservations (FDPIR), and foster children can get free school meals without reporting household income. Also, children can get free school meals if their household income is within the maximum income shown for their household size on the instructions.
Do foster children qualify for free meals? Yes, foster children who are the legal responsibility of a foster care agency or court are eligible for free meals regardless of household income. Complete an application identifying the children who are in foster care.
I get WIC. Can my children get free meals? Children in households participating in WIC may be eligible for free meals. Please fill out an application.
May I apply if someone in my household is not a U.S. citizen? Yes. You or your children do not have to be U.S. citizens for your children to qualify for free meals.
Who should I include as household members? Include yourself and all other people living in the household, related or not (such as grandparents, other relatives, or friends).
What if my income is not always the same? List the amount that you normally get. If you normally get overtime, include it, but not if you get overtime only sometimes.
How will the information I provide be kept? Information you provide on the form, and your child’s approval for school meal benefits, will be protected as private data. See the back page of the Application for Educational Benefits for more information about how the information is used.
Will the information I give be checked? Yes, and we may also ask you to send written proof.

Children and Foster Status • List all children in the household in Section 2. Check the box if a child is in foster care.
• Include any regular income, for example SSI, to children other than foster children. Do not list occasional earnings like babysitting.
Case Number Complete Section 3 if any household member currently participates in one of the programs listed in that section. If Section 3 is completed, skip Section 4 (adult names and incomes).
Adults / Incomes In section 4, list all adult household members, whether related or not (such as grandparents, other relatives, or friends). For each adult household member, list their gross incomes (not take-home pay) and how often each income is received. Signature The form must be signed by an adult household member in section 6.
Last Four Digits of Social Security Number The person signing the application must provide the last four digits of their Social Security number in Section 6. The Social Security number is not needed if a qualifying case number is provided in Section 3, or all children in the household who need school meal benefits are foster children, or the person signing the application does not have a Social Security number and has indicated this in Section 6.

SCHOOL SECURITYOur schools are equipped with video surveillance equipment to ensure the safety of our students. Video content will be viewed by school district personnel on a random basis and/or when problems have been brought to the attention of the school dis-trict. Video content (on DVD or CD) will be released only in compliance with the Minnesota Government Data Practices Act, Minn. Stat. Ch. 13 and the family. In addition to school surveillance technology, our schools regularly practice lockdown and emergency procedures. Parents are encouraged to visit our schools regularly as participants in their children’s education. ALL VISITORS ARE REQUIRED TO CHECK IN AT OFFICE AND WEAR A VISITOR’S BADGE.
VOLUNTEERING AT SCHOOLLa Crescent-Hokah Public Schools required criminal history background checks on individuals who seek to enter a school or its grounds for the pur-pose of serving as a school volunteer. More specific information on volunteer background checks and criminal history consent forms can be obtained through the individual schools.
COmmUNICATIONOur school district values a strong partnership between home and school to further enhance and ensure the opportunity for your student to succeed in our schools. To that end, communication between home and school is very important. The District website serves as a primary source of information on activities taking place in our schools and classrooms. Teachers, office personnel, and administrators also have individual voice mail and email accounts. In addition, the District’s Skymail System is used to communicate announcements and day to day information via email. Emergency notices, such as school closings, are sent through the District’s Skylert automated communication system. Skylert notifications are delivered through phone calls, texts, and email.
Beyond the communication listed above, each parent is encouraged to visit their child’s school regularly and to be actively in-volved in our schools. If at any point during the school year you have questions or concerns, please feel comfortable in calling the staff member who is involved. It is our goal to work together to provide your child with the best possible learning experi-ences in our schools.
Student tranSportation
Ready Bus Lines, in conjunction with the LaCrescent-Hokah School District, will pro-vide individual student information regarding assigned buses, stops, and times, which will be mailed the week of August 18th. If you have recently had a change of address or are new to the school district and have not received busing information by August 27th, please call Ready Bus Lines at 895-2349.
Our goal is to assure a safe and pleasant busing experience for all students trans-ported. The Ready Bus Company and school district personnel are working together to promote this goal. We ask that students and parents support this effort. To promote school bus safety and to clearly state that riding a school bus is a privilege, not a right, school bus safety training will be provided at age appropriate levels. It is crucial that student behavior on each bus be positive and respectful, with safety being promoted. Students who live within one mile of their school are not eligible for busing unless they must cross a hazardous road, such as a highway, and there is no crossing guard or pa-trol. Students requiring special busing due to their disabilities will be bussed from any distance.

PUBLIC NOTICE REGARDING STUDENT RECORDSIndependent School District No. 300 gives notice to parents of students currently in attendance in the District, and eligible students currently in attendance in the District, of their rights regarding pupil records.
1. Parents and eligible students are hereby informed that they have the following rights: a. That a parent or eligible student has a right to inspect and review the student’s education records within 45 days after the day the request for access is received by the school district. A parent or eligible student should submit to the school district a written request to inspect education records which identify as precisely as possible the record or records he or she wishes to inspect. The parent or eligible student will be notified of the time and place where the records may be inspected; b. That the parent or eligible student has a right to seek amendment of the student’s education records to ensure that those records are not inaccurate, misleading, or otherwise in violation of the student’s privacy rights. A parent or eligible student may ask the school district to amend a record that they believe is inaccurate or misleading. The request shall be in writing, identify the item the parent or eligible student believes to be inaccurate, misleading, or in violation of the privacy rights of the student, shall state the reason for this belief, and shall specify the correction the parent or eligible student wishes the school district to make. The request shall be signed by the parent or eligible stu dent. If the school district decides not to amend the record as requested by the parent or eligible student, the school district will notify the parent or eligible student of the deci sion and advise him or her of the right to a hearing regarding the request for amendment. Additional information regarding the hearing procedures will be provided to the parent or eligible student when notified of the right to a hearing; c. That the parent or eligible student has a right to consent to disclosures of personally identifiable information contained in the student’s education records, except to the extent that federal and state law and the regulations promulgated thereunder authorize disclosures without consent; d. That the school district may disclose education records to other school officials within the school district if the school district has determined they have legitimate educational interests. For purposes of such disclosure, a “school official” is a person employed by the school district as an administrator, supervisor, instructor, or support staff member (in cluding health or medical staff and law enforcement unit personnel) or other employee; a person serving on the school board; a person or company with whom the school district has consulted to perform a specific task (such as an attorney, auditor, medical consultant, therapist, public information officer, or data practices compliance official); or a parent or student serving on an official committee, such as a disciplinary or grievance committee; or any individual assisting a school official in the performance of his or her tasks. A school official has a “legitimate educational interest” if the individual needs to review an education record in order to fulfill his or her professional responsibility and includes, but is not limited to, an interest directly related to classroom instruction, teaching, student achievement and progress, discipline of a student, and student health and welfare and the ability to respond to a request for educational data; e. That the school district forwards education records on request to a school or post-secondary educational institution in which a student seeks or intends to enroll, or is already en rolled, as long as the disclosure is for purposes related to the student’s enrollment, including information about disciplinary action taken as a result of any incident in which the student possessed or used a dangerous weapon, suspension and expulsion information pursuant to section 4155 of the federal No Child Left Behind Act [and data regarding a student’s history of violent behavior,] and any disposition order which adjudicates the student as delinquent for committing an illegal act on school district property and certain other illegal acts; f. That the parent or eligible student has a right to file a complaint with the U.S. Department of Education regarding an alleged failure by the school district to comply with the requirements of 20 U.S.C. § 1232g and the rules promulgated thereunder. The name and address of the office that administers the Family Education Rights and Privacy Act is Family Policy Compliance Office, U.S. Department of Education, 400 Maryland Avenue S.W., Washington, D.C. 202022. Copies of the school board policy and accompanying procedures and regulations are available to parents and students upon written request to the Superintendent.3. Pursuant to applicable law, Independent School District No. 300 gives notice to parents of students currently in attendance in the school district, and eligible students currently in attendance in the school district, of their rights regarding “directory information.” “Directory information” includes the following information relating to a student: the student’s name; address; telephone number; electronic mail address; photograph; date and place of birth; major field of study; dates of attendance; grade level; enrollment status; participation in officially recognized activities and sports; weight and height of members of athletic teams; degrees, honors and awards received; the most recent educational agency or institution attended by the student; and other similar information. “Directory information” also includes the name, address, and telephone number of the student’s parent(s). “Directory informa-tion” does not include a student’s social security number or a student’s identification number (“ID”) if the ID may be used to access education records without use of one or more fac-tors that authenticate the student’s identity such as a personal identification number, password, or other factor known or possessed only by the authorized user. It also does not include identifying information on a student’s religion, race, color, social position, or nationality. a. The information listed above shall be public information which the school district may disclose from the education records of a student or information regarding a parent. b. Should the parent of a student or the student so desire, any or all of the listed information will not be disclosed without the parent’s or eligible student’s prior written consent except to school officials as provided under federal law. c. In order to make any or all of the directory information listed above “private” (I.E., subject to consent prior disclosure), the parent or eligible student must make a written request to the building principal within thirty (30) days after the date of the last publication of this notice. This written request must include the following information: (1) Name of student and parent, as appropriate; (2) Home address; (3) School Presently attended by student; (4) Parent’s legal relationship to student, if applicable; (5) Specific category or categories of directory information which is not to be made public without the parent’s prior written consent. 4. Pursuant to applicable law, Independent School District No. 300 hereby gives notice to parents of students and eligible students in grades 11 and 12 of their rights regarding release of information to military recruiting officers and post-secondary educational institutions. The school district must release the names, addresses, and home telephone numbers of students in grades 11 and 12 to military recruiting officers and post-secondary educational institutions within sixty (60) days after the date of the request. Data released to military recruiting officers under this provision may be used only for the purpose of providing information to students about military service, state and federal veterans’ education benefits, and other career and educational opportunities provided by the military and cannot be further disseminated to any other person except personnel of the recruiting services of the armed forces. Should the parent of a student or the eligible student so desire, any or all of the listed information will not be disclosed to military recruiting officers and post secondary educational institutions without prior consent. In order to refuse the release of this information without prior consent, the parent or eligible student must make a written request to the responsible authority (building principal) by October 1st of each year. This written request must include the following information: (1) Name of student and parent, as appropriate; (2) Home address; (3) Student’s grade level; (4) School presently attended by student; (5) Parent’s legal relationship to student, if applicable; (6) Specific category or categories of information which are not to be released to military recruiting officers and post-secondary educational institutions without prior consent; (7) Specific category or categories of directory information which are not to be released to the public, including military recruiting officers and post-secondary educational institutions.
Notice: Refusal to release the above information to military recruiting officers and post-secondary educational institutions alone does not affect the school district’s release of direc-tory information to the public, including military recruiting officers and post-secondary educational institutions. In order to make any directory information about a student private, the procedures contained in the Directory Information section of this notice also must be followed. If you do not want your child’s or eligible student’s directory information released to military recruiting officers or post-secondary educational institutions, you also must notify the school district that you do not want this directory information released to any mem-ber of the public, including military recruiting officers and post-secondary educational institutions.
AHERA ANNUAL ASBESTOS NOTIfICATION
During the past school year and continuing into the 2014-2015 school year, the following asbestos management plan activities have been or are currently being completed. These activities are pursuant to the Environ-mental Protection Agency 40 CFR Part 763- “Asbestos Containing Materi-als in Schools; Final Rule and Notice.”

LaCrescent-Hokah Public Schools has contracted through the Southeast Service Cooperative with the Institute for Environmental Assessment to provide environmental consulting services. The complete updated Asbestos Management Plan for La Crescent-Hokah Public Schools can be found in the District Office or in the administrative office at each of the school facilities. The Management Plan can be viewed, without cost or restriction, during normal working hours. Copies can be obtained for a fee of $.10 per page. INDOOR AIR QUALITYLaCrescent-Hokah Public Schools have developed an indoor air quality management plan for the District. The management plan addresses policies and procedures that will be used in the schools to address indoor air quality issues. The Indoor Air Quality Committee has approved the management plan and will review this plan annually. PESTICIDE NOTICEAll Minnesota schools are required to inform parents/guardians and school employees that they may request to be notified prior to pesticide application on school prop-erty. Pesticides include chemicals which are used to control insects, weeds, rodents or other pests as defined by the law (M.S. 123B.575, Subd. 9). In order to provide the required information, schools must maintain a list of parents/guardians and school employees who request notification. Schools must also provide information about individual pesticide applications upon request, including the name of the pesticide product, and the time and location of the planned application. An estimated schedule of pesticide applications is available for review or copying at the District Office.
Pesticides and non-chemical treatments may be used in combination in and around schools for a variety of reasons, including the control of pests which have the po-tential to bite, sting, spread disease, cause asthma, and/or trigger an allergic reaction. Pesticides may also be used to prevent or control damage to materials within the school building or the building itself, or to control pests or weeds that are seen as a nuisance.
The long-term health effects on children from the application of such pesticides or the class of chemicals to which they belong may not be fully understood. Children may be more susceptible to pesticides than adults due to their smaller size and rapid growth and development. Also, their playful behavior may expose them to more pesticide residue.To limit the potential pesticide exposure, this school follows safety regulations to ensure pesticides are applied properly. All pesticide products, which are used in and around school buildings, are required to be registered with the Minnesota Department of Agriculture and the U.S. Environmental Protection Agency. In addition, school employees and certified and licensed applicators are required by state and federal law to comply with all requirements of the pesticide label, including building sites, application rates, re-entry intervals, posting, use of personal protective equipment, use restrictions, and disposal on the product label.

PARENT INfORmATION REGARDING SECTION 504Of THE REHABILITATION ACT Of 1973
Section 504 is an Act, which prohibits discrimination against persons with a handicap in any program receiving Federal financial assistance. The Act defines a person with a handicap as anyone who:1. has a mental or physical impairment which substantially limits one or
more major life activities (including caring for one’s self, performing manual tasks, walking, seeing, hearing, speaking, breathing, learning, and working):
2. has a record of such an impairment: or3. is regarded as having such an impairment.In order to fulfill its obligation under Section 504, the LaCrescent-Hokah Public Schools recognizes a responsibility to avoid discrimination in policies and practices regarding its personnel and students. No discrimination against any person with a handicap will knowingly be permitted in any of the programs and practices in the district.
The district has specific responsibilities under the Act, which include the responsibility to identify, evaluate, and if the child is determined to be eligible under Section 504, to afford access to appropriate educational services. If the parent or guardian disagrees with the determination made by the professional staff of the school district, he/she has a right to a hearing with an impartial hearing officer.

BLOODBORNE PATHOGEN COmPLIANCEThe LaCrescent-Hokah School District is consistently developing and implementing policies and procedures to safeguard the health and well-being of our District employees and students to promote a safe educational and working environment. In keeping with this, the District has implemented an Exposure Control Plan to comply with Occupational Safety Health Agency’s (OSHA) Bloodborne Pathogen Standards. The Standard was enacted to “reduce exposure to Hepatitis B virus (HBV), human immunodeficiency virus (HIV) and other bloodborne pathogens” to which employees and students may be exposed in school.
The School District has implemented its Exposure Control Plan to comply with the intent and letter of the law regarding the Bloodborne Pathogen Standard. Our main objective is to protect our employees and students from potential workplace hazards by reducing occupational exposure to HBV, HIV, and other bloodborne pathogens. Debra Best is the District’s Exposure Control Officer. She is responsible for the overall management and support of our program. Questions regarding the Standard or how it may affect you personally can be directed to her attention at 895-4484.
The District has assessed its employees and their job descriptions to determine those persons and job classifications covered by the Standard. In addition, we are provid-ing training, hepatitis B vaccinations and personal protective equipment as is necessary. The District’s Exposure Control Plan and accompanying records are public docu-ments and available for public review (except for information protected by the Data Privacy Act). Copies can be made upon request, provided the District is reimbursed for costs associated with reproduction. Please contact the District’s Exposure Control Officer for specific locations of the Plan. The District supports implementing, where possible and appropriate, self management of injuries. Staff and students will be assisted in managing, treating, cleaning and disposing of contaminated materials themselves, thereby avoiding contact by a second party.
